Very good but like everyone else the field 9 for other names was glossed over. Do they want just your last name so if you were borth john doe, and changed to john smith do you put just smith or do you put john smith. It is as clear as mud.

Hey my pleasure to help! They require black ink and for it to be in big block letters, I don’t know why the type of pen would matter much as long as it writes well and legibly

If you've never gotten a passport, you should use DS-11 and provide the documentation showing you are am American citizen. I also have a video about how to prove citizenship, if you need help with that you should watch that video 🙏

@@NickDemski No I didn't. Since it was issued before I was 16, and expired decades ago, the way I understood the rules, it was not necessary to report it as lost?
Correct, if it's already expired, it doesn't need to be reported as lost or stolen. So to handle that section, you can simply add an additional sheet of paper and inform them that you can't remember that information given the situation...

Any idea where to put suffix? some people say after first name, wouldn't that show on my passport as "John III Smith Doe" on my birth certificate it shows "John Smith Doe III"
@@NickDemski Thanks for the reply but my name shows as example "John Smith Doe III" but there is nowhere on the passport application to put the suffix III. I don't know why passport applications don't have a box for the suffix because I don't know where to put it, contacting the passport agency for help by phone just has a robot answer saying they are experiencing high volume and then they end the call, can't even wait on hold. Email is only a robot response linking me to passport information that I already know, not a single mention of suffix. I searched everywhere in google and some people say put it with the first name and some people say put it with the last name, if I put it wrong then it's just going to get rejected.

Don’t glue it. It should be stapled in a specific way. But you could wait to turn it in and ask the passport officer about how to do that when you turn it in to them. I also have a video about passport photos that might answer any more questions
The passport I applied got misprinted and the error was from my side. They want me to refill a whole new ds11. The question is on bottom saying have you ever been issued a us passport? In a status what I am supposed to write. Please help
they told you to re-apply with DS-11? Usually data errors are corrected with form DS-5504 and it's much simpler than getting a whole new passport. Are you sure you're supposed to re-apply with DS-11, who told you that? If you are supposed to use DS-11 for some reason, then I would yes to your question and include the data you have...but again, I'm thinking you should be using DS-5504
@@NickDemski I did apply ds 5504 at first then they returned it back to me saying I have to reapply evething again since the error was from my side and they can’t help me with anything. Basically they want me to pay.

In that case, i would indicate that, yes, you've been issued a passport, fill in the relevant data, and I'd even attach an additional sheet of paper that notes that your initial passport was issued with mistakes, you've tried to correct it thorugh DS-5504, and was told to re-apply for this new passport

Hey Kurt, I prefer capital letters for clarity and to avoid mistakes. Legibility is the most important factor, however. As long as your mailing address is also your physical address and not a PO Box or something like that, then it's okay to leave that section blank.

What if you are not a hundred percent certain but you at least have the correct State and birthday for one parent.?
You should include as much info as possible. If there’s anything you need to add or clarify, you can attach an additional sheet of paper to the application and include it there

Any idea where to put suffix? some people say after first name, wouldn't that show on my passport as "John III Smith Doe" on my birth certificate it shows "John Smith Doe III"

@@NickDemski Thanks for the reply but my name shows as example "John Smith Doe III" but there is nowhere on the passport application to put the suffix III. I don't know why passport applications don't have a box for the suffix because I don't know where to put it, contacting the passport agency for help by phone just has a robot answer saying they are experiencing high volume and then they end the call, can't even wait on hold. Email is only a robot response linking me to passport information that I already know, not a single mention of suffix. I searched everywhere in google and some people say put it with the first name and some people say put it with the last name, if I put it wrong then it's just going to get rejected.
Gotcha! You can always attach an additional sheet of paper with the information and explanations you want to include

My wife is a muslim woman who wears a head scarf will her photo be acceptable at the appt. The Walgreen passport store said it was. Please confirm if you can.
Religious stuff is allowed, but she may need to provide a written declaration with it stating it’s for religious purposes

If I took my own passport photos and had them printed, should I take those with me for the section, “Stop Continue to Page Two” or should I staple/glue it on beforehand?
Don’t glue it. It should be stapled in a specific way. But you could wait to turn it in and ask the passport officer about how to do that when you turn it in to them. I also have a video about passport photos that might answer any more questions
The passport I applied got misprinted and the error was from my side. They want me to refill a whole new ds11. The question is on bottom saying have you ever been issued a us passport? In a status what I am supposed to write. Please help
they told you to re-apply with DS-11? Usually data errors are corrected with form DS-5504 and it's much simpler than getting a whole new passport. Are you sure you're supposed to re-apply with DS-11, who told you that?
If you are supposed to use DS-11 for some reason, then I would yes to your question and include the data you have...but again, I'm thinking you should be using DS-5504
@@NickDemski I did apply ds 5504 at first then they returned it back to me saying I have to reapply evething again since the error was from my side and they can’t help me with anything. Basically they want me to pay.
oh bummer sorry to hear that! In that case, i would indicate that, yes, you've been issued a passport, fill in the relevant data, and I'd even attach an additional sheet of paper that notes that your initial passport was issued with mistakes, you've tried to correct it thorugh DS-5504, and was told to re-apply for this new passport

just write your name as it's legally known and as it should appear on your passport...do you use a hyphen?

that's the confusing part so you just leave mother/father/parent without circling which parent it is? so like just put my mom and then dad without saying which one it is

Also, by the permanent address, it says complete if residence is different from mailing address, my mailing address is the same as my permanent address, so can I leave this blank? Or would it not hurt to fill it out anyway?

Hey Kurt, I prefer capital letters for clarity and to avoid mistakes. Legibility is the most important factor, however.
As long as your mailing address is also your physical address and not a PO Box or something like that, then it's okay to leave that section blank.
