We've been practicing this when we go out the back door to take my puppy out, she hasn't quite figured out that this applies to all doors, but we're trying to be as consistent as we can. We also make her sit to put the leash on and take it off.

I use one. But have a big strong breed. Great Dane. I'd rather pinch skin and let them know whether they want that to happen or not. And it really helps with control.
No choking.
