The Dragon Ball games are some of the ones which have an excellent excuse for offering ring-outs, considering how important they are to at least half of the tournament arcs.
I was shocked when I got desynched after killing a guard in Assassin's Creed 3, since they are not considered civilians. Turns out that the guard was actually George Washington.

It's worth failing in QTEs sometimes, to see the protagonist die a hilarious horrible death (Tomb Raider is case in point), right?
What about that one Spider-Man QTE where he lands face first on the floor and is exploded?
I remember that vaguely, and now I have to find it!
best I've seen is in telltales: Jurassic park. gets impaled by a triceratops then flung into a T-rex's mouth with an awkwardly long NOOOOOOooooooooooooo! that doesn't fit the daughters mouth movement remotely and takes long enough before it cuts to the 'failure' screen that you begin to think that you were meant to fail it...

oh, I just realized there are compilations and lists and top 10 videos of QTE fails, that's gonna keep me busy.
The worst one definitely had to be GTA: SA's train mission.

.... It's a fucking train, It's on rails, it can only go in 2 directions, forwards and backwards. Literally just catch back up to it.

Mortos DerSoulStealer I did not cheat myself but Big Smoke aim was so bad that you could fail even if you managed to stay right next to them pretty much the whole way. Honestly you needed to actually give him some distance so he could shoot them with at least some degree of success.
But I was so annoyed with him, that what I learned to do for that mission, was to use the stairs and jump ON THE TRAIN with the bike so I could shoot them myself :-D
All you had to do was follow the damn train
other way for me. The train mission took me maybe 3 tries. The RC mission took, maybe 20? and a whole lot of misery. That whole campaign was horrible, Im not sure why I played it, I could have done something productive with that time, instead of hating myself for playing haha

Building on the 'following someone too closely' fail, what really grinds my gears is games like GTA. When you have to follow someone in a car, you have to keep distance but this makes no sense when your target stops at a red light or in traffic. You have to drive erratically by not staying in traffic or driving up to the light and stop 5 car lengths away from them leaving masses of room for no reason. Surely that's more suspicious?!
Falensarano's Channel mafia is really bad at this with how narrow the streets are
Gta4's three leaf clover has to be the worst first you have to follow the crew or they will leave you to deal with that 5 star wanted level and if you manage to follow you have to deal with escaping 3 stars but with wonky driving controls

One time in AC2, I was chasing a story target on foot, and I saw the assassinate prompt pop up. But instead of plunging a blade into my target, Ezio thought it would be best to pull off a leaping double assassination on 2 civilians over to my right, instead of my target directly in front of me. This gave me an insta-fail, and restarted the mission for a very salty me.

or you do notice that its aiming wrong and you are like no not them. then it constantly switches back and forth between your target and some unlucky civilian till your target leaves. desynchronyzed.... might as well jump those civies then.

Well, an Ellen escort mission should be better than most. I can just ask her to stand behind me, so the enemies can't see her. Simple!
That's the thing with escort missions, you can't control them and it becomes RE4 for a few minutes.

My most rage inducing moments come when the game gives you an AI partner and they decided that the best place to be is right in my line of fire.
how about the arbitrary timers that run out even when you are seconds away from reaching the next checkpoint or final goal for the game to blow you a giant raspberry and boot you all the way back to the start to do it all again.
there's also the ever dreaded 'Network connection failure' that always appears when your about to win...

or let's say the times when you are fighting a timed boss then the timer runs out and you instafail... because the boss decided to skip the scene right in the middle of shooting a bullet.
I tell you what is an annoying auto fail. When you try to lasso a dead man's horse and the game decides that that's just not in character or something. It's what the horse would have wanted
i bet the reason killing big smoke at the end was so satisfying for people was soley for that train mission and not for actual story reasons of him betraying you lmao

I was having this discussion just the other day!: Isn't it weird that some games are so frustrating that you don't want to play, while other games are so frustrating and that's WHY you love playing then.

