It is a screening test, to confirm present or past infection the doctors will advice additional tests like chest X ray, sputum for AFB etc. If TB is diagnosed, a 6 or 9 month long treatment (some cases take longer) will start using specific antibiotics, it cures most of the cases but it must be followed strictly, no daily dose can be missed. In many countries the TB medication are provided free in DOTS centers that also ensures patient is taking medication in front of them. Take care and consult with your doctor to start treatment as soon as possible if it is present infection.
Yes, it spreads from person to person via droplets released during coughing, sneezing, talking. Infected person should be kept isolated, use mask when in a room with other people and close contacts should also be screened for TB.

So persons in contact with patients with smear-negative TB are also at risk of infection due to M. tuberculosis and the subsequent development of active TB. The risk is less than the risk seen in sputum positive case, but the bottom line is sputum negative person can spread it too. Also most cases will become sputum negative after 2 weeks of starting anti TB medication. Isolation and wearing mask when other people in the room will prevent TB from spreading.
