I heard a lot of people wondering why John didn't recognize Sherlock sooner so here's my take away. It is quite obvious that Sherlock had an ENORMOUS role in John's life before he "died," and that there is absolutely no way that he wouldn't have recognized him. So this implies that John CHOSE intentionally not to see him. After Sherlock's "death," John probably often saw shadows of him. People all around began to look like him because John's brain couldn't accept the fact that Sherlock wasn't alive and next to him basically 24/7. This was his method of coping. Next, we come to the restaurant scene, and in my opinion, the most definitive evidence of this theory. John, with his military background, must've scanned and seen all of the people in the room including Sherlock in his "disguise," and Sherlock goes near the table more than enough times for John to get a good look at him. Here is the scary part. Considering the repetition and irrefutable likeness, John must be consciously suppressing anything including Sherlock. What this demonstrates is that John can't distinguish delusion from reality. Sherlocks appearance in his life occurs so frequently and vividly that anything he thinks he sees he immediately denies, no matter how striking the similarity. The real Sherlock could've stood in front of him, and he wouldn't recognize him.

He wouldn’t have even thought that someone would jump in front of him to safe him because in his mind, people don‘t risk their lives for Sherlock Holmes. It was Mary‘s decision. And yes, Sherlock did promise, made his first and final vow that he wouldn‘t let anything happen to them but c‘mon John, you‘re a doctor and a soldier. You of all people should know that a promise like that can‘t be kept, no matter how much you try… I understood his initial anger and frustration, but shutting him out completely and putting the blame on him for so long was uncalled for because, News Flash, Sherlock just lost one of his best friends too and he had to do something he had never had to do before (except for redbeard but he probably couldn’t remember much of that): grieve. And he had to do that completely on his own. That‘s my take on it anyway.
