  • It is common for adults in their 50s and 60s to have degenerative meniscus tears of the knee along with arthritis changes in that knee. In this Ask Dr. Geier video, I explain why orthopaedic surgeons often do not proceed with surgical treatment for the meniscus tear like we do with younger patients.

    Manny in California asks:
I am a 77 year old male with dancing as my main sport plus some activity in the gym. Almost three months ago some small pains developed in the right knee mostly on the lateral and rear, mostly with flexion. Extending the knee usually causes a click. An MRI showed a horizontal cleavage tear with 10-13 mm cyst and grade 2-3 patellar chondrosis. I've seen two orthopedic surgeons who have different views on whether I should have surgery now or wait. I can't decide. Almost minimal symptoms. They say it’s nonrepairable. No surgery could cause tear to worsen though. What are your views?
In the past, orthopaedic surgeons often performed arthroscopic debridements to clean up degenerative meniscus tears and arthritis of the knee. The long-term results of those surgeries have not proven as successful as we might have hoped. There are patients for whom surgery to trim out a meniscus tear can be helpful, though. In this video, I discuss the pros and cons of the surgery and offer some points to consider.

Adding to my previous post, I had a tear repaired 34 years prior on the same knee. Never had a bit of limitation. In fact, I had forgotten which knee had the surgery. After 34 years, and after the surgery, my physician stated I had very little arthritis and my knee appeared healthy.

I developed a meniscus tear left knee lateral. I'm 57 worked out since 12yrs old. It happen 5 months ago from Deadlifts and Squats. Did no steroids ever. I did air squats and very very light squats no deadlifts at all. and now after 5 months much improvement. I'm back up into the 200's on my squats and getting stronger little by little. But no deadlifts. And a lot of bike riding.

    Great video Dr Geier!
I have had a meniscus tear for over 10 years and when it was first diagnosed I was told to have surgery but I did not like the idea of having cartilage cut away and turned the opportunity down. I must admit there has been times when I get frustrated when It pains if I do certain movements but in general I can manage the pain and when i want to go hiking i wear a brace which is fantastic as it keeps everything in place. Normal walking no Pain but squatting can cause some discomfort along with certain stretches. I would only have surgery if I felt I could not do my normal daily activities and after watching your video you have confirmed I done the right thing not to have the surgery.

Hi, I have a chronic displaced detached bucket handle lateral meniscus
tear which I've had I suspect for about 18 years. It used to lock at
around 20-30 degrees but because I didn't know any better I used to sit
on the ground and straighten my leg, there'd be a loud pop and something
would go back into place! Then since I reinjured it a couple of years
ago (and did the hamstring of the same leg at the same time) it doesn't
lock anymore - it will hurt briefly but then swell for 3 days or so
during which time I'll be limping around. The MRI report from shortly
after I reinjured it a couple of years back said that most of the
posterior horn is torn off and sitting over the anterior horn, a
surgeon's told me that this would in theory stop me straightening my leg
fully which I can kind of see (a few degrees maybe). If I have just this
bit that's sitting in the wrong place removed, what's the chance of
making my knee worse? I'm pretty functional now, but it seems to be
happening increasingly easily (I managed to do it once a few days ago
just doing a simple straight kick in the air - no twisting at all and no
weight on the knee!). Sorry for the long question, great video!

Hello Dr. Geier, I suffered a small to medium meniscus tear 21/2 years ago. My initial Orthopedic initially did not recommend surgery. I recently have experienced a pretty dramatic increase in my pain level up to 6-8/10. In fact, I have great difficulty even walking, let alone pursuing normal activities like hiking, golf, etc. I went in for another MRI and the report stated the same, small/medium tear accompanied this time with arthritis and a cyst. Hi,
I had an injury about 10 years ago when I was 23 yrs old and was diagnosed as a second grade meniscus tear, my knee used to lock many times for a long time. though I lost about 15 kilos of my weight then symptoms have almost gone.. it no longer locks but sometimes if I did a wrong move or had an excessive effort it feels like there is something went out of its place and it hurts me and I cannot step on it. this symptom rarely happens but it does happen some times.
the latest MRI report indicates: Grade 1 sprain of ACL and grade 3 tear of posterior horn of medial meniscus with minimal intra articular synovial effusion and focal bone marrow contusion.
would physiotherapy do any improvement? or should I have surgery

I did trekking mountain for 15 kms ,every other sports but intense sports regularly like badminton was causing little swelling in my knee. Now today I am 34 yrs ago ,it was ok till today and now a,week back i feel some cold sensation in my knee like some floating thing feeling but no pain or swelling . I can walk ,half squat but full squat is paining..
Everyone is saying surgery not good in old people what about cases like us.. ours was a acute tear now almost 17 yrs past that, causing minor strain only . I dont have any mechanical symptoms now like locking or catching that much, range of motion also fine.. but every now and then a minor strain comes in that knee. Should we do surgery to avoid arthritis or should we keep it like as it is . Will that be a risk and eventually lead us to arthritis or total knee replacement..i am a little purplexed

Dr. Geier, I have what I suspect is a meniscus tear in my right knee. It's been 7 weeks since I've twisted my knee while skiing. I'm 49 and always been active (hockey, skiing, jogging and training). I've had an MRI and I will see the doctor in a few days for a definitive diagnostic. The symptoms that seem to point to a meniscus tear are the following : after several weeks of physio and strengthening, I did make some progress, but my knee flexibility has now reached a maximum threshold and it just locks and can't flex any further. I can't bend it to its normal angle. I don't feel pain, but I limp and it's starting to have an impact on my left hip. I hope to have surgery (if it's needed) so I can ski next season !

