The worst thing is... this episode premise has so much potential. A plastic surgeon who struggles to be intimate with a partner without resorting to sexual contact, a couple who gave up sexuality, who might be asexual to begin with but not aromantic... Have the surgeon see their happiness badabing badaboom you have a 40 minute episode with a good take away for both the characters in the show and for the viewer.
As a woman who is asexual and also somehow doesn’t experience sexual stimulation at all, I would LOVE to be rid of my nipples. They’re stupidly sensitive but the only sensation I get from them is discomfort.
I'm not asexual, but I totally get the discomfort with nipples. When I had a partner I didn't like them messed with much. Now it gets annoying if I wear the wrong shirt or bra.

I think he is purposely not going into describing different details of his patients to keep it more anonymous. Some may recognize the story & realize he's talking about them. He's mentioned turning down 1/5 of people who come to him for consults and why he did. Most of the reasons are due to him refusing to do certain procedures. This includes a range of certain injections (no silicone and I think he mentioned not doing fat grafting for the butt either), certain implants (textured implants etc.), and surgeries (such as butt lifts, thread lifts, etc.). Another reason and what appears to be the minority of why he rejects patients is when he senses the patient is trying to get surgery that is too extreme or unnecessarily risky. He implied those cases were motivated by body dysmorphia and patients not understanding the risks. I don't think he's mentioned that he's ever suggested to a patient they may have a mental disorder. This is most likely because it is inappropriate for a plastic surgeon to say to a patient that they have a mental disorder. I'm sure in more severely obvious cases of body dysmorphia, he probably may have suggested therapy but I can only speculate on that. For the most part, he's only ever mentioned disappointment in terms of reactions, but nothing further or specific. I know this doesn't answer your question really, but I hope it helps!
I mean as a trans guy, it's not uncommon for people to forego nipple grafts on top surgeries and end up with no nipples -- sometimes insurance won't cover them so they cost extra, sometimes people don't want to deal with the longer recoveries and higher risk of complications. Personally I knew I'd much rather have no nipples than a nipple on one side and none on the other, so I decided to forego nipple grafts because of the risk of graft rejection. It also meant I could shower sooner after surgery and my recovery was a lot easier. I'm happy with my decision. I was always going to have massive DI scars anyway so it's not like my chest was ever going to look "natural."
I also didnt have my nipples placed back 😊 I didnt think the result would be worth the struggle, and artistically Im very excited to have my chest be a blank canvas

When I get top surgery (I am trans FtM) I don't intend to keep my nipples. Nipple grafts add to the cost and increase some risks - like nipple death or infection. I joke with my husband that afterward I am gonna get nipples tattooed on my buttcheeks xD

The no nipple thing is also not too unrealistic, ive seen more than 1 people ask for such a thing... I guess I've seen some weird shit in my life, I hate hospitals in Greece so much...
