What? 13 Disastrous movie productions and The Conqueror (1956) isn't on the list? For Shame! Maybe part 2 then? It should actually be at the top of ALL lists.
1977's "Sorcerer" was wrecked by director Friedkin's refusal to kowtow to original star Steve McQueen's request to give Ali MacGraw, his then wife, a role in the movie. At the time, McQueen was the most popular movie star in the world. McQueen then bowed out of the movie. The two other stars originally scheduled for the movie, Lino Ventura and Marcello Mastroianni, then also left. Mastroianni avoided starring in English language movies but had made an exception for "Sorcerer" to be in a movie with McQueen. He left when McQueen left. The casting changes caused a months long delay in production, delaying filming the river crossing scene to when the river was almost dry from a drought. "Star Wars" was released before "Sorcerer" in June 1977, and it grabbed all the audiences. Friedkin's career never fully recovered from the failure of "Sorcerer" at the box office. But seen on its restored blu-ray release, "Sorcerer" looks great. Too bad Steve McQueen could not star in this movie.
Try looking for the making of The Island of Dr. Morgan, with Val Kilmer and Marlon Brando. There have been two documentaries made describing the chaotic shoot. One female actress tried to flee production and was found at the airport and was forces to return to the set.
The Island of Dr Moreau was a bomb, Brando weighing 400 pounds and Val Kilmer temper tantrums were a disaster, so were their fights and disagreements on the set, two huge entitle egos too much for any studio. As you well pointed out one of the guys from production stated in one of the two documentaries that Brando and Kilmer were placed there to mess things up, very sad, it could have been a good film, I myself ended hating the flick too.
The munkins in the wizard of oz actually were completely unruly and basically drank, smoked and did whatever they wanted on set, they caused so many problems. This is why their time on screen was so limited. They were never hired again by any productions and the actors made due with other gigs.
The munchkins were little savages, Judy Garland stated they even tried to touch her private parts and make up with her against her will. Luckily they were removed from the set for most of filming until they had their parts to play, then they were fired, it appears these guys thought they could do same thing they did in their country.

Heaven's Gate MUST be #1! After the town set was built, Michael Cimino decided that the buildings on either side of the street were too close together - by 6 feet. So he had EACH SIDE of the street moved back by 3 feet.
For Star Wars the big special effects shots involving the spaceships were so behind schedule because they were basically inventing the technology of motion controlled cameras, building the computers by hand to control and program them, and allegedly the special effects crew did not have the strongest of work ethic. Lucas was always disappointed with the special effects although the audiences didn't seem to mind the finished product! Lucas was so stressed out he said that he suffered a heart attack during the production. Luckily, he was able to recruit his wife Marcia away from another production to help with reediting the film and the result was a much improved product!
Actually you're wrong about sets built in London for Cleopatra not being used. Some of the interior sets were used in Carry on Cleo (1964) a comedy send up of the Cleopatra film of 1963. In fact I'll go further, the blue and gold armour which Richard Burton wore in a barge scene about half way through the film was also worn by Sid James in the Carry on film as well as a number of other unused props. Richard Burton and Sid James were good friends, I don't know all the intricacies, but somehow Burton allowed Sid James to wear the chest piece for his film. If you don't believe me, go watch both films.
The Passion of the Christ. The actor who played Jesus Christ was accidentally hit with a whip when the whip was supposed to be hitting a piece of plywood but it hit him in the back. Also in the crucifixion scene he was hit by lightning and was electrocuted. Fortunately the actor survived. Jim Caveizel was the actor’s name.
Several actors swore to never work with James Cameron again after _The Abyss._ They refuse to speak of the shoot. However, without _The Abyss,_ we wouldn't have gotten _Terminator 2: Judgment Day_ or _Titanic,_ and the CGI revolution in film would look completely different.
You forgot to mention with "Jaws" that Roy Schiender, who hated doing the movie, was difficult, and only did the second "Jaws" because he was contractually obligated to.
Filming The African Queen was pretty messy as well with ants & illness, I've read. P.S. I got a good chuckle from "Flaws"! I've never heard that one. 👍
