Wow! Am I glad I stumbled onto this video! I had seen other videos from Dr. Devgan comparing two different brands of lenses, and another that said he would choose monofocal for his own eyes. Now I know exactly why he said that. As a 70 year old who flies RC planes as a hobby, I'm looking for the clearest distance vision possible. Wearing glasses for closer distances won't be an issue since I've been wearing them for the last 35 years anyway.
This is the best info. I originally thought I would go panoptix but I'm glad I went mono yesterday. I had a hard time getting the right explanation of each. I didn't see this video but it gives me piece of mind I went the right way per your explanation. I wore glasses all my life. My glasses are providing so much safety from tree branches and different cleaning sprays that double back to hit the glasses. I want to wear glasses for the safety features. I still tinker around at 80yrs old. I can't even notice I had my first surgery yesterday except for the clear vision compared to my other eye. All my friends had the mono lens and are very happy. Mentioning pilots and perfectionist was all I had to hear.

I have seen a number of videos, yours is the absolute best. Really providing insights for what is meant by halo, etc...a nice "So how do you choose?" questionaire I wish you were here in Seattle washington, I would go with your company
Thank you for this! I just had first Cataract surgery & none of this was completely explained. I was asked to make a choice by checking a box on a paper. I chose to stay nearsighted because it is what I am use to. A nurse did show me what my near vision would be if I chose to see distance. All my life I can read things up close without glasses . Rather have what I know.
Thanks for sharing this info! I wish he had showed what it would look like to choose a monofocus lense set to near vision. My biggest hobby is reading, and I don't think I would be ok if I could never read a book again without wearing glasses. Could you explain a bit how your vision has turned out after surgery? Are you happy with it? Even though you chose near vision, is your intermediate vision still liveable without glasses. Do you need glasses for everything outside of reading or just for driving? I would be so grateful for any info you could share!
I am getting scheduled now and have been contemplating which lens to get, I have watched numerous videos on this and can now finally say I know what I want all due to this one video. Thank you!
@@jenniferkeefe5842 sorry for the late response, I picked the long distance as my right eye can see close distance well and combined they switch back and forth in my brain automatically so it’s much like normal sight. If I get a cataract in my right eye I am pretty sure I’ll get a close distance lens for that eye.
Thank you for the best information on the IOLs. It, especially the last part of "so how do you choose?" does help me to choose the very IOL I need for cataract surgery.
Thank you so so much, this is the most and best detailed education on cataract/ IOL premium lenses I have seen. The graphic pictures explains everything. I wish you were in my state. I kept watching this. Great education.🙏🏾🙏🏾🙏🏾👍🏼👍🏼❤️❤️/🇯🇲/🇱🇷
I borrowed one of Uday Devgan 's book about cataract surgery before I had my cataract surgery a couple of weeks ago. It was very helpful and informative. Also I watched this video a while back which helped me decide which lens to get. Thanks Dr Devgan.
@@mypluto1234 I got the up close vision mono lens. I have been near sighted my whole life and did not want to lose that vision. Can see very clearly out of that eye now. Before the cataract made my vision very blurry close up and in the distance. Surgeon said my cataract was very thick and needed to removed.
The optical transfer functions for each lens type are documented. I would think a computer simulation with VR goggles could easily be made to allow a user explore an environment and switch between lens types to see the differences. Try it before you buy it.
Thank you, Dr. Devgan. Your explanation is so clear and makes a lot of sense. I love it ❤. Many doctors just do not offer other lenses beside the monofocal ones, including mine. Hope I live near your office.
Thank you for an excellent explanation! Now I know why my cataract specialist recommended to me the monofocal lens! As a surgeon and a photographer i need a perfect vision and clarity. I would rather use glasses for reading than loosing that vivid vision.
As a surgeon did you choose to have your lens set for clear near vision? Or did you pick distance and wear glasses to operate? I'm a veterinary nurse and I'm hesitant to set my natural sight to distance because all of my work requires good near and intermediate vision. I'd be so grateful for your Input!
@@ashleyc7287 I have “chosen” distant vision and to bear glasses for near. I miss my near vision dearly. Like you I didn’t realize that 90% of my work is in near and intermediate vision. I should have chosen that and to wear glasses for driving. Make a good discussion with your cataract surgeon before you decide!
Very helpful....but what if I want to see up close and intermediate, then wear glasses for distance? Also, is there a lens without a prescription added but just takes the cloudiness away?
