Thomas DeLauer interviewed Dr Tim Noakes about thre weeks ago. Both are very atletical while Mr. Noakes are a seasoned gentleman and experienced marathon runner who eventually got himself damaged by all that of fueling with quick carbs. Thomas DeLauer has also diabetes and sort of swayed between being very fit (he ran his first marathon at age 11 and in his early twenties he was severely obese). They both think that it's a great part of the mental expectation of that energy pack of carbs that functions as that "switch". Dr Noakes also states that it is to a great extent a placebo effect that is very powerful to the extent that the body actually may rise the glucose levels even if they don't actually consume the energytablet or drink. It's a very interesting interview for those who are concerned about low carb diets and endurance sports (or need arguments to those who for some reason persist to claim you need to eat a lot of carbs if doing that type of excercise), it's on Thomas DeLauer's YT channel , the episode is called "You won't eat carbs again".
A logical, sensible discussion of what insulin does in the body and if you want to remove insulin resistance, you don't eat a lot of carbs. The body is an efficient machine that can take care of itself if you don't add sugar to the mix.

Doctor Westman, My Blood Sugar level was between 8.00 and 9.0 for many years. After being on PHASE 1 of your diet for TWO WEEKS my blood sugar is between 5,`0 and 6.40 every day. I also had High Blood pressure and was on medication. After TWO WEEKS on PHASE 1 my Blood Pressure went too low. I have been OFF my medication for about 1 week, and my BP is 120/80.
I really appreciate your commentary in general and specifically about this topic. I saw my HbA1c also increase slightly while on a very low carb (under 20 grams total) carnivore type diet. I also watched my post-prandial blood glucose. Weird result that my blood glucose never reached the levels implied by the HbA1c. Think this lends support to Dr. Westman's conjecture that the life of the red blood cells has increased. Thank you for the video.
First Doc I've ever seen that understands the math/physics behind how an A1C measurement works well enough to use the idea of a derivative to explain it. Why doesn't anyone address that the only real way to get insulin down is to give it someplace to put the blood glucose. If the muscles are at capacity and the liver is at capacity then the glucose has no where to go except out in urine. In which case insulin will always be high. It doesn't matter if you are eating keto or not at that point.
I live in France, and was a type II diabetic. I've been on keto for 1 year, and my HbA1C is back down to 5.6, but my fasting glucose is stubbornly at 131 mg/dl. I asked my doctor for a fasting insulin test so that I can calculate (and start following) my HOMR-IR. He didn't have a clue what I was talking about, and refused to prescribe a fasting insulin, saying that fasting glucose is all that's needed ! When will mainstream doctors get up to speed with modern knowledge on reversing type II diabetes ?

