- Milestones is a 17-minute film focussed on care in the last days of life in an acute hospital setting. A suite of educational materials have been created to support the film, including slide sets, handouts, notes for facilitators and evaluation tools. These can be used to deliver end of life care education for acute hospitals, and include specific modules on the following aspects of care:
• Recognition of deterioration
• Effective communication, including DNACPR decision-making and discussion
• Making an individualised plan of care
• Physical care at the end of life

The Milestones film and educational materials have been developed by the UCLPartners End of Life Care team with funding from Health Education England working across North Central and East London (HEE NCEL).
A dear friend was seemingly unconscious. She cried when I came to see her. She cried when I said it's her time to rest. She cried when her son came. I kept talking and rubbing her hand. She was gone just hours later. I believe 100% she could hear us until the end.
I'm so proud of my daughter who is a palliative care doctor in a hospice, I know in my heart that she helps people like this after seeing her grandmother die in a local hospital where she was treated appallingly, I think it shaped her career path.
Thank God mom had a living will. Not that she used it when it came down to resuscitation. She didn't want to be kept alive on machines but when push came to shove she chose to use the machines at first, but when she fought as long as she was able she indicated to just keep her comfortable. She went to palliative care where she died peacefully 2 days later, on January 15th 2019. RIP mom.
This video made me cry, 8 years ago my husband died with lung cancer, we brought him home, he wanted to die at home, with his three daughters around him. He was trying to hold on for me and his girls, he was getting weaker and weaker. The hardest decision I had ever made was to give him the permission to die. I promised him that I would take care of his daughters and grandchildren. Each daughter spent a few hours with him talking to him about the fun they had. My youngest stayed with him and he waited till she was asleep and then he passed.

@@mariannenimon2746 I am so sorry for the death of your mom, Ms. Nimon. It seems we struggle coming into this world and barring a sudden death, we struggle going out as well. How fortunate your mom was to have such a lovely daughter-but I’m sure you’d see it more as your great fortune to have such a mom. Please take care, my friend: You need some TLC as well.
One of my sisters is a hospice nurse. We also lost our stepdad to Cancer in 2017. Knowing my sister, I think she would really strive to listen to the patient and the family. Nurses are crucial members of the care team.
Just went through this exact same scenario with my dad....end stage lung cancer, COPD, kidney failure,... 2 weeks after losing my sister ,and my mother in August.... my brother passed in 1990.... such a difficult time....but I agree...being open and honest is the best for everyone 😢
I lost my Dad two years ago to lung cancer and COPD. He died exactly the same way this video portrayed. It was hard knowing that he was dying, but I was relieved to know he wasnt suffering. He died at home with all of us around him. Sorry for your loss. I know how hard it was to lose your Dad.

I felt the same exact way the daughter felt . Dont anyone here know my mom or have a clue! Er doc says well mrs bailey you're cancers back your dying. No one told us till then . I'll never forget.
My husband and I will be going through this soon. My husband was diagnosed with Head and neck cancer 8 years ago. He has fought it twice and went into remission. It has come back a third time and the doctors say there is little they can do. He is on immunotherapy to slow down the progression, but he is now in a lot of pain and doesnt have the energy he used to have. Im not sure how much longer he has, months, maybe a year? Only God knows. Thank you for the video....it makes me feel better that they can make him comfortable and keep him from suffering in his last days.
Laura T - Mine’s at the same crossroads. Liver cancer. We only found out a few weeks ago. He starts chemo next week, but we’re looking at months as well. It’s hard.
My friend died after been told she was dying of lung cancer and secondary brain cancer, she died 8 weeks later I cant believe this she never even suffered any pain. Luckily she told me she wasn't afraid of dying

The doctors should have relieved his pain before the serious discussion. Wrong to let the patient be in so much pain. I found the doctor cold and casual, whereas the nurse Stephen showed compassion to both patient and daughter. Sad scene, the end of life can be for some.
A lot of doctors are cold and casual unfortunately, especially at larger hospitals where you often get more than one doctor (general, specialists - e.g. pulmonologist for lung cancer, anesthesiologists, etc), and you'll inevitably get one who is more callous.
One of the doctors in contact with us just ambushed my mother and father with "the discussion", and it almost seemed like he was trying to force us to accept that my father could die from any operations. Sure he was probably rushed but I didn't appreciate him making my mom cry like that in his callous rush.
Totally agree with you, most important thing is to keep cancer pain free. Unfortunately this is NHS.

I just recently lost a good friend of mine to cancer. It was so hard to see my friend go. I never really understood why my friend was not recovering. Watching this video gaved me a better understanding of what was going on. Thanks.
Everyone should have a care team that is so straight forward. They explained the process and what to expect very well.