This is what I want to know! I'd much rather glasses only for driving and distance tasks. But I can't find any information on what that would look like. I wonder if anyone can see this and comment their experience?
Doctor thanks for the wonderful explanation. I am planning for cataract surgery. Can you please give your opinion on eyhance. They say it is a monofocal lens with enhanced inter meditate vision and hence give the same vision quality as normal monofocal lens and not an edof. Can I go for it.
Now I get it. I was going to go with the vivity, but I am a perfectionist. So my optometrist's suggestion of the monovision distance was better. Thankyou! I have worn trifocals since 5th grade, could not adapt to contact lenses. I tried about 30 kinds. Also I want to see stars at night.
I went with PanOptix had I seen this video b4 my surgery I’d have done mono focal I can tell I’ve lost clarity yes I can read better but not sure it’s worth the loss of clarity plus I’m out 5K not fully healed yet but I may still need readers anyway so what’s the point!? Thinking mb there’s an incentive for the Dr to upsell!
I ran into a problem. Two months ago, I went trough cataract surgery in my right eye. After discussing the type of surgery with my cataract surgeon, he recommended doing it with laser because it is more accurate surgery. However, for 3 weeks I was not able to see even few inches away because of a very dense fog in front of my vision. Subsequently, my cataract surgeon referred me to a retina specialist due to developing Systoid Macula Edema. After the fact, I was told by retina specialist that most likely the systoid macula edema was the result of trauma to my eye. My question to you : does cataract surgery with laser can create unwanted side effects Or damage to retina ?
@Sun Shine Thanks for posting this. Not medical advice, but I did a quick search and YES, Cystoid Macular Edema (CME) MAY be associated with Femtosecond Laser surgery, according to a 2015 study titled "Cystoid macular edema after femtosecond laser-assisted versus phacoemulsification cataract surgery" found on PubMed conclusion: "Increased CME might be a subthreshold retinal injury safety signal after femtosecond laser pretreatment and warrants further study." J Cataract Refract Surg. 2015 Nov;41(11):2373-8. Thanks for posting this, I am going to reconsider. Always been concerned about high energy lasers and the retina.
Thank you so much for your helpful video. I was recently diagnosed with early stage glaucoma and am extremely near sighted with moderate astigmatism too. The final eye measurements are yet to be done. I may choose a toric lens to correct the astigmatism, but I don’t think the trifocal lens will be an option. However will mention it to my doctor as well as mono vision lenses. You didn’t mention mono vision lenses here so maybe 🤔 you do not care for the option. It’s all so much for the patient to absorb! Thanks again!
Hi Dr 15 days ago I had Vivity implanted in both eyes. I see perfect cell phone, pc, tv, but the lights both during the day and worse at night cause halos and glare, I would like to know if that goes away with time or is it permanent?
What kind of lens implant did he get? What is happy about with them and what does he not like about what he got? I’m going to have to get cataract surgery and don’t know what kind of lens to get. Thank you
One option you didn’t discuss is mono vision where one eye is corrected for near and the other far. I would like to see what the vision ranges are with that option.
This is a wonderful video, but I wondered if you had a similar presentation for choosing a multifocal near vs intermediate vs distance. Current suggestion from surgeon is to set one eye near and one eye intermediate. I'm wondering what ranges those each get me. I will need glassed from where to where in distance?
@@UdayDevgan Thank you! Have you seen EDOF lenses used for near and intermediate rather than far and intermediate? Do they provide clear image like a simple monofocal would? Are there any issues with glare/halos or other optical artifacts?
Dr. Devgan, I have watched this amazing video several times as I am planning to have cataract surgery at age 82. I am wondering how accurate the comparisons are between mono-focal and tri-focal implants. Are these exaggerations needed to show differences, or or these differences truly realistic, especially the night comparison?
Thank you for your superb content! I am due to have cataract surgery soon to fit a lens in my other eye having previously had a successful monofocal implant! Please advise whether you have had good outcomes using the Alcon Vivity EDOF lens and what is the minimum distance at which I should expect sharp focus.
Thanks for the informative video. I have pretty much decided on the monofocal lenses. My question for anyone who has gone this route, do readers do as good a job for varying close distances (0-3 feet) as a near sighted person wearing progressive lenses would have?
Thank you for this excellent explanation, the best I have found yet. I am devastated to learn I have a cataract in my left eye, especially because I do not need glasses for any purpose at this time. So the thought of now having to wear glasses the rest of my life is overwhelming. Devastating to think I will no longer be able to see my cell phone or my computer screen without glasses now. I will be talking to surgeons about mono or mini mono options. The first surgeon I consulted said I am not a candidate for the PanOptix lens because I have too much astigmatism. However I have lived my entire life without having correction for astigmatism so it’s a difficult choice.
