I think she was performing a lot and when you perform and overuse your voice a lot it can cause stress on your vocals especially when you are constantly singing. No doubt smoking yes but also performing over and over and over again. There are other singers where their voices have lowered over the years because they overused it and performed and most likely sung in a key that was too high for their norm and it did damage to their vocals. I believe you can sing high but if you strain alot it can cause damage. MJ sung high as a child and as the years went by his voice lowered.
