Can we achieve the workflow without "Hierarchy" or "Report to Position"?
Yes, can be assigned to a specific user as well.

What additional step is required to above video to ensure that originator cannot be the approver?
in Systm admin module > workflow paramteters ; there is a boolean using which you can prevent the submitter/originator and approver being the same.
While assigning the user to the workflow all users not display in the list some users only showing why ?

Yes, definitely organizational hierarchy, employee and report-to (manager) must be set if you want to use hierarchy in the workflow.
