It is most likely due to physical trauma such as bone fractures and crush Injury.

Any specific reason why you don't use Lignocaine with Adrenaline ?

I'm not sure. But epinephrine causes vasoconstriction, so further blood supply occlusion will occur if we use it.
And we want to avoid that at all costs.