I know that I am only one person but I found just the opposite - after 6 months on less than 10 grams of carbs/day (and my fasting insulin went from 35 to 8 and I got off of shooting insulin in the first 3 days) I ate some carbs (a hamburger bun) and my blood sugar didn’t go up near what it did six months earlier and it was back to normal in about 2-1/2 hours (which it never went back to normal, 82-84, before). This got me in trouble because I thought that I was healed and could eat some carbs again. But after just a few days of eating about 50-80 grams/day I quickly discovered that I was right back where I started & had to shoot insulin again. Now I’m fighting old battles all over again trying to get back into regular ketosis. I wish that I had never strayed. For me the big thing to remember is that carbs tempt me when I’m not even hungry, I just want a taste experience. Whereas I’ve never been tempted by a rib eye steak when I’m not hungry. I just need to retrain myself to only eat when I’m hungry.
Oh dear…I have been keto for 8 months..I have adhered I would say…fairly strictly. What do I do when I go to my doc and have my yearly blood work. I am Scared 😶. I live in a small town and it would be hard to find a keto friendly physician I think and I do like my doctor.
This was most helpful. Prior to switching to Carnivore last October, I had an A1C of 5.1( was dirty Keto then). Then after 5 months Ketovore/ Carnivore (and a higher carb weekend right before bloodwork)I had my A1C tested again and it shot up to 6.1! I’ve never been over 5.5 in the 6 years that I’ve been doing lower carbs. I’m going back to Ketovore/ Carnivore now and will test again in 3 months. Hopefully this was just a fluke 😮
There are suggestions that on carnivore / keto diets red blood cells love longer. This means they just happen to become more glucated over time, so the HbA1c levels are inflateď simply because of that fact as opposed to higher blood glucose levels. Worth looking into.
I've been on keto since my first appointment with you in September 2022, and I've lost 45 lbs. I was so excited yesterday to get my A1c test results of 5.4 (normal) after being diagnosed as diabetic (6.5) last March. My only issue is the cholesterol level went from 202 to 220. I would love to hear more about keeping cholesterol levels down on keto.
As long as your HDL is normal or above and your Triglycerides are normal or below you are good to go My LDL is 325 and after taking Dr Westmans cholesterol class I am not concerned
Your HDL to Triglyceride ratio is more important to pay attention to. If the ratio is below 2.0, the research shows there is very little chance you have small, dense LDL particles. These are the particles that cause arterial plaque. Large, fluffy LDL particles do not. When you go low carb, it is common for everything to improve in your blood work (HA1C, glucose, HDL, Triglycerides) but LDL will often go up. This makes sense because Keto involves increasing your healthy fat consumption. Despite all the improvements in metabolic health, the average MD will focus on the elevated LDL and in knee jerk fashion insist that someone benefitting from Keto/low carb-high fat take a statin. You need to do your own homework because the average physician is not remotely up to date on this stuff and is financially motivated to treat chronic conditions instead of cure them. Good luck!
My friend wants to go on a keto diet to get healthier but is already a diabetic. He is taking Metformin. How should he handle this to work on getting off those meds? I hope I’m making sense with my question. Thanks for all responses in advance.
Have your friend follow Dr Westmans advice even take a class. Watch out this is very important, if your friend does an actual 20 carbs or less diet they will need to monitor medication carefully with Drs over-site because they will might need to drop meds fairly fast. Watch as many Dr Westman videos as you can. Get informed and proceed carefully. It works!
I’m two years on keto. Lost over 80 pounds in the first seven months, but the last four months something happened and I’m still struggling to find out what it is. I gained 16 pounds, and my blood glucose went up by 20 mg/dl on average. Before that, my fasting morning glucose was in the lower 80s and my A1C was firmly at 4.7-4.6%. Plus, my fasting insulin was below 3, actually 6 months prior to the sudden change it stayed 1.6 with c-peptide of 0.73. All of a sudden, in just 5 days my fasting morning glucose incrementally went up by about 20 mg/dl, A1C followed subsequently to 5.1%, but fasting morning insulin and c-peptide stayed low (1.6 insulin and 0.73 c-peptide). It’s month five now and this drives me nuts. Can’t figure what’s going on. I begin to suspect I’ve become so much fat adapted because almost the entire fuel is fat, that I’ve become not insulin resistant but glucose resistant. My cells simply refuse to take the glucose in the blood. Unless I walk briefly - just 5 minutes walk and my blood glucose drops significantly - by about 20 to even 30 and sometimes 40 mg/dl. As soon as I climb stairs and my glucose is up again - from say 70 to 80, to about 100. And if I then sit or lay - it stays this elevated for hours (it barely changes during the night and sometimes it’s a bit higher in the morning). Postprandiial glucose used to almost never exceed 100 mg/dl (on average like once in a month) and typically stayed well below 100, and so was throughout the day and night, but now it exceeds 100 8 out of 10 days and quite often climbs to 110-120 for hours EVEN when not fed. Could be the strong coffee I love to drink - increasing cortisol levels. But this wasn’t the case prior to these past 4 months. I’m guessing my liver is pumping glucose because of such a low insulin while glucagon is supposed to be up. And when it pumps, because this is more or less the glucose I have when I’m eating only twice a day in 16 to 18 fast and 6 to 8 hour feeding window and I rarely eat more than 30 to 50 grams of non-starchy vegetables carbs max, I guess my cells are so well adjusted to run on fat and ketones that they simply refuse to take up that circulating glucose in the blood, and so it pools for longer. My reticulocytes stay low - 5-6 out of 15. Probably my red blood cells have lengthier lifespan? I also practice extended water fasts. I reduced this practice to less often because I suspect it decreases insulin even further and increases glucagon much higher. I also cut out any dairy for 2 months and observed slight improvement of the glucose, but then I did 5 days water fast and since then - 1 full month already my glucose went up again and in these last few days it went up even higher with seemingly no reason. Something is wrong I think, something bad is happening. Or, I may have glucose resistant instead of insulin resistant. Past 2 months my insulin and c-peptide slightly increased (from 1.6 to 2.3 insulin, and from 0.73 to 0.95 c-peptide) - I intentionally started to eat more non-starchy vegetables to try and increase the insulin a bit. This coincided with the slight improvement but last couple of days things got much worse. I’ll try to lower fats in my diet and slightly increase non-starchy vegetables and see if it helps. It is true that II upped dairy consumption a lot from about halve a year so far. And I mean a lot. One last thought if cells refuse to take the glucose - it follows that they don’t get damaged. No? The “sacrificial lamb” in that case as professor Bart Kay says - is the red blood cells - the A1c, and they renew much faster than the more complex and more precious cells in the rest of the body. Who knows?? I’m still scratching my head and trying out different things. Probably long term keto has this effect on some people. Who is to say if it’s safe or not? If higher blood glucose is NOT OK in general, why should that not be the case for someone like me just because I’m on keto and keto is considered safe? There are no research studies on this, everyone just makes educated guesses and gives his/her opinions. Either way, I want to get back to my low levels of blood glucose, just to be on the safer side of things. I’ve scouted the internet and I’ve not found satisfactory answer to this. It was much easier and straightforward when I started keto because it worked miracles by the weeks and months, but something changed very abruptly. Although my markers of inflammation are very low, as expected on keto, my blood sugar is elevated to pre-diabetes and more often lately in the diabetic range. This worries me much, it terrifies me. I was a diabetic type 2 and I had a heart attack 2 years ago. This lead me to research things and keto worked like magic, until something changed. What do you guys think?
I don't have a gallbladder, and I am 6 weeks into Keto. Thursday I was vomiting a meal I had 5 hours earlier with a lot of bile and extreme nausea. Is this caused by too much fat? My ketones were high on the test strip.
In my 3rd year keto I've reaped its health rewards including weight loss & insulin sensitivity with my fasting glucose levels in the 80s. After 2 years keto I decided to try carnivore for its simplicity in keeping carbs low. But after 3 months I found my morning fasting glucose levels would routinely stay above 100 & would take most of a day to get below 100. The level would never be high, even after eating, staying in the 140s then or lower, but would come down very slowing even after taking walks. When I was on keto, my HOMA-IR scores on my first 2 yearly physicals was 1, indicating insulin sensitivity. I've since gone back to keto & my morning glucose levels have improved but often are still just above 100. I already know I do have the dawn phenomenon, but it was far less pronounced before I tried carnivore which I also gave up because I found it to be too restrictive for me as well as being a boring and cheerless dietary lifestyle.
I have Dawn phenomenon, so have fasting numbers anywhere from 128-160..after two hours or less, numbers level off great. I’ve recently started taking a teaspoon of apple cider vinegar (with water) before bed..this has enabled my fasting number to drop 10-15 points. Went keto mid 2021…lost 33 pounds..but seemed to have hit a plateau now..but not giving up. Wonderful video..
Most likely you are in a glucose-sparing state. Your body knows you don’t need to leech onto so much glucose (organs/muscles) so it lets it float around in the blood for fast use needs.
Something I question about the A1C and Hemoglobin living longer.........it is my understanding that the average life of Hemoglobin is actually closer to 120 days...give or take. However the HbA1C tests the Hemoglobin at the 90 day mark. So since your Hemoglobin typically lives up to 120 days the HbA1C test does not reflect this longer lifespan. With that in mind I'm not sure how the HbA1C test can go higher just because your Hemoglobin is living longer with Keto. Perhaps someone here can answer that one.
Don't vegetables have a tiny bit of sugar that you wold still need insulin to process? The body itself can create glucose if necessary. Isn't insulin needed in that circumstance as well? It doesn't sound like the body completely stops producing insulin with a keto diet.
As Carnivore, my morning insulin is low, I am in ketosis, but often my Fasting Blood Sugar is high, like 110-114, instead of 85 or less. Although I may not be insulin resistant, is the excess blood sugar damaging my arteries, etc? Should I use a drug to lower the sugar?
The body normally produces glucose shortly before waking in the morning, commonly known as the dawn phenomenon or dawn effect. I usually test my blood glucose level shortly before my first meal of the day, which is around 11 am or noon. My glucose is elevated when I first wake up but then drops to the mid 80s a few hours later.
After 2 years lowcarb/keto, I have developed a similar issue (higher fasting BGLs). I will get another Hba1c test later this month. If it is below pre-diabetic level, I will not be concerned. I cannot imagine that the answer would ever be to add more carbs! I'd like to avoid drugs, and if the Hba1c is normal - no need. If not I'll have to consult an endocrinologist and see.
Doc, thanks for the video. One question regarding your comment "If the insulin is low, then you don't have insulin resistance". I thought insulin was produced by the body as a reaction to glucose. If you graph insulin, you will see a spike when someone eats carbs and protein and if you wait a few hours, the insulin levels will go down to zero. If you measure during the period of time after the spike when the insulin has gone down to zero, it would seem to me that you will have no idea if you have insulin resistance or not. I thought people with insulin resistance still had a graph where insulin goes down to zero if they wait long enough such as early morning before eating anything.
My pancreas is amazingly efficient. I aimed for that because of my mucosal melanoma that thrives with elevated glucose and insulin. Almost two years NED. My fasting insulin level is great and my A1C is fine. I get dawn effect glucose. My glucose Standard deviation is like 8. I’ll take that!
When I stopped keto my A1C increased a point from where I began. Before keto 5.6, during keto 5.4, after keto 5.7. Now it is 5.6 again. I did like keto because it reduced cravings but I was never able to completely get rid of them. I would love to do keto again, but I can't sustain it.
Good to explain to the public "Insulin Resistance" is a construct, not an actual directly measurable item. It is a theory we share and not empirical evidence. Many patients now ask for a insulin resistance test because they heard about it on UA-cam. They don't understand it is a series of tests where we infer their health status, not just one number. That would make a great video. Might also do a video on those athletes on a high carb diet but have a lower Hba1c than many on a true keto (75% fat) diet. Good stuff. The only way to get each person on the correct diet for the correct reasons will be to show why and where some do great on high carb or high protein but why they don't match the criteria for such a diet based on their specific circumstances/ situation (age, race, activity levels, sleep patterns, lifestyle, goals, past health history and present health status).
Sorry Dr. AJ, but it is not a construct or just a theory. You should be checking a fasting plasma glucose and a fasting serum insulin level on your patients, then calculate their HOMA IR score. This is a direct measurement of insulin resistance, plus your patients may be showing other physical signs of the condition. You should educate yourself first before saying things like this. Take better care of your patients. Before you ask, yes I too am a doctor.

