OK, I asked around. There is indeed an easier to way to do this. From Nathan 'jesterKing' Letwory of Mcneel. "Around 12:30 instead of deleting the one face just use _InsertPoint. It works similarly to the Knife tool in Blender. Also, you could have just added full ring loops in the beginning instead of just the ranged bits for the fingers."
The thumb does not stick out to the side of the hand like that. Knowing 3D software is good but make sure you know your subject better. In this case anatomy is lacking.
Thank goodness that Subdivision is developing nicely in Rhinoceros so that users need not be stuck to rhino5 with Tsplines for this type of form.

Great, but it generates a lot of surfaces, I think T-splines generates less surfaces when converting to Nurbs
Unfortunately T-splines have been bought and incorporated into Autodesk products :(
It is not allowing me to choose one single part for extrude
You can use SHIFT + CONTROL to select the individual face. Or you can use the selection filter as well