@@mypluto1234 yes, I decided on single vision lenses with a monovision approach. I have always been nearsighted, and I don’t mind staying a little bit near sighted. But I wanted to be able to see my phone and computer screen without glasses. And that is what we achieved. My left eye is for seeing up close, and right eye for distance. My right eye did not achieve the target we were aiming for (still too nearsighted) so I wear a contact in my right eye for distance, and have about 20/40 on a good day. I have a pair of glasses if I need to drive at night. What I have learned is that no matter how hard you target a certain eyesight afterwards, you may or may not actually hit the target. Luckily there are some tweaks after the fact, such as wearing the contact in my right eye. I will add that I did develop posterior capsule opacification in my left eye, and just had the YAG laser treatment done about a month ago. Super easy. My up close vision is about 80% restored. I wish it was 100%, but it’s better than it was.
@@maryanngrisham5182 thanks for sharing! If you dont mind my asking, how bad is the distance vision in the eye set for near vision? Are you able to see intermediate and function fairly normal without correction? Or is everything blurry thats not reading distance?
Bravo! Outstanding lecture, both technical and easy for anyone to understand, Doctor Devgan! I wonder if you could authorize or care for subtitles in Portuguese soon! My patients would love it here in Brasil! Again, bravo! Congratulations!
A very good presentation Dr. Devgan. I would, however, like to understand why the Vivity lens is rated higher than the Panoptix, as regards quality of vision, as shown on the graph at minute 6:11. From the info I have found, the Panoptix has a max MTF at around 0.4 whereas the Vivity has a max MTF of about 0.25. Also the Panoptix maintains a higher MTF over the entire defocus range. This MTF measurement would seem to indicate greater contrast available with the Panoptix, which would provide better vision than the Vivity. If you could shed some light on this it would be much appreciated.
This is an excellent explanation! I was struggling to understand what my future vision will be like after cataract surgery. Iam mid 50 s and have been using reading glasses for about 6 years. Now I need to have cataract surgery in only one eye after an auto accident. I realise I will still need reading glasses after the surgery. Watching this video i think monofocals are my best choice. In your opinion is s this right @Dr Devgan? I don’t expect to have cataract surgery in the other eye for another 10-15 years . As a result, will I have issues with double vision, imbalance etc ?

I wish you were here in Seattle washington, I would go with your company
Sir your video has helped me to take an informed decision on my choice as I am planning for a surgery next week
That's about the most detailed and fair explanation I've come across.
Same here.
I concur. I wish I had seen this before I had my first eye done.
@@ashleyc7287 hi. Did u choose monovision?
I am getting scheduled now and have been contemplating which lens to get, I have watched numerous videos on this and can now finally say I know what I want all due to this one video. Thank you!
What did you decide? I'm painstakingly trying to confirm a decision for myself as well.
I will have Cataract surgery next week. This is a very good decision maker Video
Thank you so so much, this is the most and best detailed education on cataract/ IOL premium lenses I have seen. The graphic pictures explains everything. This is in 2018, hope you have a more updated video which I know will be high end. Thank you again Doc. Am about to get cataract surgery, I choose panOptic ( that will be the trifocal ). I wish you were in my state. I kept watching this. Great education.🙏🏾🙏🏾🙏🏾👍🏼👍🏼❤️❤️/🇯🇲/🇱🇷
.same choise thats one month but i cant read yet are you like me after one month
I borrowed one of Uday Devgan 's book about cataract surgery before I had my cataract surgery a couple of weeks ago. It was very helpful and informative. Also I watched this video a while back which helped me decide which lens to get. Thanks Dr Devgan.
How did it go & what kind of lens did you choose ultimately?
@@mypluto1234 I got the up close vision mono lens. I have been near sighted my whole life and did not want to lose that vision. Can see very clearly out of that eye now. Before the cataract made my vision very blurry close up and in the distance. Surgeon said my cataract was very thick and needed to removed.
It’s the best explanation ever proved with pictures
You are excellent
Thank you so much for your time and effort ❤❤❤
Is there a way to test out the different lenses in advance by using a special contact lens that will simulate the experience of the installation?
