    A patient in his early 60s returns to my office after 3 1/2 years. Many years of heavy smoking have led to a stroke and poor circulation in both legs that required bypass surgery. He now has an ulcer on his big toe and on the bottom of his foot. Part 1 of this case shows the severe effects of smoking on the feet, in particular the toenails.
    Smoking can have devastating effects on the feet, especially for patients with underlying conditions such as heart disease and diabetes. Nicotine in cigarettes is a stimulant that raises blood pressure and is a vasoconstrictor resulting in small veins and arteries. This is why some smokers notice coldness, tingling, or a change of color in their hands and feet. This restriction in blood flow makes it hard for the heart to pump blood through the constricted arteries. The skin and nails of the feet may also change. Often the nails may become thick and brittle, and the skin on the feet can become dry, flaky, and fragile. Some patients may notice sores or injuries in their feet causing the feet to be slow to heal and in worst cases may lead to amputations.

      It's sad that some people just want listen to their Doctors until it's to late!!! Cigarettes I hate them!! I cannot stand the smell of them !!! People please please listen to your Doctor God place them here for a reason!!

      @@wingsfan99994u you can do it. And don't give up if you fail, it took me 3 tries to quit. 2 packs a day for 30 years, haven't had a cigarette since Sept 29 1998. Sending prayers your way. PS think of all the money you'll save.

      At 56, I found myself developing aches and pains that caused me to begin skipping showers, throwing a hair band over my hair instead of holding my arms around my head for an hour to fix it, wearing the same thing twice (hanging it up to air) so I wouldn't have to haul so much laundry for washing. My standards decreased a lot until I looked around and said wow, younger me had higher standards. I am fighting to make myself get back to that. Even my eyebrows got shaggy. That is why we may look and smell older than our time.

      Sometimes, age and illness make it all but impossible to take care without outside help. This doctor seems very kindly and respectful of this patient.

    My younger brother was a life long smoker. I spoke with him about a recent Dr.’s visit, and he shared his big toe was dead. He had it amputated. Shortly thereafter he passed. For all your loved ones express the need to stop smoking.

      I smoked 2 1/2 packs a day when I quit 9 years ago. Didn’t start out that heavy a smoker but gradually built up to it. Am so glad that I quit-now if I can just get my husband to quit wed be okay. My Mother’s feet looked like the patients in this video although she had a callus and a whole nest of corns(to quote the Doctor who treated her). When she came down to live with us, she always wore shoes when I was around her. Did not realize how bad her toenails were until she went into the hospital.

    I'm sorry to have to say it this way but this patient is very much like my ex (now 75) in that he is so addicted to cigarettes that he's willing to suffer and die for it, I gave up smoking over 30 years ago (I'm now 62) after having a scary succession of asthma attacks over one summer that scared me enough to quit and it turned out to be a good thing.

      so true ,I have a sister in SC just had a new knee ,didnt smoke for 7 days .Been smoking since 16 now she 63 . As soon as she came home picked up a Cig..I told her if she could stop for 7 days she can stop .her answer to me was " I am addicted and I can't do with out them " .I told her she has to .she has COPD ,diabetes, high blood .Your putting you life in jeapordy..Just have to pray ..Amen

      I forgot to add that with my ex husband it wasn't just cigarettes he was badly addicted to, he's also a severe alcohol, even a stroke couldn't break him of these 2 nasty addictions, he's still alive but I had a dream about him dying a couple days ago, these types of dreams that I occasionally have always come true within a few months or so, years ago I had a death dream about my mother (she died within a few months), soon after that I had a death dream about my beloved cat Bebe's who was 20 years old in human years (she had a stroke within 1 week) so when I have a death dream I believe it unquestionably.

    Doctor - We have often seen you take samples that are sent to a lab for analysis. Would it be possible to video document that process, then show and discuss the results of a specimen test?
    I think it would give an added dimension to the fine work and care you provide to your patients.

    Had a doctor friend tell me that no matter how many cigarettes a patient reports smoking per day, he increases that number by at least 1/2 - maybe more, depending. He says smokers who aren’t actively trying to quit tend to be a little dishonest about their habit.
    Deaths in my family from smoking:
    Grandfather, small cell lung cancer. Age at death; 60.
    Grandmother, emphysema. Age at diagnosis, 70. Death at 77. Meaning it took 7 years for her to s-l-o-w-l-y asphyxiate to death.
    Eldest sister, small cell lung cancer. Died @ age 37.

