If you didn't do any of the sidequests that had Elaine's dad appear in them, then the Quincy President option wouldn't have appeared. Van wouldn't get in time to save him, so he would end up in a coma for who knows how long.
If you don't do the rounds well enough, the 3rd option for the ETWS president doesn't appear either. And unlike Auclair, those ones aren't marked with quest markers. I think its the conversation about the mismatching balances in the bank that enables the option, which are just normal NPCs.
