A patient in her late 80s returns for regular foot care. In her previous visits, she presented with ulcers beneath calluses on the tips of her toes. This time she has ulcers beneath 2 toenails, as well as fungal toenails and calluses. Her poor circulation is made worse by her smoking. She does not have diabetes, yet she has some of the same foot problems due to this condition.

I agree with you. He does an amazing job & I love watching his videos. I’ve watched plenty of videos and many foot doctors are not as meticulous So I really appreciate the work he does.

Silvia Latoni agree with this! My three favourite are Dr Jefferson, Toe Bro, ans and ms foot fixer as they don’t just hack away. They really make sure a persons foot feels nice and are meticulous in their treatment. I imagine for many patients having their feet “look good” really helps their self confidence as well.

Hello Sir, it would be reassuring to know there was a doctor of your caliber in my vicinity. The respect and attention that you give is awesome! The way you approached the smoking issue was the best way to handle a touchy issue. Instead of condemning her. You used it as a part of her treatment plan. I was listening and replacing smoking with weight issues. as you spoke with her at the end. It made me feel good. It is so easy to become defensive regarding these issues. You didn't belittled her. I know plenty of healthcare individuals who need to learn your technique.
I've learned a lot watching you work.

And making concrete suggestions for how. So many think they should lecture the light switch off and even refuse to treat the other issues. Dr. J listened to her talk about not smoking till she moved from MS to DC and enlisted her and her grandson in problem solving how to reverse that. I hope she is able to make some progress. (Even if that progress is just identifying triggers or obstacles!) Granny Mississippi is "not helpless!"

My parents have been smokers all their adult lives (in moderation, not heavy at all for sure). They are both mid-70s now and their foot health is excellent. No issues at all. So it's not always "just" the smoking. It's a total picture. And well, I guess, smoking doesn't affect everyone the same. But it's true, it is hard to give up and I don't think talking about it will make her quit. It might even drive her away if it is brought up all the time.

The think that concerns me about telemed is that they're not seeing the in person issue in person. Would he be able to tell if there are abscess under the nails or calluses without see them? It worries me of what my doctors are unable to truly see an issue. They have ordered tests for me though.

Love the new water-assist nail drill! How do you choose your drill bits? Of the ones I've seen you use, it seems the cone shape works best, followed by the thin bit you use on the side walls and cuticles. Do you prefer metal or ceramic? The round one you used on this video seemed to skip over the nail a lot ( or was that due to the lady's twitchy feet?) Glad you could help this lady, and I'm glad you understood her. I was only catching 1 in 3 words. Great job! Her feet were so improved! I hope she stops smoking.

Dr. Kevin, you are an ABSOLUTE ANGEL. You always go above and beyond for your patients, I see other channels on UA-cam that show patients with the same type of foot problems that go to a “Doctor” who cuts the nail length and some thickness, then they “file” the nails to make sure there are no sharp edges and clean up and send them home. They claim they feel like sculptors, but in my very humble opinion, they do the minimal for what insurance requires and believe they’ve done a “great” job. Dr. Kevin out of the 10+ channels I’ve been watching over the years, you are absolutely amazing with your patients, you actually take care of the WHOLE person, versus just their feet. You ask questions about almost every aspect of their health and how they’re doing and if they have adequate help. You are an Angel. I know I’ve said many times on here, but every time I see how you change a patient’s life by fixing their feet, I can’t help it. God Bless and keep on doing the great work you do.

Smoking causes poor circulation, especially with sugar. Then we start losing digits. I don't want 2 see that happen 2 her, or anyone.

My favorite part of the cleaning is the wet dremel! I wish you would show the entire Dremel process in all your videos. It’s amazing how well it cleans up the nails. You do such a great job with getting good video and editing it all together. Thank you Dr.

Im a little over halfway done watching the video, and I must say....Im loving the music during the drill portions of your vids 👌1000x better than the annoyingly loud drill sound that scared me half to death and I'd quickly turn my volume down! 😆
