  • Dr. Ebraheim’s educational animated video describes clavicle fractures and explains Allman classification in a simple and easy way that helps you understand all you need to know.
    The Allman classification is based on the anatomical location of the fracture. Clavicle fractures are traditionally treated conservatively and there is more evidence that conservative treatment is not ideal for displaced clavicle fractures. Conservative treatment may result in a higher incidence of malunion, nonunion and permanent shoulder dysfunction.
    Three groups in the Allman classification:
    Group I: Fracture involving the middle third of the clavicle. Accounts for approximately 80% of clavicle fractures. As a general rule, when the fracture is less than 100% displaced, then it is treated conservatively with a sling. When it is more than 100% displaced, the fracture is treated with surgery. When there is displacement, the medial fragment is usually elevated and the distal fragment is displaced downward due to gravity and the weight of the arm.
    Group II: Fracture in the lateral third of the clavicle which accounts for about 10-15% of all clavicle fractures. Neer classification is applied to fractures in the lateral third of the clavicle.
    Type I (Neer) - Fracture occurs lateral to the coracoclavicular ligaments, which remain intact to the medial segment.
    Type II - Type IIA (Neer) - Both conoid and trapezoid remain attached to the distal segment. Fracture occurs medial to the intact ligaments. The medial clavicle is unstable and the nonunion rate is high without surgery.
    Type II - Type IIB (Neer) - Fracture may occur either between the ruptured conoid and an intact trapezoid or fracture may occur lateral to both ligaments that are torn. The medial segment is unstable with conoid torn. Nonunion is high without operative treatment. Fracture will require surgery!
    If the fracture occurs lateral to the ligaments the medial clavicle remains stable.
    Type III (Neer) - Extension of the fracture into the AC joint. Intraarticular extension fracture that is stable. The patient may develop post-traumatic arthritis.
    Group III: Fractures involving the medial third of the clavicle. Accounts for 5% of clavicular fractures. Displacement and nonunion is rare. Treatment of this fracture type is usually nonoperative and the fracture is rarely symptomatic.

    Thank you. Do you have a source for when the displacement of Allman group 1 fractures is greater that 100 percent, it should be treated surgically?

      @@madbross9879 How ypu doing now and what you opted for? I had Z type 3 piece fracture on 17th Oct'22. Please advise what is better, conventional way of healing or surgery. I got mixed advise from different doctors.

    I fractured my clavical in 2 spots and the end of my clavical healed 1.5" from the AC joint. My arm hangs lower and I get grinding noise when I lift my arm. I asked the Doctor about the gap between the end of the clavicle and the AC joint and he said thats normal. The way he said it brought up a flag for me. I would like to get it corrected as I enjoy swimming and dont want to deal with cronic tendonitis. Can it be corrected after healing? It healed in a clump of bones.

      @Adrian Chong Perfect no problems at all just my bone sticks out a bit at the site
      of the break. I stopped wearing the sling the hospital gave me and was back on my bike around 3 weeks after break which was a bit naughty of me. WHEN I broke my collar bone I was 8 miles from home so I cycled 8 miles to my nearest hospital then cycled to supermarket and then cycled back home all this with my broken collar bone. then all hell broke loose with the bruising and pain which was terrible and I really suffered.

    Can a malunion of a right distal clavicle fracture cause pain numbness easy fatigue and pain in the rotator group of the Left shoulder due to the asymmetry as a result of the fracture?

      i wanna know this too, i just broke my collarbone few days ago and I'm still trying to decide if I should do the surgery or not. I think I have the Group 2 (neer) Type 2 IIA . it's so painful

    Hello,
    My name is Kelly Rollinson.
    I hope all is great, I had an accident back in 2016 and had some x-rays turned out to have a broken collar bone, I hadn't done any surgeries due to personal issues but from 2020 till this stage 2022 at the age of 22years I have been experiencing much pain, numbness, pain at the back, there is no that I can do all I can do is to get used to the pain whenever I talk to my mom she's just giving me hope that everything will work out soon, I totally understand but I'm the one who is in pain.
    Kindly advise me if there is any prescription medication that I can take because whenever it's cold I'm completely numb I can't feel my hand, head, lips, and leg well to feel better I massage using my right hand. I hope there is any treatment to release the pain that I'm able to bare for it rest of my life

    I have a Clavicle fracture at my left shoulder, i think its a group 2 (neer) good thing theres no surgery needed. they just gave me medicine and a vitamin for the bones and a arm sling, they said it took 3 -4 months of healing period.

      I have a type 2 neer and it’s been about a month and a bit it’s getting better but my full movement still has a lot of pain I suggest start doing physical treatment to it