It is not a problem or a negative comment, just the truth we cannot provide a patient with their insulin resistance reading. YES we have several tests, as I stated, that provide a big picture view of the situation. Please show me a hospital lab report the reads: insulin resistance value = xx. Doesn't exist. Not yet. Someday we hope.
How can i just eat like a human without all the craziness🤦🏾♀️ Ever since modern food technology ( 13:29 Big Food) comprised our food supply with all the ultra refined carbs, added sugar and industrialize seed oils, our metabolic health declined. Your liver may be dumping a bunch of stored stuff. People who know a whole lot more than me can explain it better but I bet you have fatty liver that's resolving.
judys • Take it slow, don't rush it. For some of us it takes years of doing strictly healthy keto to normalize our diabetes, colesterol, high blood pressure, glucose metabolism, fat liver, fat pancreas, thyroid, etc. Reducing your stress is very important. If you do high strength exercises, you should know that they increase your stress hormone cortisol too much, which increases blood glucose even if you eat keto. Any stress of any kind acting on your body ( physical stress, work stress, family stress, too much mental work, exercising too much, working too much, worrying too much, etc ) increases your blood glucose.
Have been Keto/LCHF for about 3 years, recently blood work showed my fasting insulin has gone from 6.8uIU/ml to my last test which was 19.1uIU/ml. This really surprised me as everything else is perfectly within range. I do however consume closer to 30% protein, 45%fat, and 25% carbs - should I change something, should I be worried.
I've been carb, sugar, starch free since Jan and only lost 2 pounds. I have hypothyroidism and I'm 60. Don't eat lunch anymore. I have eggs, butter, fresh spinach for breakfast, a steak or burger or chicken for dinner w broccolli with butter for dinner most days. I have coffee w cream in the mornings but it is decaf. I never sit I am always doing something, walking around somewhere or cleaning the house and walking up and down 2 flights of stairs in the house. I don't watch tv, I am not a sitting type. I can never seem to lose and keep weight off of me. HALP! PS my A1c went from 6 to 5.7 but my morning sugar is still over 100 so not sure what that is about either.
Those are doctors telling people to eat more carbs? The results of this approach is everywhere in the western and modern societies. People are metabolically sick. Obesity, type 2 diabetes, fatty liver disease, etc. What other results do you need that sugars are not good for us.
I've been doing low carb/carnivore, not keto for 3 months and have lost 25 lbs and yesterday I had labs and my A1C was 4.9 but for some reason my urine showed I had a level of 80 mg/dL for ketones. Is this bad? Why is it so high? What am I doing wrong?
I’m almost exactly the same. 3 months ago I was alarmed after seeing blood glucose of 190 after sport drink. Cut out carbs and went carnivore. Weight after 3 months: 185 to 163. BP lowered to 115/75 from 145/95. But urine sticks sometimes hit 80 mg/dl. Bothered me enough that I added 10-15 g carb a day, spread out. Now around 20-40 mg/dl. Little info on urine ketones that I could find.
Hi Dr Westman from Perth Western Australia, insulin down from 140 - 10 units lantus a day since May 2022 on Keto + 2 metformin a day but blood glucose a little high in a range of 6.0- 8.0 and hba1c in 7-8 range. Is it better to take 1 Forixga a day, or increase insulin to 12 units - which is worse insulin or the drug ?? - even hoping the forixga may , may enable me to lower insulin to 8 units, I could then maybe ? just use 1 unit of humalog in the morning to get levels in the 5 - 6.5 range ?? I am not seeing my keto Dr until mid April - thanks
Dr Cywes says that your pancreas will get lazy on keto or carnivore and if carbs are higher than notmal one time your body will exaggerate and spike bood sugar because the body isnt ised to that and wont have the ability to push p ut enough insulin to react. Has anyone else heard this from him?
Interesting my blood glucose has gone up since I switched from low carb say 80g to Carnivore 0 g from 80 to 95/100. Possible explanation is some people secret more glucagon( trial should measure that). I am only 3 weeks into Carnivore so will keep monitoring. Pre Carnivore insulin was 8 glucose 72 and C peptide 0,7. Will repeat 3 months into Carnivore. Feel great and losing wt
Ironically, I posted a similar comment concerning my experience trying carnivore before I read your comment. On carnivore, my fasting morning glucose levels would stay above 100 while on keto they were usually in the 80s. Also, on carnivore it would take most of a day of continued fasting for my glucose level to go below 100. I'm done with carnivore because I found it to be too restrictive a diet as well as being boring and cheerless.
Is it healthy to be in a ketogenic state all the time? Carnivore advocates say no. Bart Kay For example, says you should devour one giant meal a day to produce an insulin bike high enough to kick you out of ketosis without eating any carbohydrates. I can’t do that. I eat two meals a day 12 hours apart. I am afraid over time my muscles may become resistant to absorbing glucose since I’m only burning fat all the time. I recently started to consume, gummy bears (Just a handful) After my workouts to kind of remind my muscles to absorb some glucose from my bloodstream. I don’t know if this is safe to do. Trying to find a balance, Because I am convinced, I should not be eating very many carbohydrates at all now.

