From the title I wouldn't have realized how *very important this subject could be* to a lot of people. You should consider doing a "Part-2" going over the Who, What, Where, Why, and When of the entire process scenario. From who would need to do this, to why it's important, where to get and submit these forms, when and how to submit and when to expect a response, and who to contact...etc., and any other pitfalls that may arise and how to possibly overcome them.
I love that idea. I'll probably do that. I like making helpful videos and so I appreciate you watching and commenting even though this video doesn't pertain to you.
I have 7 or 8 more of these already lined up, they go over other important forms. So while those release I'll try to build on your ideas that you mentioned about the big W's, etc and get those videos made...thanks Shotelco!

Question? If the father lives in another country and he has no email, no social media and I don’t know his address what would I be able to put on the form.
I just came across, I am about to turn in my forms & application because moving on in life, I just never had the chance to get court orders as my child’s father disappeared and it’s been YEARS! I hope they understand 🤞🏼

What if I forgot the exact date I divorced, and lost the document that will support my divorce ? Can I skip that portion on DS-11?

Would this work if the Father of a minor won't communicate with us at all. He Doesn't want to do anything with his daughter since she was born. We don't even know where he lives he don't give us any information like that and he blocked us in social media. Can we get the minor a passport with all this circumstances? Or do we still have to go to the court?
You typically need two-parent consent for passports and international travel. In my case, however, I won full legal and physical custody of my daughter so I am allowed to get her passports and travel with her without permission from her mom. Idk your situation so it’s hard to say what path would be best for you

It's a strong likelihood and my advice is to include as much supporting documentation as possible. In my case, my divorce decree stated that I have physical/custodial rights with joint custody with his father. I text him multiple times and sometimes he responded and other times he was unresponsive. I did not have his address and he refused to provide it. Finally, I got a lawyer.

I have full sole custody of my child; however, Dept of State is asking for DS-3053 or DS 5525. At first, the father is not responsive, but he eventually responded back that he will only sign the DS-3053 with 2 conditions. Should I include all those details on the DS-5525 form? What should I write down under "Result" section?

1)Whose address needs to be given under section 5 (statment of consent). I guess the person who is giving his consent has to give his address?
2)Do we need to send orginal notarized form or scanned copy will also work at passport office?

Are you sure you're using the correct form? DS-5525 is used when you cannot obtain a notarized letter from the other parent and you can't prove they consent to a child's passport...

You can leave items blank, and also you can attach an additional sheet of paper to add any info or explanations
I got divorced over 10 years ago. What if I have no clue what my marriage and divorce dates are?
i would do my best to find that info. worst case scenario you can always attach an additional sheet of paper and let them know what's going on and why you can't get that data
Can you please help me I have 3 children under 16 and the father was issued an order of protection in 2009 and I haven’t heard from him since 2011 last I heard he was deported in 2018 all attempts to communicate with him are unsuccessful . Please help

Yep, just double check all the info is filled out correctly, and if you look at the couple of pages that appear before the actual application, they will inform you what documents/protocols you should follow for your appointment :)

Provide evidence of that and you can include any info on another sheet of paper

If you dont know their email, you can leave it blank. But provide as much information as you do know. And who are you trying to contact, and for what purpose? Sorry, I'm a bit confused about that part...
@@NickDemski Nevermind. I was able to get a passport without the other parent’s consent. I just left parts I don’t know blank but I had to provide as much evidence to prove that I really did not have their information.
