    MICRO-BOLUS! You are much better than the OP5 at bolusing. I used to bolus huge amounts, as omnipod suggested before a meal and I would either drop way low before I ate, or I would miss the high glucose. I bolus about half what OP5 says, and after eating for 15-20 minutes I check and bolus according. There is an indicator right above and to the left of your main NUMBER which says, IOB. That means "insulin on board" or how much insulin is in your bloodstream. (estimated). Use that number, with the main mg/dl number PLUS how much food you think you're digesting or will continue to eat. If you're at a party you may be bolusing for hours for example. You will eventually learn to balance how much food and how many carbs you are digesting with how much insulin is "on board" . Thanks Meg! just adding what I've learned. No doctor taught me any of this! 🙂

    Glad you are learning to “work” your own system referencing your own needs. I haven’t had the success I was expecting with a new algorithm system. I am having to switch back to a more traditional pump system at my own expense. I hope to make my first video soon about my own journey with the “algorithm nightmare “ system. My drug plan makes the Omnipod system financially untenable for me. Thanks again for sharing your strategies!

    I was having a similar issue with my insulin to carb ratio. On TslimX2 with Control IQ, the system would not reduce the bolus if you were over target but had IOB, it would just give you the exact amounts for the carbs (unless you need a correction due to insulin on board, and then it would give you a correction regardless of what it predicted your blood sugars were about to do), which is what my body was used to and needs. What I found with Omnipod 5 is, if I made the insulin to carb ratio more aggressive (1:17 instead of 1:20 on Tandem) the amount that the Omnipod gives me is closer to what I would get with TSlimX2 when I was over target but have IOB. Sometimes a little more or less, but only by 0.15 units in either direction.

    These videos are so so helpful!!! Thank you for sharing your experience! I do have a question about how you bolus for foods that you traditionally would have used an extended bolus (ice cream, pizza)? Is it just about doing more boluses in alignment with your digestion and blood sugar spikes?

    I’m so confused with the corrections- I overide the system all the time even though omnipod 5 says “try not to overide”
    I don’t understand how my range is set to 110 and I rarely ever am at that number? I’m consistently 150-200 befofe eating and then hang out around 270-290 for hour and hours. If I didn’t over ride and correct aggressively I’d never get the bS Down

  • @yl8901

    Hi Meg, I am confused by you saying that you learned that you need to correct and correct early. What you mean by that? When my son bolus for a meal he waits 15 minutes and it is still spiking so I have to wait until the automated mode brings him back down. Where does a correction come in place and how do you do that? Also the "The Maximum Basal Rate sets the upper limit of any basal insulin rate you can use
    while in Manual Mode." so this does not work while in automated mode. So far the only method to lower the spike that I was able to find out is you have the correct carb/ratio and if you pre bolus and wait perhaps longer that what I have him wait.

    Great info. Speaking of the algorithm. If you’re trending low, do you record your uncovered carbs into the pump? It will say 0 insulin, but the pump knows we are putting carbs in the body.
    I’m playing around with this to see if it helps with some on my son’s lows. Is there any validity to this?

    Hi Megan! A few questions. I read below about “reverse correction” - what is that on the Omnipod 5? Also you said you increased your insulin to carb ratio. What was it before vs what you increased it too? Do you also pre-bolus like 15-30 mins before a meal still or do you enter everything right before you eat?
    Thank you!!

    • @megtypeone
      @megtypeone  2 роки тому +1

      Hi! Reverse correction is basically if you are below the target glucose, it will suggest you take less insulin for a meal bolus. So if my target is 110 and my blood sugar was 90, it would take some off the bolus. And I’m sorry I don’t share my ratios on social because I don’t want people to compare, but I did do a 45% increase on the 5 (I said 30% in the vid- I did math wrong lol!) and yes I still prebolus when I’m able to!