omg….this is EXACTLY how my beloved dear uncle Steve, who was also my godfather, died a short time ago, same cancer, same liver and kidney failure, same everything, it’s like watching him die all over again, and my heart is broken. The last few days he lay unconscious in bed, he stopped eating, refused any fluids, then stopped talking….a few days of laying in bed on heavy pain meds, aside from the occasional coughing fit, but not aware of his surroundings, with one foot in eternities door, and he was gone. No words can describe the pain you feel, it was as though my heart was ripped out of my chest, a part of me died that day, right along with him…I shouldn’t have watched this, it’s far too real. I’m right back in the hospital room now, watching uncle Steve take his last few breaths and hearing that horrific “death rattle” (mucus secretions build up, the patient can’t clear it, so they make a ghastly sound that haunts me forever). I appreciate describing to people what it’s like losing a loved one like this, but this video was so much like what I went through, the only difference is,, we’re white, that’s it, everything else is exactly the same, it’s just brought it all rushing back like a flood, I can’t even see my computer screen, got to go, I can’t watch anymore and I can’t talk anymore, my emotions are crippling me, and part of me wishes I could join him right now, dammit I miss him so much, Rest In Peace uncle Steve 😰😰😰😰😰😰😰😰😰😰😰😰

I would rather total honesty,than magical thinking! Also,please dont ask me to fight anything,just to stay here for you! If Im dying and in pain,please administer as much painkillers that will keep me comfortable until my death.

It's very sad for the family to understand that the staff and the patient communicate better when their not around. The patient asks questions and communicates all their worries, but when a loved one is there they don't want to worry them so they stay quiet. I know it's so very hard for the family. I've seen this too many times. We become selfish when we don't want our loved one to die 😔 However they're the ones suffering.

Watched this video with my class, now I'm revisiting it to point out some medical errors
(Apologies for being heartless)
1. Nurse should've washed his hands before dealing with the patient
2. Nurse should only discuss patient info with his primary care doc, not just a random doc passing by since it is a HIPAA violation. On top of that, the patient was able to hear them.
3. Daughter was correct. The female doctor should've not walked in since he's not her patient.
4. The male Doctor actually seemed happy when telling him the cancer was spreading lol. 10+ years in med school and still no manners lol.
5. (Maybe I'm wrong), but if the patient has a chest infection (possibly pneumonia) where are your N95s people?!
6. Nurse should've checked his vital signs before assuming he is dead.
Here in Canada we have legislation called MAID. Medical assistance in dying. People can apply for the procedure under strict guidelines. Autonomy over ones life is merciful... I have included this in My living will....
Here in the US ,it's called "Death with Dignity"

Scripted or not, I have seen this in REALITY, and I stand with what I said that the doctor in the video should be more compassionate!
I felt it was handled very well.Most pts know they are dying. Niceties and “ beating around the bush” only confuses them. I could only hope in the times of open & honesty for all the patients.bThat this model is used often. In my years of nursing, I have found that once the darkness of this discussion is done, most patients are very peaceful with their reality.

If they pumped James full of painkillers before the dr dealt with the facts about his illness, James' will be too sedated to participate in the decisionmaking, to the realities of what he's facing.
The daughter's having a hard time accepting that, at this point, her father is terminally ill, treatment is no longer an option, and she needed to be a part of this discussion.

I don't think so...She understood her dad was sick but needed to wake up to the fact that he is dying. She was in denial and the frank discussion helped her to understand why further treatment and revisitation is a bad idea. Even after the discussion she was in denial a bit and this happens all the time. Often time we wait until it's too late to discuss end of life issues and patient wishes which as the doc stated leads to needless suffering.

The woman dr is exactly what makes the hospital a horrid place. All those different doctors to come and see her father. The daughter is right. All these random dr’s who come in they just have hear say from the other dr’s and nurses. Her little attitude and then agreeing to see the patient looking irritated, is just so real. I have been in and out of hospitals for 10 years. I know that the nurses ignored him with his pain. Horrible horrible.

The gospels show what Christ has done on the cross for us. The book of Acts shows us the beginning of Christ's Church and how to enter the Church, obeying Acts 2:38. Are we supposed to follow the teachings of the apostles? Acts 2:42, They continued steadfastly in the apostle's doctrines. Ephesians 2:20, We are built upon the foundation of the apostles and prophets, Jesus Christ himself being the chief cornerstone. Jesus Christ was our perfect example back to God. He was baptized and received the Holy Ghost and we must be baptized and receive the Holy Ghost. He was holy and walked in righteousness and we must be holy and walk in righteousness. He died and rose again, and we must die to the things of this world and rise to walk in the newness of life. He prayed to the father showing us how to pray to the father. @@jeanseide2955 One has to first believe that Jesus Christ died on the cross for our sins. We need to be willing to obey Acts 2:38 like it was when the church got started. Acts 2:38, Then Peter said unto them, Repent, and be baptized every one of you in the name of Jesus Christ for the remission of sins, and ye shall receive the gift of the Holy Ghost.
Acts 5:32, And we are his witnesses of these things; and so is also the Holy Ghost, whom God hath given to them that obey him.
Many are being taught that you don't need to be baptized of the water and the Spirit, but John 3:5, says differently. John 3:5, Jesus answered, Verily, verily, I say unto thee, Except a man be born of water and of the Spirit, he cannot enter into the kingdom of God.
Find a place where it's just you and God. Start calling out to God. John 4:24, God is a Spirit: and they that worship him must worship him in spirit and in truth.
Be willing to give us everything to God in your mind. Tell God exactly what you want and believe that he will give it to you. It took me about 45 minutes before I started to fill his presence so don't run away after a few minutes. Everything was coming to my mind as I was calling out to him but I kept calling and trying to concentrate on him. I believe when you start getting in the Spirit you will start to fill his presence.