Fun fact, my husband was part of a G-force study and they injected him with a radioactive material that stuck to his hemoglobin so they could test how g-forces affected the blood. He knows exactly how long his red blood cells live. At least at that time in his life. (I didn't ask him how long his lasted and I do not remember the material they used)
I know you say "Just don't eat carbs." We eat carbs to feel normal again. Please do a Video on how addictive artificial sweeteners are. I did your program last January and used them. I am off artificial sweeteners for 3 weeks now and am still craving sugar. Sugar is worse then crack.
I’m the poster child for this carb sensitive issue. I’m 64 years old, still overweight, off of Type2 Diabetic Med, Glyburide for 15 months. Dr. was extremely upset about me losing 70 lbs. on a Keto diet. Started screaming at me. Begged me to get back on complex carbs. I argued back, 6 months later, 24 lb. gain though my blood sugar did stay in control. Back to Keto. I have your book, End your carb confusion and the page4 . My weight has become very stubborn to lose and my challenge is portion control. I think I’m as Casey Durango says, ‘Eating too much fuel’. How do I figure out portion sizes. I just know the limits on veg/salad and fat. I’m a retired ICU Nurse so of course I understand you but still this is frustrating. I’m 64 years old. January I can switch Drs. I read that guy's book. It caught my eye at the library. He's on the cover without a shirt. He's very into strength training and he's a neurosurgeon. I'm not saying it's impossible that he could be wrong about something based on this but just thought someone might be interested in some details. In other words he's not some fat chiropractor quack selling books or supplements.

Why can carbs lower fasting glucose? Comparing keto, 20 grams of carbs per day, to low carb, 100 grams of carbs per day, my fasting glucose was about 80 on 100 grams of carbs compared to about 100 on 20 grams of carbs. I have been on keto over 6 months, its not a adaptation phase.
Doesn’t the pancreas need to excrete insulin anyway with protein and fat? Not sure why the pancreas would become ‘lazy’ on keto? More efficient sounds more accurate. Better than deadly or overworked!

So Andrew Huberman states the same --that keto will diminish the body's ability to process carbs as efficiently as it once did. He didn't go into detail though. Some devotees like Peter Attia have gone back to eating a more traditional diet. Bikman eats what's on table w his family. It may be that we can break the machine with extremes as when trying to go back to eating normally after super low calorie diets. Though I have utmost respect for Dr. W, the history of medicine is shocking in its hubris, ignorance, & cruelty. Let's keep that in front. The human body is light years ahead of our current state of knowledge about it.

