I think he had a broken foot during the famous hell in a cell match with Mankind, and he had to work with a broken orbital bone during the time he wore the mask, etc. Back then you had to work with injuries. If you took time off to heal you would lose your spot. I also remember Kurt Angle had to work with a broken/fractured neck.