Yes wouldn’t that be great maybe by year 2050 ! 😂 just like eye drops to dissolve cataract! 🎉
The optical transfer functions for each lens type are documented. I would think a computer simulation with VR goggles could easily be made to allow a user explore an environment and switch between lens types to see the differences. Try it before you buy it.
Thank You Dr Devgan. The side by side images helped me make my decision as to which lens to go for. God Bless You.
Very well explained and very informative as I will be undergoing my cataract surgery this week if my blood glucose level becomes normal.

A very informative video for us trying to decide the best IOL before operation. Thanks a LOT Doc 😊😊
Thank you sir I got the clarity of the knowledge of lens before my catract surgery
Thank you Dr Devgan for the explicit explanation. 🙏
Doctor thanks for the wonderful explanation. I am planning for cataract surgery. Can you please give your opinion on eyhance. They say it is a monofocal lens with enhanced inter meditate vision and hence give the same vision quality as normal monofocal lens and not an edof. Can I go for it.
Alcon acrysof vs Tecnis 1 piece monofocal? Alcon has glistening but Tecnis does not ? Correct?
Thx I also wasn't told of pro's and con's of each so I think the monofocal the best for me.
Thx for explaining it so throughly.
Thank you Dr. Devgan you are amazing.
Best explanation out there! I now feel comfortable with my lense choice on my upcoming cataract surgery.
What did you decide? I'm painstakingly trying to confirm a decision for myself as well. Are you happy with the outcome?
Not medical advice, but I did a quick search and YES, Cystoid Macular Edema (CME) MAY be associated with Femtosecond Laser surgery, according to a 2015 study titled "Cystoid macular edema after femtosecond laser-assisted versus phacoemulsification cataract surgery" found on PubMed conclusion: "Increased CME might be a subthreshold retinal injury safety signal after femtosecond laser pretreatment and warrants further study."
J Cataract Refract Surg. 2015 Nov;41(11):2373-8. Thanks for posting this, I am going to reconsider. Always been concerned about high energy lasers and the retina.

Excellent presentation. Does this video cover light adjustable lenses?
I had retinal detached since 3 years ago i must cataract now, is it safe?
Which lenses are fit for me multi/mono?
Do you have a video showing the toric lens for astigmatism?
Thank you so much for your helpful video. I was recently diagnosed with early stage glaucoma and am extremely near sighted with moderate astigmatism too. The final eye measurements are yet to be done. I may choose a toric lens to correct the astigmatism, but I don’t think the trifocal lens will be an option. However will mention it to my doctor as well as mono vision lenses. You didn’t mention mono vision lenses here so maybe 🤔 you do not care for the option. It’s all so much for the patient to absorb! Thanks again!
I wish you could recommend a good cataract eye surgeon in northern CA????
Thank you! So informative and detailed vs other sources I've found re: choosing a cataract surgery lens.
This was great! It really has helped me understand what I need/want to know. Thank you!
Great presentation👍 The best explanations I have found anywhere, Thank You.
Hi Dr
15 days ago I had Vivity implanted in both eyes. I see perfect cell phone, pc, tv, but the lights both during the day and worse at night cause halos and glare, I would like to know if that goes away with time or is it permanent?
Thanks for the presentation. Can you please comment on the lens Tecnis Eyhance IOL, thank you
Wish I would of had come accross this video prior to my fathers surgery performed here in Florida.

Wonderful and very informative presentation.
Thank you,
This is a wonderful video, but I wondered if you had a similar presentation for choosing a multifocal near vs intermediate vs distance.

So well explained I was told to get cataract surgery & was not comfortable now I understand. Too bad I am in Florida.
Dr. Devgan, I have watched this amazing video several times as I am planning to have cataract surgery at age 82. I am wondering how accurate the comparisons are between mono-focal and tri-focal implants. Are these exaggerations needed to show differences, or or these differences truly realistic, especially the night comparison?
God Bless You doctor for this and all other videos.
Awesome informative presentation!! Thank you!
Great video! Would it ever make sense to have the PanOptix in one eye and the Vivity in the other? Thanks!
great question to discuss between patient and ophthalmologist. it's a possibility for the right patient.
My question for anyone who has gone this route, do readers do as good a job for varying close distances (0-3 feet) as a near sighted person wearing progressive lenses would have?
this is an excellent resource for patients.
Excellent. Thank you. Why does my surgeon want an extra $1,750 per eye for astigmatism monofocal lens? Is it much harder to implant? Thanks.
Mind sharing your experience.....? :)
Hola Ana hablas Español?
Much much much appreciated
Great explanation
