    I’m a med student and I can’t imagine how you make time to record/edit/post videos… Much respect, I appreciate your effort on giving us content, especially for us med students to really prepare mentally when the time comes (residency) Lol… I’m actually watching this on my “study break”… now I need to go back to studying 📚 😫

    This is just sad to see. The brain is seriously affected of that amount of sleep. You get clumsy, forget things, memory is bad etc. Why would this be allowed? It's madness.

    24 hour shifts are not safe. Our medical system is broken. I don’t understand why this is not our number one priority to fix in this country. I have been dealing with my mom’s health care and it is hard and exhausting. Just getting medicine takes multiple calls etc. I appreciate you doing the work of becoming a doctor as well as documenting the good and pointing out the bad. Maybe we can all come together to fight for change in this area as it touches us all.

    I know you get shit for "crying all the time" (which is bullshit) but I can't thank you enough for continuing to be vulnerable with us. Plain and simple, you're a fucking badass. So to see you have moments of doubt, overwhelm, anxiety, and all the normal human emotions.. at least for me, makes me feel less alone. As a 44 year old full-time undergrad student (with a full-time job) I get overwhelmed, doubt my capabilities, and suffer from impostor syndrome at work AND school. You show us that you can have all these big emotions and still kick ass. I just have to continue showing up and trying my best.
    Our life paths are completely different but I can't stress enough how inspiring I find you. Thank you.

    Addressing the elephant in the room - having to work 24 hrs is insane??!! I just wanted to say my life is totally different from yours but I like watching your videos to see what somebody else’s life is like. Everybody is raised to believe that being a doctor is one of the best jobs but actually seeing what it’s like is surprising and very interesting. Wishing you the best with everything

    I feel for you, Rachel ❤ I agree that it's not a flex to be able to work a 24 hour shift, and it shouldn't have ever been normalized as part of medical school/residency training. If anything, health care workers should have more strict cut-offs for working more than 8-12 hours than any other profession. It's backwards. Stay strong, you got this, and keep on having your voice HERD 💪 ❤️

      I agree the fact that surgeons and any doctors who have peoples lives in their hands are expected to work for 24 hours straight is ridiculous and scary.

      I agree as well. Both my son and son-in-law are truck drivers,( hazmat certified) and can only be on the road so many hours and must get enough sleep without losing their license. You are delivering babies or worse dealing with the stress of a bad outcome in delivery with very little or no sleep, insane!

    That is wild. I cannot even imagine staying up for 24 hours, let alone staying up that long and also delivering babies and trying to remember all the very complicated things you need to know.

    I know it’s “how it’s always been done” but the 24 hr shifts are ridiculous. Learning rate decreases and mistakes increase. I just don’t understand it. It’s like hazing of some kind. Where do they require pilots to fly 24 hours straight when they start flying? Heck, no one would ever expect that. There are so many occupations where there is a max # of hrs that can legally be worked. I know you can make it through this but I just hope it doesn’t take a permanent physical and/or emotional toll on you. We have your back, Rafiki. ❤

    Remember, this is where you wanted to be at 4 years ago. Many people work hard and dont achieve it but you did. Take it day by day and before you know it, you will be board certified.

    You truly inspire me. I have been trying to overcome severe health anxiety for such a long time and as an introvert. The fact that you can be ON and interact with people, coworkers, patients that need and depend on you, it gives me a goal to work towards.     Rachel hope you get(got) some much need rest after your crazy/busy shift. Keep speaking out on the unfair and dangerous work schedules 1st year residents go through. Like you said it is dangerous for you as a doctor and your patients. Hopefully this will change. But in the mean time prayers for you as you continue on your journey.       Math is difficult too. I have a relative who earned a bachelor of science in mathematics. He used to stay up all night studying then go to classes in the morning (with no sleep.) I studied history, thankfully. I had to make an effort, but it wasn't nearly as difficult as math.

    I´m so sorry that the system works this way. It´s just not fair. We have a similar situation here and young doctors are trying to change that. I don´t know what my life is going to look like when I finish the school, but I also gonna be probably stuck at hospital for 5 years. Anyways, I´ve been watching your videos since we were both in the first grade, that means I´ve been watching you for the fifth year now. Your videos have helped me a lot because when I started the first grade of med school, my classmates were not very nice and it was a really competitive environment with everybody trying to look absolutely perfect and showing that to others. It helped when I could watch your videos and see someone dealing with stress, lot of work, not knowing every word form every textbook and just real. Then it got really better and I changed my class (not because of those reasons, but it was a perk) and met a lot of great people. Your videos have become a tradition for me, I guess I´ve seen all of them over that five years.

    I've watched a few of these as a doctor in my second year in the UK and it's still mind boggling to me that 24 hour+ shifts are a thing. The on call demand in the UK is generally quite intense with hospitals running at beyond max capacity most of the time so ours are capped at 13 hour shifts but even those can be so draining that further hours worked could/would be totally unsafe. I mean I sometimes get semi-delirious on my first night in a stretch of nights so I know there's a much higher chance of me providing suboptimal care in the last part of a 24 hour shift. Obviously there's a lot wrong with practicing medicine in the UK right now but I think relative to the US / some other countries, I'm thankful for our tight restrictions around working hours/shift pattern regulations. Fair play to you for looking so cognitively switched on (relatively 😅), good luck with residency!

