In the late 90s a polyp was found on Marko's vocal cords and a doctor suggested removal. There were difficulties during the surgery - aka they had to cut that thing out with more force than hoped for - which lead to a permanent damage on the right side. Due to that, his vocal cords aren't vibrating in sync anymore and he had to relearn how to sing. For months he couldn't even speak properly and he went through a massive depression. The unique "nasal" falsetto we all know him for has its fault in that surgery, which, later on, another doctor admitted that it may not have been necessary at all, leaving Marko even more pissed off of course. He has meanwhile come to terms with it as there is really nothing that can be done anymore, but is still angry/sad when having to think about it. Also he has to still put a lot more practice into vocal techniques he is certain he would have had no difficulties with, had he not had that polyp removal. But we'll never know. I really highly suggest getting his biography. It is a rollercoaster of a read and also puts a lot of light on the evolution of metal/rock in Finland.
Just like all the other outside of Nightwish Marko stuff that I have heard so far, I do not really like this. I did like you getting confused by young Marko though. :)

Marco had some kind of surgery on his throat years ago and that killed a bit of the cleanliness of his higher notes.
