Should be in English supported by other documents. If the name is written in a language other than English, it must be accompanied by a certified, word-for-word translation that meets ECFMG requirements.

Sitting for AMC after 5 years of graduation is not a suggested approach, specially concerning changing global economic situation.
You will be required to complete related processes before your AMC exam.
