  • Educational video describing conditions and treatment of patellar dislocation.
The patella is a protective bone located in front of the knee joint. The patella is attached to the quadriceps muscle through the quadriceps tendon. The patella facilitates the function of the quadriceps to straighten the knee. The patella lies within the patellofemoral groove. This groove allows the patella to slide vertically with normal movement of the knee joint.
Dislocation of the patella occurs from the patella sliding laterally out of its position on the knee. It may occur from a blow to the knee, acute trauma or severe twisting of the knee.
In majority of the cases dislocation is associated with pain, swelling and a lump on the lateral side or outer side of the knee.
The patella will usually relocate by itself back into the normal position in the groove as the patient straightens the knee.
Sunrise view demonstrates lateral dislocation of the patella. X-rays should be carefully evaluated for the presences of osteochondral fragments displaced within the knee joint.
    Mechanism of patellar dislocation:
    •Medial patellofemoral ligament failure.
•Lateral dislocation of the patella may also cause direct impaction injury to both the medial patella and lateral femoral condyle.
    Causes of patellar dislocation
•Weak VMO muscle (vastus medialis oblique).
    •Large Q angle ( normal male 14- female 17)
    •Excessive foot pronation
    •Patella alta
    •Genu valgum (knock knee)
    Treatment
•First episode is treated nonoperatively unless there is a loose fragment inside the joint which should be removed surgically
•Use knee immobilizer or a brace for 2-3 weeks to allow medial structures and ligament to heal.
    •Quadriceps strengthening exercises.
    Complications
•50% of the patients treated conservatively will have recurring episodes of instability and some patients will require surgery.
•Common complication of patellar dislocation is chondromalacia.

I just had my kneecap dislocated yesterday, probably the most painful thing I've ever experienced. The X-ray didn't find any permanent damage so they just taped it up but I'm terrified of it happening again.

My kneecap first dislocated 13 years ago. Ever since then, it has dislocated about 1-3 times per month. It is one of the most painful things I have ever experienced and it happens quite often. I just recently went to the doctor for it and am due for a surgery. Hopefully this can fix it and I can play sports without being terrified of my knee dislocating.

This hurts like hell, wayback 2013 I started to injure my knee its the most painful thing happened to me because of basketball then in 2014 I dislocated it. I had a trauma so I had a hard time playing basketball again but now in 2018 I'm thankful that I'm recovering from it and started to exercise for my knee to be strengthen. Believe me it's too painful

I dislocated my left knee twice, first when dancing at age 17, and again at age 21 whilst wearing a knee guard. I had ligament transplant surgery that same year. I am now 63, and no more dislocations. I have learned to adapt automatically to some of the permanent limitations from the surgery, mainly the lack of " suspension " in the joint. I still dance in community theatre. And although I have not been able to do some simple activities due to the lack of spring in my joint, I am ever grateful that I have been dislocation free for over 40 years now 🙏

I have a condition where my tendon is weaker than normal people have, so as a result I was born with a natural dislocated patella and I grew up with it (both of my knees btw). There was no way of holding it in place without surgery, but I couldn't get my surgery until I was 13. And as a consequence of growing up with both dislocated knees, my thigh muscles are weaker than usual people. The worst part is when I'm not paying attention a little bit when walking, I can just fall down without reason because my muscle is weak. And if it's in the public, you know how embarrassed I am, what's more if somebody asked me if I was okay and helped me up, and I just say no it's really nothing like literally nothing.

Good day! Just want to update. I had my dislocation 4 months ago and it happened again for the third time last April 2016. I was so depressed that time and I almost stop from studying. I'm still thankful to my practicum prof for letting me to undergo in my ojt. Sadly, I had to wear a knee support because I don't know when it will dislocate again. My parents don't want me to have surgery because they're afraid of complications. I thank God that I finished my ojt and for everything.

Thank you so much for this video. Reading these comments made me feel so validated. I have floating patella’s so I’ve Dislocated my right kneecap 11 times since I was 11, I’m 21 now. Got surgery in 2017 but dislocated it again recently. Doctors always tell me there’s nothing permanent or really nothing they can do. Most frustrating and painful thing and it’s hard to get people to understand the anxiety of it. Grateful for you all and these comments
