Actually, NET/JRF is not compulsory for Ph.D application. If you have NET/JRF, that will help in your Research during Ph.D by financing you a stipend/month from the Fellowship offered bu the University. If not, however you can apply for Scholarship if you do not have a NET/JRF. Edit: Some University also offer Non-NET fellowship and the stipend depend on the University.

Yes it has changed since Dec 2017. I think the benefit of ls will be given if you opt for 1st, even if you don't cross jrf cutoff. I don't know if in 2nd and 3rd options, the benefit for other stream will be given or not. Like in option 2, jrf, they may not consider for ls. Either way, choosing the first option is safest

No matter what, you still need to finish your Master course and as per UGC rules, its mandatory to have Ph.D for lecturship from 2021.