    This gentleman’s toenails are extremely long. I’m sure I’ve asked this question before, how does one wear shoes? I hope and pray he continues his care under your direction. I need to show my father these videos. I wish he would stop smoking. He has issues now he’s not addressing. We shall see. 😒

      Really difficult situation with your father, biggest issue seems to be that most people don't believe it will happen to them! My father was smoker and alcoholic, died at 51 after heart attack. My younger sister started smoking at 12 despite her asthma, stopped once in mid twenties but our mother and friends apparently kept her socially smoking. She developed lung cancer at 41, died aged 42. Eldest of 6 was 17, youngest turned 4 the week before she died in 2014. Beginning of 2015 my 59 year old aunt died, 8 years after developing COAD. Lung cancer joined and she died within weeks. My mum got to 70, much denial with her, refused to check asthma changes, had seizure ... brain metastasis from lung cancer. Died 5 days later. Prior to them and among non smoking family, i'm on of 32 grandchildren on mothers side, no on else has ever had cancer, then or since. It can and does happen to people!

    God bless the medical professionals that specialize in the "less pretty" and overlooked areas of the body. Proper foot care is so important. We also have to be careful about what we put in our bodies. Good to know that there's a foot doctor 20 minutes from me that knows what he's doing! 👍🏿

      Sharon Varner They know. They just can’t stop. It becomes such a part of their lives. They literally can’t imagine how they would live without it. My father smoked for over 50 years when he died from bladder cancer caused by smoking.

      ever drink hard alcohol? beer? smoke pot? any type of opioid pain pills? eat sugar? caffeine? everything is bad in used in large amounts, shouldn't stigmatize someone just because their choice of self destruction is different than yours.

    I really hope the patient takes to heart to quit smoking. My Mom was a brittle Type 1 diabetic. I was with my Mom in her hospital room after she had had a below the knee amputation on her right leg. Her endocrinologist told her that her smoking was was why the amputation was done. Unfortunately this info didn’t faze her and she kept smoking until she died at age 60 on 12/1999. It was so hard for me to hear her doctor tell her this. He was a kind and very caring doctor. Patient, thank you for sharing your treatment. Dr. Jefferson, you are a tremendous care giver. During the current virus, everyone take care and be kind to each other! 🙏🏻

      Denise Bohnke - I know exactly how you feel. My dad was a smoker most of his life. He eventually quit smoking but it was ‘too little too late’. He was diagnosed with lung cancer at 52 yrs old. He also worked all his adult life in construction so he had the added contaminant of asbestos to the mix - a deadly combination. When he was diagnosed he was already considered ‘terminal’. He died at age 55. That was over 30 years ago. I’m 62 now so I can fully appreciate how cheated he must have felt. Dying at 55.... or 60. So unfair! We all we’re cheated of many many years.

    The Dr is 100% correct when he said, "the salon us the last place you should be." With this patient's issues he should be turned away from the salon.

    I think this gentleman's middle name is non-compliant. I'm sure when he had his leg bypass the doctors explained to him the importance of not smoking any longer, timely follow ups with doctors, and wound prevention etc. Waits a year at least to see a podiatrist after leaving nursing home. Doesn't take care to cut his nails or find someone to cut his nails. Knows he has a wound on his foot with poor circulation, on a blood thinner, and still waits a month to see a doctor for it. I don't know if this guy could be any more non-compliant. I do sincerely hope that he does follow up with you doctor and glad he did go to see you. Better late than never. Excellent job doctor! ; )

When he said those that didn't have family members coming to visit them, they just left them sitting without changing or taking care of them. 😢 My thoughts are always, "how would I want to be treated?" I hope I have a doctor like this! He's so good to his patients!!

      Many old people were jerks and their family members don't want to be bothered with them. I have an aunt with 4 kids, 9 grandchildren and no one goes to see her. She was a mean woman. Now she's in her 70s with deteriorating health 🤷‍♂️

    It amazes me that a person is prepared to put up with this mess, rather than try to stop smoking. I stopped about 30 years ago, after a bad chest infection. I’m so glad I did. A lot of folk think that medics are talking rubbish when they say how much damage smoking can do to the human body. Keep burying your head in the sand, smokers!

    Once again doctor you have shown yourself as a great physician and a wonderful person. You take time to educate your patients while treating them at the same time so that the patient is able to participate in their own health care. Beginning as a firefighter/emt, then to nursing, and finishing as an instructor I would have been honored to work with you.

    These are extreme cases. Not the "standard" of smoking.
    As a nurse I regularly see elderly people who smoke.
    And in 26 years I have never seen anything like this.

