This happens when the VT controller fast forwards the wrong VT. The VT controller isn't always in the gallery so they can't always communicate besides saying they have cued the video and it's ready to go. When the ITV1 / STV LOGO comes up it's the ITV and STV presentation taking over until the producer says they can take over again.
Take note that STV briefly showed the ITV1 breakdown card before cutting to theirs.