    I am continuously proud of you for the work you do in the L&D unit as well as the work it takes to document, with such intimacy and honesty, the lesser known struggles of your field.
    Why is the 24hr shift a standard for the medical field? What is the reasoning behind it?

    I did a little research on internship, and the US is not the only country that does it. But, like you say, intense sleep deprivation is counter productive to a safe work place. Are they keeping internship around just because they had to do it? Even the hazing at Annapolis and West Point has chilled over the years.

    Rachel, you definitely put in the work!!! No one can you do not put in the necessary and needed effort to help new mommas delivery their bebes! I do agree with you, not sure who thought it was "cool," to make someone working as stressful and hectic and crazy a job as being a resident work 24-hr shifts... Can we please split that into 12-hr shifts????????? This woman is working her butt off and half the time not able to complete all the little things she needs to, and starts to feel zombie-ish towards the end of her shift. This can also affect patient care (I know you seriously pay attention to what your're doing when your in the moment!), and can lead to dangerous scenarios for mom, baby, resident, etc. Come on people, change/rework the system!!!

    I just got off my ob/gyn rotation for med school. I was tired but never got tired of it. We had one call day during the week and every other weekend. I would lose track of how many deliveries, too. It’s great fun. Do you know anything about switching to gyn only? That seems like the move after a while.

    My job in the Marine Corps does 24 hours on, 48 hours off shifts. Our sleep schedules are in the toilet. On our days “off” they call us in for meeting, field days, trainings, missing gear, and any other stupid thing. Luckily most 24 hour shifts, we can sneak in about 3 hours of sleep.

    Love your content. I’m in nursing school and already thinking about medical school butttt…. being as tired as you are is not normal. That’s like a pilot trying to fly a plane on no sleep. I can barely study with no sleep let alone deliver a baby safely lol.

    Eat when you can and sleep when you can. 14 years in EMS throw a PowerBar in your doctor's coat. Also try liquid IV it helps rehydrate you extremely well.
    Also a 20 minute nap will do wonders. Then get back up and chart.

    Absolute lunacy. I would've been dead after the 13th hour. This is absolutely not safe whatsoever. No human should have to work these insane hours. Either way, you were amazing and did such a good job lasting throughout that shift.

    Im thinking about going into OBGYN but I don't think i have the mental strength for it.

    I don’t think it’s a flex either. I think it is dangerous - opening up medical errors that may life threatening to patients because doctors are overly tired. I admire you so much and have been a long time follower. Just know you’re killing it ♥️

      Right like they made regulations on the hours because a patient died one time due to residents being so tired. But then they capped it at 80 hours…. Like wtf does that do? That’s still double a normal work week!

    Now I know why L&D MD notes are not as detailed as ICU MD notes. If you don’t have time to eat or rest how can you possibly have time for documentation. Thank you for giving me some OBGYN perspective.
    Our hospital does not focus on OBGYN and we probably deliver in a day as a whole about the same number of newborns you did as an individual. Great work Dr. Southard.

    In my opinion, no doctor should be working a 24 hour shift at a hospital. There needs to be a law that states doctors cannot work more than 12 hours straight, like police officers. Even in the military, just standing duty for 24 hours was exhausting, and that was just sitting at a desk & walking around the barracks every hour or so to check on stuff. Can’t even imagine working 24 hours straight at a busy hospital. I just think hospital administrators could do a much better job of rotating people in and out throughout the workday, residents included. Just because the system has abused residents (and attendings)for decades doesn’t mean the practice needs to or should continue. 😡

    Idk why, but your videos make me want to pursue this as a career. But, I am also at awe at how much time you spend working. I would not be able to handle it as well as you do. So, I hope I can later find something that I am passionate about or that I enter a residence program that doesn't overwork you. I really enjoy that you do surgery+medicine+also use microscopes. For some reason, I really love working under microscopes, but I do feel intimidated when its Histology. I should be in medical school next year hopefully, so I still have time to think through what to do as a doctor. Thank you for sharing your experiences with us. I never thought I would enjoy L+D if it weren't for your insights, but again, I am not making a decision any time soon. Edit: But, girl how can you drive after all this? Don't you feel afraid that maybe one day your sleep while driving or maybe not pay attention due to sleepiness and crash yourself or into someone else? I will send you my prayers🙏

    Hola how’re you.
    When I had my first born 34 years ago in 89 I was watching a scary movie (I was a day past my due date) when I jumped and my water broke. I was calm and I slept pretty much the whole night. I got up at 5 am showered cause I was told to come in by 8 am the next day. By 9 am I was in the elevator walking no less and by 9:41 my Beautiful daughter was born. There was no doctor no nurses just my hubby and I in the room when I told him I needed to push. He panicked and said Nooooo and he looked down and saw that her head was already out. He ran out of the room screaming and some doctor that was walking the hallway delivered my Baby 😁. My regular OB said why didn’t you wait for me ??? Lol