The "accidentally killing civilians" made me think of my first playthrough of Oblivion. I went full story-mode, only did things in line with my super-optimistic wood elf archer characters' moral standing. So one day I have to save this farm from.. I think is was goblins? The place was being overrun by goblins.
Now I'm a pretty decent archer, so I took position while the goblins were busy fighting the farmers' two sons. One, two, three, four goblins down.. And as I let go of the fifth, perfectly aligned arrow - one of the brothers JUMPS INTO THE WAY, takes an arrow through the head and is flung across half the field. His brother was so caught up in the fighting he didn't notice I just shot his brother. I'm really good at stealth, so yeah.. No bounty or anything.
My character kills the rest of the goblins, offers her condolences for the loss of a son and brother, and is on her way.
In time, I forget completely about this having happened. My character is of good moral standing, doing swell stuff all across Cyrodiil. One night, I decide to let her sleep at an inn.
I got SO SCARED when my character wakes up in the middle of the night by that guy from the Dark Brotherhood.Figuring it's play along or get assassinated, my character joins the Dark Brotherhood and one thing led to another and whoops she now runs it. I imagine she still just sort of hates that she's in it.
Frida Rendert what about Shayogorath (yes I know I can't spell)? Attack him and he sends you thousands of feet in the air to fall to your death?
Not to mention the Skyrim giants..
Frida Rendert I did something somewhat similar. My character was a tad less noble because he was a theif. Not a murderer but a theif. So i finished the theves guild ang got the Grey Fox mask. So i was using it for something... (i think it was an investigation mission) and when i took it off my noteriaty glitched never reset so everyone hated me and I had a bounty. So, I didnt have enough money to pay the fine (it was INSAIN) and I didn't feel like loosing my skill points. So i broke out of jail. Jail breaks are violent but i managed to get my mask back without being noticed so i kiiled guards with the mask on took it off when i got outside and surendered so evry one forgot. Ofcorse i was like "now hes a murderer... what should i do." Wake up and theres Freaky McWeirdo waiting for me. So that was my charcters descent into debauchery

it seems impossible to get both brothers out alive
at lower levels your iron longsword won't cut it
at later levels the goblins are all shamans and skirmishers or other high end types
On my next playthrough I DID manage to save both brothers. I took extra care not to shoot anywhere near those suicidal men >_>

My favorite was in Assassin's creed getting a game over for accidentally killing George Washington.

I have a few:
1) Failed mini games/missions/objectives during a fight (DANCE WATER DANCE)
2) not having the one item you need for that particular moment (worse if you need to go out of your way to get it)
3) enrage timers (not clearing a fight fast enough causing an auto fail [heavily used in FFXIV])

The third One is jak and daxter in a nutshell. The problem is a lot of missions have a time limit to be completed. Defenitely not annoying

First time I played breath of the wild (aka 'go anywhere: the video game) I walked to the edge of a cliff, climbed down until I could see the bottom was maybe 2 metres away, and then jumped off. Link insta-died and I was instantly discouraged.

The "Ring Out" has a plus side too. In some particularly tough fights in Weapon Master mode in SoulCalibur II, you can use a Ring Out to get an easy win. The ferryman mission is a good example. You have to hit them 20x before they kill you, but a Ring Out, oddly, counts too. It's also a good method for surviving missions where you're outnumbered.

Not mention that damn bar fight in Whiterun's inn. 'Look, I'm totally down for duking it out with you, but lets take it outside yea?' You hit one civilian by mistake and the whole inn goes apeshit.
The most painful QTE that happened to me didn't result in an insta-fail; it was much much worse than that, in assassins creed when Da Vinci wanted to hug Ezio. BUT I WAS TOO SLOW. The look of disappointment on Leo's face haunts me to this day.
If I'm doing a long game session, I'll use cutscenes to use the bathroom. I leave the controller behind, crack the door open and watch the cutscene from the comfort of my toilet. And then a QTE happens and I'm trying to decide if it's worth it to run from the bathroom with my pants around my ankles to see if I can press the button in time.

