what could be wrong? wrong/faulty fuel pump, kinked or holed fuel line, faulty fuel pressure valve, wiring issue.....etc

Your fuel pressure is WAY too low. The ECU is probably overadjusting (rich) for the low pressure. Check the fuel pump again