Thomas DeLauer interviewed Dr Tim Noakes about thre weeks ago. Both are very atletical while Mr. Noakes are a seasoned gentleman and experienced marathon runner who eventually got himself damaged by all that of fueling with quick carbs.
Thomas DeLauer has also diabetes and sort of swayed between being very fit (he ran his first marathon at age 11 and in his early twenties he was severely obese). They both think that it's a great part of the mental expectation of that energy pack of carbs that functions as that "switch". Dr Noakes also states that it is to a great extent a placebo effect that is very powerful to the extent that the body actually may rise the glucose levels even if they don't actually consume the energytablet or drink.
It's a very interesting interview for those who are concerned about low carb diets and endurance sports (or need arguments to those who for some reason persist to claim you need to eat a lot of carbs if doing that type of excercise), it's on Thomas DeLauer's YT channel , the episode is called "You won't eat carbs again".

Doctor Westman, My Blood Sugar level was between 8.00 and 9.0 for many years. After being on PHASE 1 of your diet for TWO WEEKS my blood sugar is between 5,`0 and 6.40 every day.
I also had High Blood pressure and was on medication. After TWO WEEKS on PHASE 1 my Blood Pressure went too low. I have been OFF my medication for about 1 week, and my BP is 120/80.

I really appreciate your commentary in general and specifically about this topic. I saw my HbA1c also increase slightly while on a very low carb (under 20 grams total) carnivore type diet. I also watched my post-prandial blood glucose. Weird result that my blood glucose never reached the levels implied by the HbA1c. Think this lends support to Dr. Westman's conjecture that the life of the red blood cells has increased. Thank you for the video.

Why doesn't anyone address that the only real way to get insulin down is to give it someplace to put the blood glucose. If the muscles are at capacity and the liver is at capacity then the glucose has no where to go except out in urine. In which case insulin will always be high. It doesn't matter if you are eating keto or not at that point.

I live in France, and was a type II diabetic. I've been on keto for 1 year, and my HbA1C is back down to 5.6, but my fasting glucose is stubbornly at 131 mg/dl. I asked my doctor for a fasting insulin test so that I can calculate (and start following) my HOMR-IR. He didn't have a clue what I was talking about, and refused to prescribe a fasting insulin, saying that fasting glucose is all that's needed ! When will mainstream doctors get up to speed with modern knowledge on reversing type II diabetes ?

I know that I am only one person but I found just the opposite - after 6 months on less than 10 grams of carbs/day (and my fasting insulin went from 35 to 8 and I got off of shooting insulin in the first 3 days) I ate some carbs (a hamburger bun) and my blood sugar didn’t go up near what it did six months earlier and it was back to normal in about 2-1/2 hours (which it never went back to normal, 82-84, before). This got me in trouble because I thought that I was healed and could eat some carbs again. But after just a few days of eating about 50-80 grams/day I quickly discovered that I was right back where I started & had to shoot insulin again. Now I’m fighting old battles all over again trying to get back into regular ketosis. I wish that I had never strayed. For me the big thing to remember is that carbs tempt me when I’m not even hungry, I just want a taste experience. Whereas I’ve never been tempted by a rib eye steak when I’m not hungry. I just need to retrain myself to only eat when I’m hungry.

Oh dear…I have been keto for 8 months..I have adhered I would say…fairly strictly. What do I do when I go to my doc and have my yearly blood work. I am Scared 😶. I live in a small town and it would be hard to find a keto friendly physician I think and I do like my doctor.
This was most helpful. Prior to switching to Carnivore last October, I had an A1C of 5.1( was dirty Keto then). Then after 5 months Ketovore/ Carnivore (and a higher carb weekend right before bloodwork)I had my A1C tested again and it shot up to 6.1! I’ve never been over 5.5 in the 6 years that I’ve been doing lower carbs.
I’m going back to Ketovore/ Carnivore now and will test again in 3 months. Hopefully this was just a fluke 😮
There are suggestions that on carnivore / keto diets red blood cells love longer. This means they just happen to become more glucated over time, so the HbA1c levels are inflateď simply because of that fact as opposed to higher blood glucose levels. Worth looking into.
Make sure you check it with different labs, you'd be amazed at the discrepancies. Also, check fasting insulin.
I've been on keto since my first appointment with you in September 2022, and I've lost 45 lbs. I was so excited yesterday to get my A1c test results of 5.4 (normal) after being diagnosed as diabetic (6.5) last March. My only issue is the cholesterol level went from 202 to 220. I would love to hear more about keeping cholesterol levels down on keto.
As long as your HDL is normal or above and your Triglycerides are normal or below you are good to go

Your HDL to Triglyceride ratio is more important to pay attention to. If the ratio is below 2.0, the research shows there is very little chance you have small, dense LDL particles. These are the particles that cause arterial plaque. Large, fluffy LDL particles do not. When you go low carb, it is common for everything to improve in your blood work (HA1C, glucose, HDL, Triglycerides) but LDL will often go up. This makes sense because Keto involves increasing your healthy fat consumption. Despite all the improvements in metabolic health, the average MD will focus on the elevated LDL and in knee jerk fashion insist that someone benefitting from Keto/low carb-high fat take a statin. You need to do your own homework because the average physician is not remotely up to date on this stuff and is financially motivated to treat chronic conditions instead of cure them. Good luck!
My friend wants to go on a keto diet to get healthier but is already a diabetic. He is taking Metformin. How should he handle this to work on getting off those meds? I hope I’m making sense with my question. Thanks for all responses in advance.
Have your friend follow Dr Westmans advice even take a class. Watch out this is very important, if your friend does an actual 20 carbs or less diet they will need to monitor medication carefully with Drs over-site because they will might need to drop meds fairly fast. Watch as many Dr Westman videos as you can. Get informed and proceed carefully. It works!

