    Great to have you guys back, can you do a full show on care post transplant. You did one about finasteride and minoxidil, but also would be interested to know when we can do LLT and microneedling after the transplant.

    I have a question regarding minoxidil long term. Does it stop working after 5-10years and will you lose those hairs that are dependant on minoxidil even if you keep using finasteride and minoxidil?

    What are your thoughts on using beard hair as donor hair for the scalp? Do you do so in your clinical practice? Also, you guys talk a lot about having FUT first and then FUE if needed. What are your thoughts on doing an FUE first and then FUT?

    Hey doctor. Could you make a video about the different fue extraction machines, for example the differences between waw fue system vs trivellini vs dr ugraft?

      I think it is a very personal choice. I have all 3 machines. One is not necessarily better. One may be more beneficial in 1 particular skin type. Your surgeon should be more comfortable with whatever machine they are using.

    hello Doc, i have a question, does hair need non stop continuous blood supply for it to grow and develop? because u informed us that minoxidil has a half life of 18 to 20 hours and it is enough to take a dose of once a day, therefore there is a few hours break where minoxidil is not in your system meaning no blood supply to already less/no blood flow affected hair follicles, so how will these few hours of no blood supply to hair follicles result in?

    I’m SERIOUSLY in distress here. So I’m 20 and my beard is slightly coming in. Dad says he had a full beard mid 20’s. The past year I’ve been experiences massive hair loss and I feel like I’m either gonna have to choose beard or hair, and I’d rather go hair. I still want to grow a beard though obviously. Do you think it’d be worth it to try a topical finasteride at maybe a .005 or .01 concentration to not crush my systemic DHT and still give me a chance for a beard. My other option is .25 mg finasteride 3-4 times a week

    Hey Guys love your videos. I got a question, earlier this past year I started minoxidil for some hair loss I was noticing. For personal reasons I stopped two months into using it, due to some side effect complications. About a month into my use of an accident I started noticing shedding already, now it’s been 2 1/2 months after I’ve stopped and I’m still noticing more shedding. I attribute this to telogen effluvium, I how long do you think it will take for my hair cycle to relapse and some hair to start growing back?

      The withdrawal effluvium can last longer than 3 months so hang in there and things will settle.

    Hello doctor , i have a question
    today i went to lab to test my hormones my (SHBG ,estradiol ,LH ,prolactin were very normal
    and my Testosterone was 3.3 ng/ ml and free T was 0.242 nmol/L ) but my ( FSH was 1.22 which is low ) and i am worried about my FSH , could Finasteride and dutesteride make FSH low ? ive been using Fin And Dutesteride for almost 2 years daily

    Hello doctor, i have a question.
    Do you think 2.5mg dutasteride a day is superior in fighting hair loss than 0.5mg as statistically there is a 79% inhibition of scalp DHT in the higher dose compared to 51% inhibition in the standard 0.5mg dose. There is one study that looked on that.
    I tolerate 0.5mg a day dut very well but my hair loss is still continuing at age of 21 even after 18 months of it's use (very aggressive AGA) and I'm not afraid of higher risk of sides

      I would only increase to 1mg per day to begin and see if that makes a difference. Be aware the scalp half-life is 5 weeks so it can build up significantly over time.

    Can you talk about minoxidil strength… I see 10-15% out there and some say it has no different affects than the 5%, hoping you guys would have more to add

      The dose response curve flattens at about 5% which is why that is the strength available at the pharmacy. Adding higher concentrations might only make a marginal difference and it seems more like a marketing strategy than anything else. It needs to be sulfonated to be active and that depends upon the natural level of the enzyme in your skin that achieves this. If you give it orally it can also be sulfonated by a second enzyme present in the liver so that is one reason we like oral minoxidil as well as ease of use.

    Any tips?
    I randomly started shedding hair out of nowhere?? My hair has never done this!! And it’s freaking me out. I put my hand through my hair and it falls out so easily and when I shampoo my hair the same thing happens. Seeing my hands filled with hair is freaking me out. Not to mention I am 18! I have always had healthy hair and taken good care of it I would say anyone know why this is happening? If this it shedding or hair loss?

    What if an individual had a hair transplant (No meds) and after 1.5 year hair started to thin with a rapid pace and almost 2 years down the line is back to how it was or worse?
    What could be the reason or what do you suggest at this point?

      Medication like finasteride to stop further loss and a growth stimulator such as minoxidil to potentially kelp regrow some of the hair back. The sooner the better.

      Sorry, this is quite useful for Alopecia Areata and potentially Frontal Fibrosing Alopecia but not for Pattern thinning or balding. Russell

    I have been taking finestride 1 mg everyday and also using topical minoxidil since last 2 years but now i m experiencing aggressive form of hair fall since last 3 months
    I don't know what to do now ,i m clueless about it
    More than 100 hairs a day is like a nightmare for me ,,
    Now i have switched from finestride 1mg to dutesteide 0.5 mg alternate days since last 15 days
    Please help me

      I don't think the switching of drugs is a good idea. I think you should " add up " dutasteride 0.5mg once a week on top of finasteride everyday . Combine both ! Don't change drugs (just my opinion though).
      Ps : there are some good studies on finasteride everyday + 0.5 dutasteride once a week (since it has a long half life). You should try this out first before totally switching drugs . Give this at least a year 😉

      From now on i will go as you told
      Going to start finestride again (every day) and dutesteide once a week
      Let's see what happens
      But i really scared of the way my hair falling..
      My hairfall started way back in 2011 but anyhow I managed to hold on
      I m still have hairs.
      I m norwood 2 or 3
      And the funniest thing is that in my temple area there are still thin hairs but don't catch it's natural length

      @@aashajain7376 you gonna be just fine on finasteride everyday and dutasteride once a week . Just be ready for a big shedding though (even only once a week dutasteride will be powerful enough to trigger at least 6 or 7 months of periodic sheddings. So be ready for that and don't freak out.
      Everything will grow back way stronger and way thicker around month 9 to month 12 up to month 24. So you have to be patient.
      But eventually it should stop your hairloss 100% by month 24 ! You got this ! 👍

      @@aashajain7376 ps : you actually don't have to take finasteride everyday. For example what you can do is :
      - finasteride everyday (but no finasteride on sunday).
      - dutasteride on Sunday.
      There is kinda no point to take both finasteride + dutasteride at the same time on sunday.
      Edit : if even 2 years later you still lose ground (I don't think it's gonna happen to be honest). But if it happens : then consider dutasteride twice a week and finasteride on the other days.
      Ps : go watch the UA-cam Channel of " HairCafe " the name of the video is : " using finasteride and dutasteride together to stop hairloss. Is it worth it ? " . He explains everything about this combinaison therapy + the studies behind it . You will find it interesting.

    seems like hair transplants have a lot more risks, than rewards. As soon as you get one… in your 20’s. You are committing to it for the rest of your life.

      I guess it all depends on what kind of a candidate you are and what age you do it. But one should never be too aggressive and plan it well. Its really tricky though because I have now gone through 2 hair transplants (6634 grafts) and my donor area is already depleted. If I lose any more hair at age 36+ then I might be in a bad situation.
