Is there a way to avoid white patches in the subjects face when doing this? Or is it just the makeup?

Not really as it's very hard, direct light. Good makeup will certainly help, or you could always remove it in Photoshop. I hope this helps!

In a nutshell, add lots of contrast, without blowing out the highlights too much and then crunch the blacks a little to wash them out. I hope that makes sense!