      Don’t point your finger. When a patient is in the hospital, family members are responsible for their loved ones’ nails. Nurses are instructed NOT to cut toenails. Ever. And because of changes in insurance, podiatrists don’t get paid to come in to facilities.

      @@Simbabe54 because he was there for the stroke their concern was to get him walking. most acute care facilities are under staffed so they only have time to do the bare minimum. but they should have notified his dr. about the condition of his feet. A wound care nnurse probably tried to keep it at bay but he has been home for a year without any attention to the sore on foot.

      Especially if they have circulation or diabetes probs, regular staff are discouraged doing foot care. Patients like this need to see a podiatrist for management.

      Right. That's what I was thinking. When my momma was in the nursing home they wouldnt trim her toes. I took in a small pedicure kit and they told me that I couldnt trim her toenails (I've taken care of her toes since I was 14) I looked at her nurse and said "watch me!" She had the cutest toes of the nursing home. Ain't no one telling me I cant take care of my momma!!

    I’m so glad I managed to stop smoking about 12 years ago now. I just suddenly woke up and said that’s it. I never thought I’d be able to quit as I genuinely liked smoking. Stupidly I didn’t have my first cig till I was 22 and that was me hooked. I escaped very fortunately as I didn’t develop COPD like my hubby nor did I develop PVD. If I’d smoked another 5 years I wouldn’t have been so lucky.
    It’s not an easy thing to do as you are dealing with both physical and mental withdrawal and unless you have smoked it’s very hard to understand why folks can’t just quit knowing how dangerous it is. I was a nurse practitioner and it sure as hell didn’t stop me.

      So I smoke and I can say I do not hate myself. For me there are several factors. I DO hate the smell, price, addiction factors (I've beat some major sht), the health factors (I work in nursing so I do see first hand effects). I do think about what I am doing to my body, my family. Not every cig, but alot. I am honest and I do try to be honest about my smoking. I dont like smoking around non smokers. I feel like I'm being offensive? I can honestly tell you this is one of the HARDEST things I have EVER quit. I'm 37. I quit both pregnancies. While watching the test sticks, I smoked my last cig. I don't know why I went back. Several reasons I guess. I do want to quit. I cant tell or explain why it's so hard. I feel almost juvenile trying to explain it.

    Thank goodness your patient came in when he did. With wounds like his, time is of the essence. I hope that he truly listens to what you're telling him and gets his health under control. Sometimes, it's easier said than done until there's trauma or someone to step in and put things in terms that he better understands/believes.

    I have had so many patients that have had vascular surgery and later amputations from smoking related PVD and venous stasis ulcers. They still smoke though 🤷🏽‍♀️

    Wow. That patient is the same age as me. I’m a non-smoker. My feet look like they always have - little change from when I was young. The health & condition of this patient is just sad.

    What has caused the striation on the big toe of the left foot? I find your work fascinating especially as I have to care for my 85 year old Mother’s feet. Which are in excellent condition thank goodness 🙂

    You're absolutely amazing and I love watching your videos. I'm a CNA and for me feet are a big deal. It drives me nuts when I see podiatrist just clip the nails and send people on. I love the fact that you clean up everybody's feet before you're done. I wish you're down here in Texas I'd take all my patience to you. Keep up the amazing work

    Thankfully, I never started smoking and don't plan on starting.
    Another miracle Doctor! His feet look so much better! I hope that wound heals okay too.
    I had two parents who smoked, took cancer (mom) and severe COPD (dad) for either of them to quit.
    Please stop before you are forced to stop please.

    My mom and dad are both early to mid-70s and have been smokers (not heavy at all) their entire lives. They still sometimes smoke, but it's far and few between. Their feet have always been well groomed. No thick nails, no fungus, good circulation. Pretty feet, actually. Nothing like this. So I think neglect plays a *huge* part in this as well.

    I enjoy how thorough you are. You also teach. I have learned a lot of good information from you. My husband feet are different from mine. He and I are different races.     This is a super professional caring, no-nonsense foot doctor who gets right down to work to help his patients without passing judgment. He doesn’t ask a lot of personal questions of his patients but does seek to find out medical issues related to what’s going on with his patient’s feet. In addition, he cleans up the dead skin and other debris on the feet and ends with a wonderful foot massage. If I lived in WDC I’d be seeing him all the time.

    Great job! And way to look out for the patient. That little toenail was amazing. I’ve never seen any that long. My little toenails are almost nonexistent. (Of course I keep them cut because they grow a little weird.)

      The lab requests as much specimen as possible. Sometimes test have to be repeated due to lab error, contamination, or an inconclusive result.
