hi ms gail kasi yung form 3 and form 1 dpat same sila ng last name. hindi kase updated yung last name ko sa prc maiden name padin siya.
yung sa form 2 ko married name na.
hindi kaya

Hay. Sana umubra yung electronic signature ko sa form 2f kasi sabi ng school ko isend lang thru email yung form dahil ever since pandemic walang physical contact sa office nila.

@@jane4300 Hi Ms. Jane, nakapagprocess na ako ng NCLEX ko after I renewed my PRC lic with my husband's last name na. Last Feb, nag apply ako change status sa PRC it took them a month to change my last name tapos nung na approved nung March, nag set agad ako appointment para mag renew ng ID na apilyedo na ni hubby there after nagsubmit na ako requirements for NCLEX. Mas mabuti po na parepareho ang apilyedo ng PRC ID at Passport nyo

Hi po Ms. Gail, thank u for this video. DIY here.. Can i ask, i passed the Nurse 3 form instead of Nurse 3F for the Verification of License, and the PRC already mailed it to NY last wk.. Would that be okay? I was able to figure it out after watching this video since, i followed another video on my first attempt of watching DIY video..

Question po, okay lang ba na kahit ung name ko sa #4 is married name but ung naka attached na sealed TOR & DIPLOMA ko is maiden name? Need ko po ba mag attached nang marriage certificate sa 2F ko? hello po pag ipapadala po yung form 2? ano po mga attachement ksma? diploma po ba and tor? yung sa form 3 naman po ano din po attachment ksma?
hi po regarding sa form 2F yung sa date of attendance wala po nilagay si school registrar ng date only (06 mos/07 yr) wala pong petsa yung start date. pero yung sa end date kumpleto naman po (04/12/21) .. sabi ni registrar okay lang daw po yun. .sa tingin nyo po ba okay lang yun?

Hello, Gail. Great content! Quick question though. My sister is filling out the first name section but it's only limited to 15 characters. My sister's name consists of three names and longer than 15 characters so it's a bit of a trouble. Do you have any idea on how do we go on with it? Do you put the third name on the middle name section? Thanks!

hello po, when i checked the nysed for the forms, its different from the one you used. ATM of checking its Nurse Form 2 Certification of Professional Education. i hope im still on the right link on searching for the forms.

Hi Ms Gail, Ask ko lang po paano po kaya mag-fillout ng form 1, form2F and 3F with my situation. Una po kong nag college sa FEU June 2007, tapos 2nd see of 2nd year lumipat ako Arellano University due to financial problems. Tapos after 1 see lang napilitan na kong mag-quit at maghanap ng work kasi kailangan magpachemo yung mom ko at mag-college natin ang kapatid ko the following year. Nakabalik lang ako sa school noon 2018 tapos nasa-graduate sa awa ng Diyos noon May 2021. Lahat po ba lalagay ko sa nursing schools, feu, au tapos au ulit. thank you.

Fill out the 3f form online before downloading with your changes - that's what I did para mgkasya. :)
