New RNs Going Directly Into ICU

  • This video is a commentary about brand new grad RNs going directly into ICU. Whether I think it's a good idea or not. Can it be done? Should it be done?
    I know I commented on a previous video that I wouldn't be doing such a commentary on other specialties (besides medsurg) but I explain why I still did a ICU commentary.
    ...because I like to choose violence!!! xD
    no, seriously, I think I have good comments on it and can provide a valuable pov.

    I appreciate your viewpoint. I graduated January 2023 and went straight into an ED. It was pure chaos at first. I enjoyed the fast pace but hated the fact that I had such a limited time with the patients. I’ve since moved on to a telemetry unit which I enjoy. I’m still learning a lot. Working night shifts gives me time to comprehend what is actually happening with my patients. More time to digest their chart as well as direct care. Building confidence has been the hurdle to overcome. I earned my license at 50 so my viewpoint on life is a lil different than many of my fellow nurses.

      a fellow 50yo nurse grad!! yes, the other unit types do offer that time to really do "nursing". ER is just triaging chaos like you said. 😅

    Great video. Not even a nurse, just a lowly EMT, but my friend just graduated earlier this year and went straight to a trauma ICU. He lamented about the treatment, all of what you are saying came true. He struggled to learn in that environment because of the pace and the resentful teaching, unfortunately at the end of the orientation he was sent to another unit. I feel for him because I know that he could excel in that environment, and I’ve seen him grow so much in the time I’ve known him (from struggling to speak English to graduating nursing school). It just takes time. Thanks for all you do!

      i'm so sorry your friend had such a bad experience. it CAN be a very rewarding experience. most old time ICUers love their work but the same people can also be guilty of the same complaints we're discussing. 🙁
      that is doubly difficult when the language isn't native! but that's amazing they were able to get thru nursing school AND basically get thru orientation. it's no shame in that btw. just had a very experienced RN go from progressive care to ICU and also washout. it just wasnt what he was expecting or they were expecting. it's not for everyone (definitely not for me! 😅)
      and you're not "just" an EMT. it's a very good prep trajectory, e.g. my daughter was EMT for 2 yrs prior to her RN. frankly, she misses it a lot and is considering truck RN after finishing ICU.

    I agree with you if we're talking percentages of new graduates entering the medsurg versus ICU. Medsurg is the backbone of nursing, hence the percentage of nurses joining the unit is substantially higher. If you ask me, ICU is completely different from Medsurg in that it is much more concentrated and requires a thorough understanding of lab results and how to handle various ailments that may arise. Medsurg is generally what you learn in nursing school; it's nothing special, but rather the foundation of nursing. I believe new grad nurses should choose whatever they believe to be capable of handling. In your video, you mentioned the word "resentment," which aligns with what I'm stating in your other video. Do I feel that all new grads should pursue specialties like ICU? No, although I have seen some new grad nurses achieve success in ICU.
    Like always keep the video's coming and great to have this dialogue.

    There's so much wisdom in this video! I love the analogy of learning to fly a plane with passengers in it. ❤ brilliant. ( lol, I need a video on teaching my kids to drive without losing my mind! Lol )

      btw i reserve the actual teaching of passing the driving test to professionals! 😅
      i just start them out to get comfortable with the car and practice what the instructors teach later. but the driveway circling is absolutely true and does ease them into it. 😉

    Hi! I'm a new Grad that went straight into the CVICU, it was defiantly overwhelming but I'm thriving. I got to shadow and floated to tele medsusrge, and during nursing school & I disliked because it was just a lot of task (getting coffee for ppl constantly, ppl keep trying to get up, etc...) , and less what I do in the ICU. Also, my ICU, after we got off orientation, they give us basically tele patient in CVICU and it has been excellent.

      yup, a lot of what you said was why my daughter didn't even try to go to medsurg first and gunned for ICU. definitely overwhelming but as i keep reminding her "it's rewarding and great training!"
      ...yeaaaah, so she's GenZ so i'm sure everyone here can picture her facial reactions to my comments and assurances. 😅
      but it is tremendous opportunity, experience and training upfront for new nurses! so congratulations on getting into ICU and good luck! 👍

    I am glad that i have a float externship. I am seeing how med surg fits with specialties and why the foundational patient care is so important and actually doing work beyond clinicals. That said... I am aiming for the OR.

      @@olddadnurse all I know at this point is I am not a fan of the emergency department but am completely enjoying the neuro unit (so far). My units for float are ED, ICU, rehab, ortho, cardiac step down, and neuro step down.

    There is a reason for med Surg experience... SAFETY. This should continue to be the requirement. If one is reall serious about nursing then one shouldn't mind putting the work in with acquiring the necessary skills to do so with ANY and ALL specialties. I don't understand WHY new nurses are so eager to work specialties and forgo the actual experience of nursing. We couldn't even apply to a speciality without med Surg experience. Healthcare is so ridiculous at this point moreover nursing is becoming a joke and not patient focused 🤦🤷🙆

      exactly my point. but i understand their eagerness and also the argument that some nurses definitely can handle the pressure and intensity.
      is it something i recommend still? nope. not even for my own kid. but i won't dissuade people from pursuing their dreams. i just caution that it doesn't function in a vacuum. there are lives at stake so they must take it seriously.

      @@olddadnurse Yes however eagerness shouldn't supercede safety via experience. I don't understand what happened to the checks and balances that didn't allow this to happen previously. This is about human lives.
