The way I see it is that Tenma is the protagonist while Johan is the antagonist. But unlike most other stories, the main character isn't the protagonist but the antagonist is.
The fact that everyone who watched this anime says at some point that Johan was right tells how manipulative he is. We all know he is the main antagonist in the anime but still even I at a point thought Johan was right. Still gives me chills how perfect this anime is.

His hungry monster only wanted to remove yohan's name from existance but yohan's sheer will had caused him into killing all the people he did just to slow down his monster. At the end tenma killed the monster inside yohan and saved yohan, so yohan had no reason to kill anymore. So in the first part of the anime he was pure evil, middle part neutral and ending anti antagonist.

The story arc of Richard Braun was, for me, the most heartbreaking in the whole series. Just had to say when I saw in the vid Johan holding out a whiskey bottle.
One of the lines from TV series The Alienist summarizes what I want to describe of Johan (might lacking accuracy but it means the same): I thought I found a serial killer, but I only found a hurt child 😢
