Im 12 weeks out from my TKP. I want to mention that I am a heavier set woman so getting the bend like the therapist wants me to is proven to be a little difficult because I do have the extra added weight in the thigh area in my leg area and just getting that bend is proving to be a little harder but right now I'm at 95 so I'm doing the best that I can I'm going to keep pushing it trying to do what I can I just wanted to put it out there about heavier weight people might have an issue with getting the bend they couldn't get even before they had the surgery but not giving up going to keep doing my best.. just discovered your videos and I'm following them..thank you for doing them..
Thank you for watching and sharing your comment. I agree that sometimes extra body tissue in the thigh and around the knee can be an added obstacle to achieving optimal range of motion. Something you can do is see how much range of motion you have in your non-surgical knee and use that as a decent target for how much range of motion you can aim for in the surgical leg. I am 6 weeks after the surgery and having somewhat success, yet every day anew it gets hard and very painful to start these exercises. Of course I also do the other exercises like ankle pumps, quad sets, etc. I am 81 and should lose weight but having a hard time with it. 2 weeks post TKR of my 2nd knee. I HATE going to PT because of the machine they have me use. Today I actually sobbed from the pain. I know you need to push for results, but it's making me not even want to exercise at home. These are wonderful, especially how you say to push when you feel discomfort/resistance.

If the machine/exercise you are doing is too much then try to problem solve with your PT to find other alternatives. I’m four weeks from TKR surgery, and my therapist has me doing about a dozen exercises (two sets of twelve repetitions twice a day, seven days a week). My “step-down” exercises are not just uncomfortable, they hurt. Sometimes it seems all I ever do is my PT exercises and sometimes my knee is stiff and swollen from all the exercises. I feel I may be overdoing it, and I’m trying to find a good balance of what works for me. BTW I’m almost 70, if that matters. I’m considered changing therapists. Thank you, and thanks for the video.
Almost 4 weeks from TKR. I go for my first outpatient Physical Therapy today. I have been working hard to be ready. I can bend over 90° to 109° last time in patient therapist was here. Thank you so much for giving of your time and talents to share that others might learn. I know I will be using this particular clip for the future.

Are instructed by your surgeon not to bend the knee or are you physically unable to do so? If you flexion is significantly limited I recommend starting very simply.. lying on your back or seated performing heel slides
Thanks for watching, most cases I do 10-15 reps and usually 1 or 2 sets of each, first about 2 weeks after surgery usually recommend 3-5x per day, 2-6 weeks maybe 2x per day and then taper down to 1x per day most days of the week... Because these are range of motion exercises and not stretching or strengthening, there really is no specific guidelines for volume... I recommend using pain / symptoms as your guide. Some pain is to be expected but if you start to experience moderate to significant pain then you may be doing too much, or if you have significant increase in pain / symptoms the next day then you probably did a little too much and should decrease your volume some (and/or intensity). Hi... please advise if i should continue with the exercises as u hv demonstrated.. i have yet to try wall slides.. i am now 5 weeks after tkr surgery right knee. Can i sleep on my stomach? A bit scared with my scar!
You can definitely try all of these exercises, but be sure to follow the guidance of your medical team (surgeon, physiotherapist). Most individuals are able to sleep on their stomach, but a lot of people find this position to be uncomfortable. You may feel better if you place a pillow under your lower legs to give your knees a slight bend in this position.
These are great, thank you. I'm a year out and at 125 degrees but I want more. I've been told 125 is the max. Is that true? Is it possible to fully bend a knee replacement?
It is possible to in some rare instances to have greater than 125 deg but very few people after surgery will get much more than this. Also, being a year out from surgery you'll have to contend with scar tissue that has developed within the knee joint... Lots of time and consistency with exercises and stretches and you may see some increased range but definitely not guaranteed.
Absolutely! Cycling is a great form of exercise for many reasons including range of motion of the knee, aerobic health and conditioning, muscle re-education of that alternating pattern similar to walking, and more.
I’m 4 weeks post TKA and I’m bending consistently about 125-130. I’d like to get it further but I feel a sharp pain in the front off to the medial from center. It is on the tibial side. I’d love to get another 5-10 of flexion but I’m wondering if this is unrealistic. I will add that I have loose joint syndrome (not Ehlers Danlos I don’t think). Can I continue to get more and if so, for how long before I reach my limit? I’m doing all the exercises that you show here although I don’t require the band anymore.
It’s not unrealistic to gain another 5-10 degrees, but also keep in mind 125 degrees is usually the target that most patients shoot for and you can accomplish most everything you need with that level of mobility. Continuing to stay consistent with the exercises and giving it time, it’s not unrealistic to continue to see some improvements still
I have TKR before 3and half months before.I have no range of motion in my right knee,try best with therapist but couldn't improve more than 40 degree angle.can u suggest any remedy
If you are continuing to have limited range of motion I would consult with your surgeon. You may require greater intervention than physical therapy such as a manipulation under anesthesia (MUA), debridement surgery, or a revision knee replacement. Your surgeon can take x-rays and provide specific advice for you.
I'm 7 weeks post TKR surgery. These exercises for me very painful but I endured and now I'm having a great result in the bending of the knee.. I will continue to do them daily as the knee is stiff. From your comments below that seems ok to do?
I'm glad you are seeing some improvements... yes you can perform these exercises daily, just be aware of your symptoms and pain. It is important to find a "happy medium" of doing the exercises enough to see improvement and benefit but not too much to exacerbate your pain or symptoms.
When you say to hold that position for about five minutes, is that a good idea for someone who still has a little bit of swelling in their knee. Wouldn't this cause more swelling? Or is the swelling worth it to be able to hold this for a few minutes?
5 minutes is just a general goal. Each person's tolerance for holding the different positions will be different. As a whole, though, I recommend starting with a much shorter duration and increase slowly over time as tolerated.
I am 6 weeks post op TKR and swelling after exercises is normal so what you can do after exercising elevate your leg and ice it for about 10 minutes, I bought a TENS little machine and using it around your knee and ice packs for about 10 minutes it helps with pain and swelling

You can continue to perform these exercises. There is potential to still improve your bend but it can be harder and take longer to see noticeable improvements.. consistency is the most important thing.

Thank you for watching, I totally understand the increased difficulties with having both knees replaced at once... There is no set time for when you should or can try this. It just depends on when you feel comfortable with standing and placing weight though the leg. Listen to your knee pain and symptoms as your guide.

Hi: all these exercises u have outlined …r they supposed to be performed immediately after surgery. As some appear to be quiet difficult and intense …ie..lunges etc. Ur input would be greatly appreciated. Thanks
No, some of the exercises you will be able to start perform the day of or next day after surgery, but others (such as lunges) you may not feel comfortable doing until at least a few weeks out from surgery. Thanks for asking!

You can get greater than 125. I’m 128 just 6 weeks out TKR. I know someone who is 136. It’s all up to you!
