Spinal stenosis treatment - Prolotherapy or surgery?

    Would this be the same if you have bulging discs that are showing pressing on the nerves on the axial MRI view?

    I am 61 yo and have central canal stenosis at L4 L5 due to spinal stenosis. I have bilateral sciatic positional pain just as you described, causing painful numbness in my right peroneal nerve region and right glute, and, at times, loss of proprioception. I had a spinal cord stimulator placed August 2020. The neuromodulation helps significantly. Previously, I could only walk/stand for about 5 minutes and then I would have to squat/ sit down to relieve them. Now I am able to walk an hour or more. However, the first 30 minutes of every walk I endure level 6-8 pain right sided symptoms which will subside to level 2-4 after 30 minutes and remain at a low level the remainder of the walk. Would proltherapy be of help to me?

    I had an MRI done and some of my C units are showing stenosis in the results. Some are 6mm and others are 8mm.

    I would go for arthroscopic removal of the defect part of the disk, thats the future 😁😁
    Local anesthesia, 7mm incision, no complication, no blood lose. You get back your life!
    Those ligaments and muscles you can do exercises since you will be pain free..

    My recent MRI report says L 5 stenosis. I have just the opposite symptoms, than what you describe. After sitting for 1/2 hour or so, I can have terrible pain in the left leg/back. Walking doesn't seem to give me any pain. The pain goes away when I lay prone for 15 to 20 minutes, and I have almost no pain at night while sleeping. Thanks for your thoughts on prolotherapy. I will investigate it.

      You've described my symptoms to a T. I'd already had a diagnosis showing where the nerve is pinched. It's, as the doctor described, a dynamic issue.
      I can only drive for a few minutes before I have to hunch over. This is the first time I'd heard of prolotherapy.

      Hi Victoria, Depends on what they are being used for. Epidurals are not Prolotherapy. One is used for temporary relief and one is working to regenerate tissue strength for long-term pain relief.

    I wish I could see you. The doctors I've been to haven't been able to make connections with whats going on in my body. I have Cervical spinal stenosis with bone spur and disc out of place. I am having so many problems. You seem to know so much more.

    Is this generally covered by insurance?

      Hi Burdette, Most patients with private insurance report to us that they received at least some reimbursement from their insurance. It is not across the board one way or the other as some people do not get coverage for one reason or another and others receive 80-100% coverage. It's like any other medical treatment- if insurance doesn't want to cover it, they will find a way to not cover it. Thanks for the question.

    I'm a 23 year old male and based on a recent CT scan at the ER, my doctors suspect I might have moderate to severe spinal stenosis. I had sciatica start when I was 19, and as of this year it's become unbearable at times and makes it hard to complete daily tasks. Right now I'm waiting on an MRI follow up. My questions are:
    1. What causes stenosis in someone as young as myself? I've always been an active healthy male.
    2. If it does turn out to be stenosis, would less invasive options as these be worth exploring before jumping to surgery?

      @@anthonyveney6946 Dude, make sure to double check with your doctor, or even a neurology specialist. Turns out they gave me a misdiagnosis. And, from a patient perspective, the symptoms are identical. I've had a slipped disc all these years and it just got worse. Had surgery last week and I'm getting my life back.

      @@anthonyveney6946 Damn dude, that's intense (mine was lower back so it wasn't as severe).
      I'm not at all a doctor, but my understanding is:
      Spinal stenosis is essentially arthritis of the back. It's much more common in elderly patients (like 60+), but not impossible in younger people. The vertebrae grow bone spurs that push into the nerves/nerve canal.
      Slipped disc is when one of the discs (the cushions in between the vertebrae) are basically squeezing out a bit and pushing into the nerves/nerve canal. It's very common in all age ranges, and typical after a high stress/impact accident. (Car crash, contact sports, a fall, or improper lifting).
      Again, I'm not a doctor so make sure to research yourself, ask your doctor every question you have, and always get a second opinion if you don't feel comfortable.
      When you say displacement I'm assuming you also have a slipped disc in your neck. Typically the first course of action is bed rest and prescription meds (they prescribed me oral steroids to see if it would help with healing). Depending on your symptoms psychical therapy might be recommended. However if it's been multiple months surgery might be the option. (I actually know someone who just had to have surgery for a cervical slip and he did really well).
      Just make sure to not do any activity that aggravates the pain, heat and ice the area, and ibuprofen is really good for helping back/neck pain.
