    Did you rule out thyroid tumor in this case. If years after ablation pt develops cancer would you take responsibility of that cancer which was not treated effectively. Is there any research on recurrence as mng is now an antibody mediated disease and it is bound to recurrence.

    I have several nodule both sides and the middle plus others. These three are the ones my doctor is worried about. One is over 3 cm the other two are over 2 cm. Is this something that might work for me. My husband is Against surgery.

    I had a steroid injection (for poison ivy that was on my neck )in July and I had a red flush on my neck after the injection. I had covid in October. I had low TSH in November. I was diagnosed with mutilnodular goiter and I have the antibodies for Graves. My blood work normalized very quickly even though I was on the lowest dose of methimazole. Has there been a case that RFA treated Graves? Could reducing the nodules with RFA help? Is any research being done on this?

      Graves involves the entire gland and primarily effects the non-nodular tissue. So, unfortunately there is no role for RFA in Graves.

    Dear Doctor Keith, I am in Australia and I will be having a Telehealth with Professor Catherine Sinclair who has introduced RFA in Melbourne and now Sydney with whom I will have a consultation with her in the next few weeks as I only became aware of this procedure. I had already initially booked to see an endocrinologist that doesn't do RFA) only surgery, thought I would get his take on this. I do have a benign 4.7cm nodule which occupies the entire right thyroid lobe, my TSH has been normal. I do experience compression and lump in my throat. I did mention RFA to this endocrinologist and he feels that an RFA Specialist would not touch this, and he recommends the only solution down the track is having a hemithyroidectomy. Can you shed some light on this? surely RFA could help with this? I need some hope as I don't want to go down the surgery path. thank you

      Sounds like you got some bad advice. You are an ideal candidate for a RFA! Save your thyroid!

    Dear Doc im in Denmark europe day after tomorow is my surgery but I really dont want.     Can several large benign nodules be treated at once? And does Medicare cover this procedure?

      Yes, several nodules can be done at once. There are some patients who require more than one treatment if there are multiple, large, bilateral or substernal nodules. Currently, Medicare does not cover this procedure, even though it is FDA approved. Typically, it take a while to get that coverage through. It will be a covered procedure at some point in the future - because it is actually cheaper than surgery. So, Medicare covering it would save the taxpayers money!

      Thyroid function is not affected so if you have normal thyroid levels before- you will after. (The only exception are toxic nodules - which we intend to ablate and thus restore normal function)

      Yes, if the adenoma (bad parathyroid) is visible on ultrasound and can be reached with the RFA probe

    Is this a process that can be done on a benign nodule located on the thyroid isthmus, not on either lobe?

      RFA works great to treat benign nodules in the isthmus! Extra care has to be taken as frequently these nodules are closer to the skin surface so there is a small risk of skin burns. With careful technique, these nodules can be effectively treated.

      Treatment costs vary widely as there is no standard costs. Many doctors will discuss price after seeing how extensive the problem is. Large, challenging nodules can take a lot more time to treat, so costs vary. Talk with the doctor in your area about your specific situation.

    I don't have a definite cancer diagnosis, but a suspicious nodule on my thyroid. How do I navigate this gray area I find myself in? I'd love RFA therapy over surgery. I wish there was a way to diagnose cancer before having to go under the knife.

      RFA is a great treatment for benign nodules. So you need a fine needle aspiration (FNA) that is either benign or indeterminate with genetic testing that suggests a benign nodule. If the ultrasound is suspicious- then you need an FNA. If the FNA results, particularly if the genetics are suspicious then RFA would not be appropriate. I’d be happy to look over your individual situation and discuss with a telemedicine appointment, but can’t offer any specific advice here. If you haven’t had the genetic testing, check out the Afirma or Thyroseq testing options that can help many folks avoid unnecessary surgery.

      @@entallergydoc1 I really appreciate your thorough reply! I've had 2 FNAs. The first was benign and I was so happy. My doctor told me to get a follow-up ultrasound in a year or two, so I did. The nodule grew by 25% which warranted the second FNA. That came back indeterminate/suspicious. Surgery was recommended. My endo said the genetic testing wasn't necessary and wouldn't be helpful when I asked. It's all so confusing. I understand that you can't give me more information without knowing the details of my case and I'll look into those genetic testing options you provided. Again, I'm so grateful for your time. I'm learning a lot.

      @@daniis.444 Thanks for asking! I'm doing well now. I did in fact need to have a surgery (partial thyroidectomy) to get a definitive answer. Thankfully, it was never cancer to begin with. I'm glad the other half of my thyroid can make the appropriate hormones so I don't have to be on supplements for the rest of my life.

