    1.for a 44 cobbs angle for 11 years old - to brace and delay surgery or to go for ASC. 2. can an over correction occur if they gain significant growth of their spine.

    Thank you Dr. Lee and Dr. Antonacci. I want to ask what the cord breakage rate is so far for adults in recent years since you started the double screw double cord technique.

    Thank you for these interviews Dr. Lee. I am a young adult with scoliosis, trying to determine weather surgery is worth it in the long run.
    Do scoliosis correction surgeries increase risk of disc herniation? Also, do the metal screws weaken the vertebrae?

    2 questions: Will the spine be able to move normally after the ASC surgery? What is the cost of the procedure?

      After tethering, there is loss of flexibility in lateral bending through the tethered levels. Studies have indicated that it's about 50% in lateral bending with single tethers and 78% with double tethers. However, spine rotation and flexion/extension are maintained.

    Thank you Dr. Lee and Dr. Antonacci. Is there any hope for older patients (over 50) with some arthritis in their spine that eventually this type of surgery could be available for them? Or a surgery that does not involve rods and fusion and preserves mobility?

      Is the main reason for not doing ASC on patients with arthritis that the pain will not go away? Because I would take that if I could have a straight spine and stop progression without rods.

      @@anamariamutis5474 unfortunately tethering in adults (ASC) is not appropriate when there are degenerative changes to the discs and joints. As we age our spines stiffen which complicates matters. It would be best to consult with an ASC surgeon to see if you are a candidate.

      @@DerekLeeis there data on how hardware holds up over time? Older patients are not recommended to have a new surgery, but is there harm to joints or discs as surgery patients age?

    Can this be done for an adult with S curve due to congenital scoliosis due to 2 hemivertebrae. The curve is S shape with upper curve being 50 degrees and lower being 36 degrees all in the L1 to S1 lumbar area or possibly starting right above L1.

      Unfortunately, this procedure is not appropriate for congenital scoliosis. The vertebrae need to be whole with very little degeneration.

    This new study, "Concave and convex growth do not differ over tethered vertebral segments, even with open tri‑radiate cartilage" suggests that tethering doesn't modulate growth. Has that been addressed by Dr. Antonacci or the VBT community in general? Would VBT and ASC differ on this?

      I've talked to other surgeons about this study and they basically tell me it's just one study. There are other studies that show that there is bone growth modulation occurring with VBT. We have to wait for more studies to confirm or deny.
      Both VBT and ASC on immature spines do depend on bone growth modulation to stabilize corrected tethered curves at maturity.
      No, Dr. A hasn't addressed this.