So if my alc is 6.1 and glucose is 128 should I ask for a blood glucose test and blood insulin test at the same time?
6.1 corresponds to an average glucose of 126. Your fasting insulin will likely be high with a HOMA-IR in the developing insulin resistance category.
I’m two years on keto. Lost over 80 pounds in the first seven months, but the last four months something happened and I’m still struggling to find out what it is.
I gained 16 pounds, and my blood glucose went up by 20 mg/dl on average. Before that, my fasting morning glucose was in the lower 80s and my A1C was firmly at 4.7-4.6%. Plus, my fasting insulin was below 3, actually 6 months prior to the sudden change it stayed 1.6 with c-peptide of 0.73.
All of a sudden, in just 5 days my fasting morning glucose incrementally went up by about 20 mg/dl, A1C followed subsequently to 5.1%, but fasting morning insulin and c-peptide stayed low (1.6 insulin and 0.73 c-peptide).
It’s month five now and this drives me nuts. Can’t figure what’s going on. I begin to suspect I’ve become so much fat adapted because almost the entire fuel is fat, that I’ve become not insulin resistant but glucose resistant.
My cells simply refuse to take the glucose in the blood. Unless I walk briefly - just 5 minutes walk and my blood glucose drops significantly - by about 20 to even 30 and sometimes 40 mg/dl. As soon as I climb stairs and my glucose is up again - from say 70 to 80, to about 100. And if I then sit or lay - it stays this elevated for hours (it barely changes during the night and sometimes it’s a bit higher in the morning).
Postprandiial glucose used to almost never exceed 100 mg/dl (on average like once in a month) and typically stayed well below 100, and so was throughout the day and night, but now it exceeds 100 8 out of 10 days and quite often climbs to 110-120 for hours EVEN when not fed.
Could be the strong coffee I love to drink - increasing cortisol levels. But this wasn’t the case prior to these past 4 months. I’m guessing my liver is pumping glucose because of such a low insulin while glucagon is supposed to be up. And when it pumps, because this is more or less the glucose I have when I’m eating only twice a day in 16 to 18 fast and 6 to 8 hour feeding window and I rarely eat more than 30 to 50 grams of non-starchy vegetables carbs max, I guess my cells are so well adjusted to run on fat and ketones that they simply refuse to take up that circulating glucose in the blood, and so it pools for longer.
My reticulocytes stay low - 5-6 out of 15. Probably my red blood cells have lengthier lifespan?
I also practice extended water fasts. I reduced this practice to less often because I suspect it decreases insulin even further and increases glucagon much higher.
I also cut out any dairy for 2 months and observed slight improvement of the glucose, but then I did 5 days water fast and since then - 1 full month already my glucose went up again and in these last few days it went up even higher with seemingly no reason. Something is wrong I think, something bad is happening. Or, I may have glucose resistant instead of insulin resistant.
Past 2 months my insulin and c-peptide slightly increased (from 1.6 to 2.3 insulin, and from 0.73 to 0.95 c-peptide) - I intentionally started to eat more non-starchy vegetables to try and increase the insulin a bit. This coincided with the slight improvement but last couple of days things got much worse.
I’ll try to lower fats in my diet and slightly increase non-starchy vegetables and see if it helps. It is true that II upped dairy consumption a lot from about halve a year so far. And I mean a lot.
One last thought if cells refuse to take the glucose - it follows that they don’t get damaged. No? The “sacrificial lamb” in that case as professor Bart Kay says - is the red blood cells - the A1c, and they renew much faster than the more complex and more precious cells in the rest of the body.
Who knows?? I’m still scratching my head and trying out different things. Probably long term keto has this effect on some people. Who is to say if it’s safe or not? If higher blood glucose is NOT OK in general, why should that not be the case for someone like me just because I’m on keto and keto is considered safe? There are no research studies on this, everyone just makes educated guesses and gives his/her opinions.
Either way, I want to get back to my low levels of blood glucose, just to be on the safer side of things.
I’ve scouted the internet and I’ve not found satisfactory answer to this. It was much easier and straightforward when I started keto because it worked miracles by the weeks and months, but something changed very abruptly. Although my markers of inflammation are very low, as expected on keto, my blood sugar is elevated to pre-diabetes and more often lately in the diabetic range. This worries me much, it terrifies me. I was a diabetic type 2 and I had a heart attack 2 years ago. This lead me to research things and keto worked like magic, until something changed.
What do you guys think?
To answer his question, Bart Kay says it takes 72 hours for your body to go back into ketosis once you kick yourself out eating carbohydrates.

