hi sindhu... pericardial fat pad lucency is seen, whereas middle lobe consolidation will be typical soft tissue density/ with without air bronchgram and proper silhouetting of the RT cardiac border. a lateral radiograph may also help though its seldom done nowadays, patients usually end up with a CT. ..thanks for watching
