Buprenorphine - Suboxone

  • Opioid addiction affects an estimated 2.5 million people. Some triumph over their problem by abstinence, through organizations typified by Narcotics Anonymous or by redirecting their energies to a new passion. For many substituting less dangerous drugs for oral or injectable opioids reduces the potential for danger.
Suboxone and Subutex and their generic equivalents contain buprenorphine with or without naloxone. Most tablets and films are meant for daily application under the tongue or on the mucous membranes of the cheek. A monthly implant exists. The drug ranks among the most frequent prescriptions in the United States.
The buprenorphine component tightly sticks to and only partially activates the brain’s opioid receptor. This effectively prevents other opioids from fully activating the receptor and providing the feeling of euphoria. Topically applied naloxone offers no significant benefit. But it dissuades abuse of the buprenorphine. When injected naloxone displaces opioids from the receptor resulting in potentially severe withdrawal symptoms.
Suboxone and Subutex function as well as methadone but without the inconvenience of requiring daily attendance at a clinic. Buprenorphine may be dispensed in sufficient quantities for a month or longer. By law, physicians must be specially licensed to prescribe buprenorphine and may treat only a limited number of patients.
Treatment begins after mild withdrawal symptoms appear. Originally the first several doses were meant to be applied under direct supervision of a healthcare practitioner. An integral part of therapy involves education and counseling. Adherence to these rules no longer seems steadfast.
Originally treatment was intended to last up to a month. Experience quickly taught doctors that relapse with early discontinuation exceeded 90%. Even slow tapering over a year or so may be associated with a 25% failure rate. The underlying susceptibility to addiction remains lifelong and not uncommonly demands prolonged treatment.
Misuse of the drug especially Subutex appears increasingly common. Some doctors prescribe large quantities knowing patients will sell the excess hopefully using the funds to pay for return visits. Unfortunately addicts may use the money to acquire illicit drugs. Buprenorphine is frequently smuggled into prisons and used by inmates.

I was on opioids 26 years prescribed by my doctor . my employer decided they wanted to fire me when i was hurt on the job so my doctor stopped giving me pain killers as soon as he learned i didnt have my ge caddillac insurance policy anymore. It changed zero i was still in pain and am still in pain today. I cannot find a doctor anywhere in my city that will help me but they are giving suboxone to just about every person i know who smokes crack. I repeatedly try to get help and all they do is give me excuse after excuse. well you dont have mainecare you have a working mans insurance policy you dont have an addiction problem you have a pain problem. then they try to sell me diabetes meds my blood sugar was 128 when they checked it right after breakfast. people wonder why other people end up dead. doctors are literally fueling the illicit drug trade by refusing to prescribe or refer people in need any type of help. they are killing people off by refusing them help.

IDK where you are in Maine, but there are programs down in southern Maine that help. IMAT programs are through Maine Medical Partners, Scarborough, Westbrook, Portland. There's a new program in Maine that will cover your meds and appointments, as long as you get the meds filled at Maine Medical Pharmacy. If you want the number, I'll check back for a reply here. I didn't have Maine Care either when I first started. You can get help. You just have to search for awhile.

One would think the true victims (you and I) would get millions! Nope! There’s the Class Action Suit that if you do the math will get us $200. I went from prescribed pain killers to Suboxone. I had NO IDEA what I was getting into! I’ve even stopped cold turkey. That got me into the ICU for six days. Then I went 41 days without Suboxone. I thought I was going to die. Almost no blood pressure and felt like death for 41 days. I finally went back to the prescribing MD. “You can’t cold turkey Suboxone, it can take years to taper properly.” Funny how he never told me this BEFORE he prescribed it to me. My self employment insurance doesn’t cover Suboxone so I get to pay $800.00 per month, going on five years now. The drug gave me chronic constipation. Yesterday, I learned I have stage two colon cancer. Suboxone literally killed me. Yes, I guess Suboxone has saved the lives of chronic heroin users. But I could of just gone to a 30 day rehab or done anything but take this poison. Then I find out the prescribing MD gets a cut of the Suboxone prescription. Evil at it’s best....

Buprenorphine will out compete naloxone for the mu receptors so if you're dependent on another opioid & were to try to inject suboxone it ain't the naloxone that's gonna bite you. I'm surprised by how many prescribers know so little about this stuff.

Yes, the naloxone is both outcompeted and not bioavailable if taken as prescribed, but even still the buprenorphine will outcompete the naloxone for the mu and kappa receptors due to its incredibly high binding affinity and long half-life. Thanks for sharing this.

@@gristamshackleford2102 nope, mothers little helper was diazepam which first came out in the sixties as a substitute for barbiturates. It was marketed as non addictive so doctors would prescribe it profusely. Particularly to women who they thought were prone to a nervous disposition because of their hormones or maybe a cold unloving husband or delinquent kids.
Dexedrine which was also profusely prescribed in the sixties to women wanting to shift a few pounds, got names like Purple Hearts and in the U.K. particularly they were known as Black Bombers. When subverted for recreational use the pills were crushed up to destroy the ingredient that made the drug time released, giving the user a rush of energy, heightened sex drive and general euphoria. Bombers, dexies, Purple Hearts etc were very popular amongst mods and rockers at that time. Later in the U.K. illegally manufactured amphetamine sulphate and amphetamine hydrochloride became widely available. Both were habit forming, the hydrochloride was especially potent and could be dangerous, long term use causing great harm.

I bought methadone pills from a friend of mine /approx 10-30 mg per day depending what she was able to save for me. Then I was fed up, Resigned. Moved to another EU country AND I am officially on methadone 40-50mg pd. I was and am about to switch to Sub. IS IT REALLY SO GOOD FOR PAIN, /I also had seizures Im on diazepam 4 mg pd/ but methadone is sooo good for pain and mood but but I can collect only a week dosage and suboxone maybe the long term solution. How is it?

Trouble with getting off a long-term dose with this where am currently is that they only reduce in 2 MG'S! Not 0.2 etc when the dose gets lower! Due to funding. Otherwise when came off it before didn't find it too bad tbh
SICK of it now and just glad (touch wood) never abused it (apart from taking too much of it), but it does give a buzz! Almost like some sort of Opiatey Phet tbh.
My cheeks look like a beetroot after being on Subutex (not Suboxone) for nearly a decade + my insides feel wrecked. Been told by various GP's etc that many stay on it until Death! '@ the end of the day' though at least it's not such a grimey life as Gear and been able to get some sort of life back.

I don’t like this guy. I don’t like the way he talks about addicts and dismisses the effects of withdrawal. He has no idea what coming off a high dose of methadone is like. He’s probably a hardcore Republican. My father is a doctor who works with addicts he has a far kinder bedside manner and doesn’t come across as a judgemental egg head. I would not like to be treated by this man. In my opinion drugs should be decriminalised. Portugal is a great example of this can work well. I also just don’t like the way this man talks. Naloxone is a cruel additive. This man really strikes me as a kind of Mengele type, the way he views people, human beings as lab rats, nothing more than fascinating experiments. He comes across as devoid of kindness and compassion for people with the disease of addiction. How lucky for him that he’s a privileged, straight white guy with obvious intellectual ability.