I don't have a gallbladder, and I am 6 weeks into Keto. Thursday I was vomiting a meal I had 5 hours earlier with a lot of bile and extreme nausea. Is this caused by too much fat? My ketones were high on the test strip.
In my 3rd year keto I've reaped its health rewards including weight loss & insulin sensitivity with my fasting glucose levels in the 80s. After 2 years keto I decided to try carnivore for its simplicity in keeping carbs low. But after 3 months I found my morning fasting glucose levels would routinely stay above 100 & would take most of a day to get below 100. The level would never be high, even after eating, staying in the 140s then or lower, but would come down very slowing even after taking walks. When I was on keto, my HOMA-IR scores on my first 2 yearly physicals was 1, indicating insulin sensitivity.
I've since gone back to keto & my morning glucose levels have improved but often are still just above 100. I already know I do have the dawn phenomenon, but it was far less pronounced before I tried carnivore which I also gave up because I found it to be too restrictive for me as well as being a boring and cheerless dietary lifestyle.
I have Dawn phenomenon, so have fasting numbers anywhere from 128-160..after two hours or less, numbers level off great. I’ve recently started taking a teaspoon of apple cider vinegar (with water) before bed..this has enabled my fasting number to drop 10-15 points. Went keto mid 2021…lost 33 pounds..but seemed to have hit a plateau now..but not giving up. Wonderful video..
Most likely you are in a glucose-sparing state. Your body knows you don’t need to leech onto so much glucose (organs/muscles) so it lets it float around in the blood for fast use needs.
Something I question about the A1C and Hemoglobin living longer.........it is my understanding that the average life of Hemoglobin is actually closer to 120 days...give or take. However the HbA1C tests the Hemoglobin at the 90 day mark. So since your Hemoglobin typically lives up to 120 days the HbA1C test does not reflect this longer lifespan. With that in mind I'm not sure how the HbA1C test can go higher just because your Hemoglobin is living longer with Keto.
Perhaps someone here can answer that one.
If eating keto, and one would need to get emergency surgery (bypass surgery), would that cause starvation ketoacidosis in type 2 diabetics?
Don't vegetables have a tiny bit of sugar that you wold still need insulin to process? The body itself can create glucose if necessary. Isn't insulin needed in that circumstance as well? It doesn't sound like the body completely stops producing insulin with a keto diet.
As Carnivore, my morning insulin is low, I am in ketosis, but often my Fasting Blood Sugar is high, like 110-114, instead of 85 or less. Although I may not be insulin resistant, is the excess blood sugar damaging my arteries, etc? Should I use a drug to lower the sugar?
The body normally produces glucose shortly before waking in the morning, commonly known as the dawn phenomenon or dawn effect. I usually test my blood glucose level shortly before my first meal of the day, which is around 11 am or noon. My glucose is elevated when I first wake up but then drops to the mid 80s a few hours later.

After 2 years lowcarb/keto, I have developed a similar issue (higher fasting BGLs). I will get another Hba1c test later this month. If it is below pre-diabetic level, I will not be concerned. I cannot imagine that the answer would ever be to add more carbs! I'd like to avoid drugs, and if the Hba1c is normal - no need. If not I'll have to consult an endocrinologist and see.
Doc, thanks for the video. One question regarding your comment "If the insulin is low, then you don't have insulin resistance". I thought insulin was produced by the body as a reaction to glucose. If you graph insulin, you will see a spike when someone eats carbs and protein and if you wait a few hours, the insulin levels will go down to zero. If you measure during the period of time after the spike when the insulin has gone down to zero, it would seem to me that you will have no idea if you have insulin resistance or not. I thought people with insulin resistance still had a graph where insulin goes down to zero if they wait long enough such as early morning before eating anything.
My pancreas is amazingly efficient. I aimed for that because of my mucosal melanoma that thrives with elevated glucose and insulin. Almost two years NED. My fasting insulin level is great and my A1C is fine. I get dawn effect glucose. My glucose Standard deviation is like 8. I’ll take that!
When I stopped keto my A1C increased a point from where I began. Before keto 5.6, during keto 5.4, after keto 5.7. Now it is 5.6 again. I did like keto because it reduced cravings but I was never able to completely get rid of them. I would love to do keto again, but I can't sustain it.
Did you eat until you were full and get plenty of healthy saturated fat.

Good to explain to the public "Insulin Resistance" is a construct, not an actual directly measurable item. It is a theory we share and not empirical evidence. Many patients now ask for a insulin resistance test because they heard about it on UA-cam. They don't understand it is a series of tests where we infer their health status, not just one number. That would make a great video. Might also do a video on those athletes on a high carb diet but have a lower Hba1c than many on a true keto (75% fat) diet. Good stuff. The only way to get each person on the correct diet for the correct reasons will be to show why and where some do great on high carb or high protein but why they don't match the criteria for such a diet based on their specific circumstances/ situation (age, race, activity levels, sleep patterns, lifestyle, goals, past health history and present health status).
Sorry Dr. AJ, but it is not a construct or just a theory. You should be checking a fasting plasma glucose and a fasting serum insulin level on your patients, then calculate their HOMA IR score. This is a direct measurement of insulin resistance, plus your patients may be showing other physical signs of the condition. You should educate yourself first before saying things like this. Take better care of your patients. Before you ask, yes I too am a doctor.

YES we have several tests, as I stated, that provide a big picture view of the situation. Please show me a hospital lab report the reads: insulin resistance value = xx. Doesn't exist. Not yet. Someday we hope.

Check fasting insulin but also ketones. One cannot be storing glucose with insulin and producing ketones simultaneously. It’s one lane or the other.

For some of us it takes years of doing strictly healthy keto to normalize our diabetes, colesterol, high blood pressure, glucose metabolism, fat liver, fat pancreas, thyroid, etc.
Reducing your stress is very important. If you do high strength exercises, you should know that they increase your stress hormone cortisol too much, which increases blood glucose even if you eat keto.
Any stress of any kind acting on your body ( physical stress, work stress, family stress, too much mental work, exercising too much, working too much, worrying too much, etc ) increases your blood glucose.
Have been Keto/LCHF for about 3 years, recently blood work showed my fasting insulin has gone from 6.8uIU/ml to my last test which was 19.1uIU/ml. This really surprised me as everything else is perfectly within range. I do however consume closer to 30% protein, 45%fat, and 25% carbs - should I change something, should I be worried.

I've been carb, sugar, starch free since Jan and only lost 2 pounds. I have hypothyroidism and I'm 60. Don't eat lunch anymore. I have eggs, butter, fresh spinach for breakfast, a steak or burger or chicken for dinner w broccolli with butter for dinner most days. I have coffee w cream in the mornings but it is decaf. I never sit I am always doing something, walking around somewhere or cleaning the house and walking up and down 2 flights of stairs in the house. I don't watch tv, I am not a sitting type. I can never seem to lose and keep weight off of me. HALP! PS my A1c went from 6 to 5.7 but my morning sugar is still over 100 so not sure what that is about either.