That was actually the second reason, the REAL first reason was because there is a global copper conspiracy, which is run by shady Vatican organization called Cuprum Maleficium Dei, and it controls ALL use of copper worldwide. The problem with controllers is that the cords contain a lot of copper, in order to transmit the commands to the console very fast and that the CMD order sees gaming as a cardinal sin and consoles as gateway to Hell, so they decided to ban all copper from console manufacturers.
To overcome this problem, the console manufacturers quickly developed the cordless controllers, using the technology they use in the Pioneer deep space probe program, by the way.

I like how 6, failing to keep up, uses an example of a mission where you really have to try if you expect to fail by letting the target get away. Freddy will stop at certain points to allow you to catch up to him. He also has god mode until he reaches the park and leaves his bike behind, which still annoys me to this day.

At least it made some sense though. The equivalent of being sent flying into and through a mountain. It's why it looks like glass breaking, up until that point your character can keep their wits about them to keep flying into the ring. Get hit to hard? You go loopy for a moment and then you hit the ground.
The real issue always was that the ring is always so damn small on that level. It didnt gel with the normal game mechanics either which just makes it all the more frustrating.

Well, it _was_ chapter 5... maybe something happened to Andy earlier in the story, maybe incapacitating him? Or maybe he was _in charge_ of that bank, and Mike had been secretly trying to learn stealth from him for years, figuring out how he and Jane could infiltrate it?

Discounting the whole Gringotts thing (unless Harry Potter exists in this universe too??) Ellen's "fanfics" sound like they could be more like journals. WHY ARE THEY TRYING TO SILENCE HER??

That moment when you realize that the player killing civilians in a video game leads to more severe consequences than a police officer killing civilians in real life.
To be fail, games don't have the same reasoning. With games it's "this is your mission, succeed or fail". And as noted in the video the failure conditions might be very arbitrary. the Rainbow Six games had hostage rescue scenarios where it was possible to fail because an ENEMY accidentally shot a hostage.

On a personal note, I've fallen victim to that 'chasing/following targets' insta-fail in Assassins Creed 4; one moment that sticks in my head was watching supposedly normal soldiers/civies (the mission target in this case) just suddenly jump in a surprisingly limber way. It was...unexpected.

Extra points if a game puts one of these in once, and it brings the rest of the game down.
Case in point Metro 2033. You've had allies before but they could generally take care of themselves. Then comes a section near the end of the game where you have to travel through an area with infinitely spawning self destructing creatures. And your ally dies far too quick.
Even more bonus points if an ally that's supposed to be helpful would be easier if you didn't have him along.
Yeah, gotta love games that ask you to protect NPCs with no sense of self-preservation who will run at enemies or into oncoming bullets.
I just love the insecere ways NPC's respond to Nate dying, that would be a good idea for a video. Worst NPC responces to death, nudge nudge, wink wink

That train mission in San Andreas gave me rage nightmares. When I finally beat that part (found a way to ramp the bike onto the train), and literally jumped up, threw the controller down, and started screaming at the TV in excitement.

My favourite insta-fail/hidden ending is in Fatal Frame 2, when Sae attacks Mio if you abandon her sister Mayu at the end of the game. (It's been several years, but I believe it's implied Mio ends up with the same fate as Yae originally did.)

"All we had to do was follow the damn train, CJ!" was the reason I quit that game. :( Respawning 5 minutes away to just start that frustrating mission again over and over was just too much.
How about the "Keep Vinnie alive" mission in Max Payne ? That was both a pain in the ass and hilarious at the same time. I spent more time working that mess out than I did playing the whole game !
There should be an entire episode dedicated to Ellen's Secret Book of Outside Xtra Fan Fiction.
