You see there's a chance of a civilian will give, when you start a mission you'll see how much is the chance. If you have higher then they will thank you

Just play the game, on each mission you have a chance of getting a thumbs up
its random. you got them from revive somebody, transport somebody or when the rescue dog founds someone
