Is it normal for a visa officer to tell you to bring an additional document/correction from your university and after the university provides it, the embassy receives them and return it to you with a refusal? Meanwhile, my other colleague who was refused 3 consecutive times got it approved on the 4th attempt

They will tell you on the basis of their experience. There are questions that are ask as a pattern. If your answers are confident and backed by documents, you will not have any issues
