I understand now why he uses the scalpel to circle the heart for the bythos section, it is because inner hemorrhages only appear when you cut over it, he circles the heart so that he doesn't need the ultrasound to find the hemorrhages and treats it right away when he suddenly sees the "OK" mark and then continues the operation. Hint: Hemorrhages only starts appearing when Angie notes that the Bythos isn't doing anything.

That was one thing I loved about New Blood - we got to see everyone actually scrubbed up during operations. It's a neat little detail that I kind of miss in the other TC games.
In every single one of the games, the boss fight is in the heart. In UTK and Second Opinion, it’s Savato. In New Blood, it’s Cardia. In Trauma Team, it’s Twisted Rosalia.

I think it’s more that the heart is symbolic of the soul and emotions, it being at the center of body regulation. Defeating the “guilt” in their heart is analogous to defeating the opponents argument (in this case that they can become Gods through medicine). Also, the heart is the most sensitive and unprotected organ in the body (aside from the brain) so it adds to the stakes.

If you mean during the bythos stage, bythos creates hidden hemorrhaging, which can burst and take down vitals dramatically. You need to cut open the hemorrhages with the scalpel to avoid this. the circular scalpel technique is so that he can take care of them without needing to use the ultrasound.

ive been trying to get past through this goddamn operation for the last 8 years, its driving me crazy probably the fact that my screen is never properly calibrated doesnt help either
I tried it before and it causes a MISS. It's suppose to be use on Aletheia to weaken it so that it will expose the vessels that it uses to draw on energy from the host.
According to Tyler, its a Neo-GUILT antibiotic which means it would work on Neo-GUILT and the mutated GUILT so it would make sense to use it on the GUILT that appear alongside Aletheia.
I wanted to throw my NDS out of window when I missed vein for the 8th time in row. I dont know if there is a trick or something to quickly cut them down, but when I leave with one, its always too late and GUILT regenerate.