    • @Ss.Stormyyy
      @Ss.Stormyyy Місяць тому

      A lot of endocrinologist offices have seen an increase in T1D cases in younger kids since COVID, unfortunately, due to it triggering the antibodies for T1D. Your body gets confused and attacks the pancreas cells that make insulin, therefore causing you to slowly stop prdocuing insulin. @bestjobdaddy6736

      @@megtypeone Hi Meg, really enjoying your videos, and loving your friendly energy! I'm having trouble finding information on the "correct above" setting for the Omnipod. My understanding is when your levels exceed the setting (e.g., 150), the PDM will suggest a bolus amount, but my experience has been that whenever a CGM reading exceeds that level (e.g., 180), the calculator always shows 0. I understand that could be the recommendation, considering IOB and future levels prediction, but with no indication otherwise, it just seems like its not working at all. Can you tell me what should be happening? Should I hear a beep or something when that "Correct Above" setting is exceeded, so that I know to look at a 'suggested' bolous . . . if its working that is. If it is working, is the correction bolus delivered automatically or must I initiate delivery? What I've been doing, when I notice levels above the setting, is go into the calculator and enter my reading, or "Use CGM" (I don't enter any carbs), and let the calculator recommend, then I initiate, or not. But it seems I could do that anyway, without the "Correct Above" feature/setting. As you can tell maybe, I'm confused. Can you 'unconfuse' me?

      I have heard a lot of concerns about this as well. I am waiting to get the Omnipod 5. My strategy since I have been on the Dexcom G6 is to start calibrating after the warm-up at about an hour or two in. I’ll do finger pricks and calibrate as necessary throughout the day if needed. By the second day, the G6 matches pretty close to my finger prick readings. Once I’m comfortable with the readings, I trust the G6 for the rest of the time I wear it. This is what works for me. But of course, go by how you feel and calibrate often if necessary. If its’s totally off I recommend calling Dexcom.

    I always get confused on the IC ratio . . . When you “increase the IC ratio” does that mean, for example, that 1:7 would now be 1:8 or 1:6? What about increase the correction factor? Does that mean, for example, 1:50 would now be 1:60 or 1:40? I do not know why these two things always confuse me . . .

    • @megtypeone
      @megtypeone  2 роки тому

      Hahah yeah it’s super confusing. As the ratio “increases” (like I’m saying I do), the numbers actually get smaller in the ratio. In your example, 1:50 would now be 1:40! It’s giving you more insulin for less carbs/value about target.

The Omnipod 5 System is my first pump but have been on insulin for eight years and oral medication before that time. Did a great deal of research before I started with the Omnipod 5 and have been very impressed with my numbers. I agree that on the automated mode the system needs to learn and that happens over a few pods. Interaction as you discussed speeds up that learning process. Thanks again for your efforts in creating these excellent presentations!

    When you said you increased your insulin to carb ratio, do you mean you went up ? For example, my son insulin to carb ratio is anywhere from 22-28 carbs. So I could go 26-30 carbs?

    • @dwightl5863
      @dwightl5863 Рік тому +1

      Insulin to carb ratio is the amount of insulin to control the number of carbs eaten. Basics here: If the ration is 1:15 and you ate a single slice of bread (15 carbs) you would administer 1 unit of insulin. If you ate a sandwich (2 slices of bread) you would administer 2 units of insulin. In your question a 1:30 ratio would result in 1 unit of insulin to cover a 2 slice of bread sandwich Divide the ratio (30 in the 1:30 ration) into the carbs (single slice is 15 carbs which would equal .5 units of insulin). 30(ratio) into 30 (for a two slice sandwich) and get 1 unit. Keeping it straight can be confusing. I'll sometimes use an extreme ratio to get my thinking straight. Ex: A 1:100 would mean 1 unit of insulin would cover 100 carbs which is ridiculous so therefore a lower ratio 1:30, or 1:23, or 1:15 are more realistic to lower glucose levels and give you the right direction to go to adjust a ratio.

    • @russellseaton2014
      @russellseaton2014 2 роки тому

      The insulin pause is when the pump stops giving basal insulin after you give a meal bolus insulin. The pump will not give any basal insulin for one to two hours after taking the bolus. The pump conflates the purpose of the two insulins. Bolus should only be for clearing meal glucose out of your blood. Or a correction bolus clearing out leftover meal glucose. Basal should be for maintaining the background glucose levels to keep your body running. The pump thinks insulin is interchangeable and all the same. So it thinks if you have meal bolus then you do not need background insulin (basal) to keep your body running.
