The importance of reporting near misses in ionising radiation

HIQA has published an overview of the lessons learned from radiation incidents reported to us in 2019.
During 2019, we received 68 notifications of significant incidents, which is a small percentage of significant incidents relative to the total number of procedures, such as X-rays and CT scans, taking place in Ireland. This short video explains what a near miss and significant incident is, and why it's important that organisations report them to us.
