Hi Vendo! Yes, this is Form N-445. We go over the Oath Ceremony and Form N-445 in this video (starting around the 5:10 mark) - ua-cam.com/video/_vUAroJ22-g/v-deo.html
The questions on the back of the N-445 need to be answered on the day of your oath ceremony. The questions are asking if there have been any changes since your interview. So, between the date of your interview and the date of the oath ceremony, did you get married or divorced? Did you travel outside the country? Did you get arrested? Did you smuggle drugs, become a prostitute or a habitual drunkard? etc... They just want to know if there have been any changes since your interview. Some people don't have their oath ceremony for several months after their interview. Still, most people have all "No" checks in the boxes.
Please write the city and state of where you filled out the form. So if you do it at home before leaving for the ceremony, write your city and state. Date it with the same date as your oath ceremony. Sign your full name and write your full address. Bring the completed form to your ceremony and give to the officer at check in.

*Please remember to check your Naturalization Certificate very carefully BEFORE leaving the building.* Make sure all information is correct. *This is VERY important.* It is complicated (and very expensive) to have it corrected later.

Hello is possible to make a video for a citizenship interview with N-400 application for a widow person, I have been studying since I applied back in February and I’m scheduled for my interview in June 15, thank you so much in advance, your videos are very helpful for my daily studies routine.
Hi Hector! Wow. That was a very fast processing time. Good for you! We don't have a specific practice video for a widow, however some of the questions regarding your marital status might be:
1. What is your current marital status? I am widowed.
2. What is the name of your prior spouse? (full legal name at the time of death....)
3. How long have you been widowed? (amount of time since death of spouse)
4. How long were you married to your prior spouse? Since...date of marriage. About... years.
5. What is the date of death of your prior spouse? (date of death)
6. Did you bring the death certificate of your prior spouse? Yes (hand officer the original death certificate. Bring a copy if you didn't submit it with your application)
Basically review the marital section of your N-400 (Part 10). Although only questions regarding your "prior" spouse pertain to you. You don't have a "current" spouse. So, Part 10, questions 1 and 9 only.