    Dr. Please I need some help I’m lost .. I am suffering from HYPER symptoms since Year ago and I have done tsh 3 times first time it was 1.60 uIU/ml within ft4 1.03 ng/dl then I redid THE TESTS ONCE AGAIN tsh - (0.70) uIU/ml
    Free t4 - ( 0.98) ng/dL
    Anti thyroid peroxidase tpo ( 0.37) IU/ml
    Anti thyroglobulin abs (0.55) IU/ml ….. I got an ultrasound it was clear that I’m dealing with nodule into my left lobe will this treatment help me ??

      If the overactivity of the thyroid is from a nodule, then RFA works great for that!

      Results are permanent in most cases. Insurance does not cover it but will soon as it is safer and cheaper than surgery.

      Typically, the RFA procedure stops the growth of the treated nodules. Long term effects are all positive - smaller, non-growing nodules while preserving the working thyroid tissue.

      "Ill-defined" can represent a broad array of findings. Sometimes, it's just changes within the thyroid that aren't truly a nodule or mass. Other times, it can be used to mean border irregularity which is a higher suspicion feature that would require follow up. Sounds like you need a follow up with a thyroid specialist!

      Small thyroid nodules may not require any treatment as most are asymptomatic. RFA is a great option for small, hyperfunctional nodules or any size nodule that produces symptoms.

    I have recurrent papillary carcinoma. One lymphoid is on a major artery plus it’s of scar tissue from five previous surgeries for this reason. I was recommended thyroid ablation but feel this will not be sufficient enough. Any advice? I was diagnosed last March 2020 and was finally scheduled this week due to waiting for the equipment to be purchased by Yale hospital. No one told me that I’d have to wait a year with cancer and that they had to purchase the equipment. I just don’t know if the risk with having surgery for me is worth it.

      RFA is FDA approved only for benign thyroid nodules. Off-label, RFA is being used for recurrent papillary carcinoma. You need someone very experienced with RFA, not someone who just got their equipment. I might suggest a short trip to see Dr Ralph Tufano or Dr. Jonathon Russel - my colleagues at Johns Hopkins. Both have the experience and expertise to properly assess and treat your situation.

      I’ve used RFA for similar situations- but I wouldn’t want someone new to RFA doing an advanced case (if they just got their equipment - sounds like they are new).

      If your hyperthyroid with a toxic nodule or multiple toxic nodules then RFA works great! It would not work for Graves’ disease.

      This treatment is not available in Ivory Coast. Unfortunately, I don’t think that it has made it’s way to Africa yet. Italy has several doctors with extensive experience.

      Costs vary widely depending on location and difficultly. Many insurances are now partially reimbursing for the procedure, but full coverage is sadly lacking. I've seen prices anywhere from $5k (Kentucky) to more than $20k (Beverly Hills).

      Yes! In selected cases. A good rule of thumb: if the cancer is eligible for active surveillance then RFA is a good option. If it’s an aggressive cancer or a large cancer then surgery is the best option. I should do a video on this as the evidence for its use is substantial.

      If the source of hyperthyroidism is coming from an overactive nodule (a toxic adenoma) then this procedure works great! Also, a toxic multinodular goiter can be treated as well. Unfortunately, the most common reason for hyperthyroidism is Grave’s disease - which can’t be treated with RFA because it involves the entire thyroid gland.

      Yes! It works really well for toxic adenoma because it can ablate the overactive tissue within the adenoma and leave the normal parts of the thyroid gland completely intact. Because it doesn't damage or remove normal thyroid tissue, I think RFA is the best option for toxic adenoma treatment.

      @@entallergydoc1 I really wish I could meet you. I had hf my thyroid removed for a nodule. We left it up to the dr if he would do all or half. Once in surgery it was so large it grew under my collar bone so he only took half. We thought that the same could happen to the other side and now it has. I have had a biopsy a few years back and will have an ultrasound next but starting to feel pressure and it bothers me everyday. I really really do want surgery.

    I went through RFA and after the procedure i had a voice hoarseness it's already 3 weeks past still my voice have no improvement, is there any possibility that my voice will recover? The findings is left vocal cord paralysis.

    I have a nodule that is benign and slow growing. This time the nodule showed as more solid and vascular. Before it was combination solid/fluid/non-vascular. Could this procedure be done on a more solid/vascular nodule? My endocrinologist advised its stable and to recheck later this year in November. I asked about this RFA option before my ultrasound/ultrasound results and she advised since my nodule is benign that she doesn’t recommend at this time. I just wanted to know your thoughts on this. My nodule doesn’t cause me any symptoms and my blood work always comes back normal.