Those are doctors telling people to eat more carbs? The results of this approach is everywhere in the western and modern societies. People are metabolically sick. Obesity, type 2 diabetes, fatty liver disease, etc. What other results do you need that sugars are not good for us.

I've been doing low carb/carnivore, not keto for 3 months and have lost 25 lbs and yesterday I had labs and my A1C was 4.9 but for some reason my urine showed I had a level of 80 mg/dL for ketones. Is this bad? Why is it so high? What am I doing wrong?
I’m almost exactly the same. 3 months ago I was alarmed after seeing blood glucose of 190 after sport drink. Cut out carbs and went carnivore. Weight after 3 months: 185 to 163. BP lowered to 115/75 from 145/95. But urine sticks sometimes hit 80 mg/dl. Bothered me enough that I added 10-15 g carb a day, spread out. Now around 20-40 mg/dl. Little info on urine ketones that I could find.
BTW, my A1C in July was 4.9 too.
Hi Dr Westman from Perth Western Australia, insulin down from 140 - 10 units lantus a day since May 2022 on Keto + 2 metformin a day but blood glucose a little high in a range of 6.0- 8.0 and hba1c in 7-8 range. Is it better to take 1 Forixga a day, or increase insulin to 12 units - which is worse insulin or the drug ?? - even hoping the forixga may , may enable me to lower insulin to 8 units, I could then maybe ? just use 1 unit of humalog in the morning to get levels in the 5 - 6.5 range ?? I am not seeing my keto Dr until mid April - thanks
Dr Cywes says that your pancreas will get lazy on keto or carnivore and if carbs are higher than notmal one time your body will exaggerate and spike bood sugar because the body isnt ised to that and wont have the ability to push p ut enough insulin to react. Has anyone else heard this from him?
Interesting my blood glucose has gone up since I switched from low carb say 80g to Carnivore 0 g from 80 to 95/100. Possible explanation is some people secret more glucagon( trial should measure that). I am only 3 weeks into Carnivore so will keep monitoring. Pre Carnivore insulin was 8 glucose 72 and C peptide 0,7. Will repeat 3 months into Carnivore. Feel great and losing wt

On carnivore, my fasting morning glucose levels would stay above 100 while on keto they were usually in the 80s. Also, on carnivore it would take most of a day of continued fasting for my glucose level to go below 100. I'm done with carnivore because I found it to be too restrictive a diet as well as being boring and cheerless.
Is it healthy to be in a ketogenic state all the time? Carnivore advocates say no. Bart Kay For example, says you should devour one giant meal a day to produce an insulin bike high enough to kick you out of ketosis without eating any carbohydrates. I can’t do that. I eat two meals a day 12 hours apart. I am afraid over time my muscles may become resistant to absorbing glucose since I’m only burning fat all the time. I recently started to consume, gummy bears (Just a handful) After my workouts to kind of remind my muscles to absorb some glucose from my bloodstream. I don’t know if this is safe to do. Trying to find a balance, Because I am convinced, I should not be eating very many carbohydrates at all now.

Fun fact, my husband was part of a G-force study and they injected him with a radioactive material that stuck to his hemoglobin so they could test how g-forces affected the blood. He knows exactly how long his red blood cells live. At least at that time in his life. (I didn't ask him how long his lasted and I do not remember the material they used)

I’m the poster child for this carb sensitive issue. I’m 64 years old, still overweight, off of Type2 Diabetic Med, Glyburide for 15 months. Dr. was extremely upset about me losing 70 lbs. on a Keto diet. Started screaming at me. Begged me to get back on complex carbs. I argued back, 6 months later, 24 lb. gain though my blood sugar did stay in control. Back to Keto. I have your book, End your carb confusion and the page4 . My weight has become very stubborn to lose and my challenge is portion control. I think I’m as Casey Durango says, ‘Eating too much fuel’. How do I figure out portion sizes. I just know the limits on veg/salad and fat. I’m a retired ICU Nurse so of course I understand you but still this is frustrating. I’m 64 years old. January I can switch Drs. I do have Dawn’s Phenomenon but at its worse, blood sugar is 130 and drops to the 100’s-110’s. Remember on no meds. The weight has to go. I’m 5’5” tall, 221 lbs. , was down to 198 lbs. 6 months ago.

I read that guy's book. It caught my eye at the library. He's on the cover without a shirt. He's very into strength training and he's a neurosurgeon. I'm not saying it's impossible that he could be wrong about something based on this but just thought someone might be interested in some details. In other words he's not some fat chiropractor quack selling books or supplements.
This is exactly what happened to me, or looks like it. The more carnivore I went, the higher my HbA1c! Totally baffled.

Why can carbs lower fasting glucose? Comparing keto, 20 grams of carbs per day, to low carb, 100 grams of carbs per day, my fasting glucose was about 80 on 100 grams of carbs compared to about 100 on 20 grams of carbs. I have been on keto over 6 months, its not a adaptation phase.
Doesn’t the pancreas need to excrete insulin anyway with protein and fat? Not sure why the pancreas would become ‘lazy’ on keto? More efficient sounds more accurate. Better than deadly or overworked!

So Andrew Huberman states the same --that keto will diminish the body's ability to process carbs as efficiently as it once did. He didn't go into detail though. Some devotees like Peter Attia have gone back to eating a more traditional diet. Bikman eats what's on table w his family. It may be that we can break the machine with extremes as when trying to go back to eating normally after super low calorie diets. Though I have utmost respect for Dr. W, the history of medicine is shocking in its hubris, ignorance, & cruelty. Let's keep that in front. The human body is light years ahead of our current state of knowledge about it.