Hello, I was diagnosed with osteoarthritis when I went in to see the doctor. I had explained that this pain started worsen like the beginning of the year and gradually started have problems like I would feel locking in my knee where I would move and I’d feel a very painful feeling. So that’s when the doctor had me do X-ray. He got the results from the X-ray and told me that I have degenerative wear and tear forming osteoarthritis cause of the inflammation in my knee. My primary doctor thought it wasn’t osteoarthritis and wanted me to see a orthopedic to see what was going on. Went in to see the orthopedic, took X-ray and same thing the first doctor saw, they also explained that it was so narrowing that it was almost bone to bone. They gave me 3 option and 1: excercise, 2: injunction 3: surgery. At the time I worked as a Budweiser merchandiser walking, being on my feet, bending was part of the job. I was almost taking time off cause I was having so much pain. I would take ibuprofen through out the day but the pain would come back and worsen through the night. I don’t work anymore cause my calling off was effecting my work performance. So what would be best for me in my case? No surgery or surgery?

Pain started 6 weeks ago while using the elliptical at the gym and got worse while swimming on 4th of July. Could hardly walk and pain was awful. Visited an orthopedic surgeon who did an MRI. He recommended surgery to trim the torn section. Says it is a small tear. Got a 2nd opinion from a sports doctor who uses non-surgical methods and he recommended cortisone and PT. Have a surgery date for Aug 25 but am uncertain whether to proceed or not. I want to get back in shape and play tennis again some day. Any thoughts on going straight to surgery or doing injection and PT first? Thanks.Phillip Miller

First of all I am 41 yrs. old and I have dislocated my patella 4 times and chipped the patella. I was lucky I hadn't torn my ACL but the MRI showed I had uneven cartilage and torn cartilage and particles floating in my knee... I recently had surgery on my knee. Dr told me after surgery, that he had removed a chunk about the size of a nickel and also stated that it was possible that the chunk was the source of all my pain. Do you think I should have had surgery? My pain just kept getting worse over the past 3 months. What happened was that I squatted down and my left knee felt like something was going to rip in half. The skin on that side went completely numb.

I did have surgery on my left lateral meniscus. I believe it has re-torn and this time my head is in a million places 24/7 whether to push through with it or get surgery.
      Hope you are well.

hello! thank you for the infos you give here! i am a 53 year old woman, in good shape and active and my problem is i woke up one morning with a slight pain and swollen knee. As the swelling was still there after 5 weeks i decided to consult with a doctor who told me i need a arthroscopy. I had a MRI which says i have a slight tear horizontal on the medial meniscus. The rest is all good. i am not in pain but i still have a little bit of swelling and it doesn't allow me to do all the movements i want to do such as easy pose (in yoga=sitting crossed legged) or nor can i squat...which bothers me. Do you think i can avoid surgery and get better (get ride of the swelling) just by doing physical therapy? THANK YOU for your help! Josiane :)

Watch your video and I have the latter part of the symptoms. Started feel a lot pain. Loud pops from my knee. Had MRI done. It was a tear in the meniscus on the inside of right knee. However, that was 3 1\2 years ago and now the pain has greatly increased. Go See a specialist on Thursday. I just been putting this off for this long period of time.
What are your thoughts on this?

I've had the same pain went from almost two months knee pain went to the ER x-rays show no broken bones and referred to an Orthopedic doctor he said it was dislocated patella no surgery needed. Been in a brace for a month with worse pain now. Tried PT now I can't walk. I think getting an MRI and knowing what's going on inside the knee can help with knowing if you need surgery. If it's really bad cartilage damage. I think surgery could repair your issue but arthritis can become your enemy.

Hello, to begin with, I am 23 years old. I injured my right knee February, 2016. I was getting up from a deep squat. Instantly, I was not able to walk. I literally had to crawl. My knee was locked, it took me a few days to straighten my leg out. One month after, I got a MRI and my knee looked normal. However, I had very bad pain, I was not able to hyperextend my leg and my knee kept locking. Since then, I have had chronic knee pain. When I workout, my knee is more prone to locking. Till this day, my knee is very stiff and I still cannot hyperextend my leg. I am very limited in motions. Therefore, I decided to get another MRI. I just received my results yesterday and the doctor told me I have a large meniscus tear. I am getting referred to an orthopedic surgeon. Before seeing the doctor, I would like to gather as much information as I can.
Any advice you can give me?
    Thank you,

Hi Dr. Geier. I am a 29 year old male, very active. I had an MRI scan done after a sports injury and it revealed a torn meniscus in my right knee. I have not undergone surgery for it as yet as I feel the pain has subsided and does not interfere with my day to day activities. I wanted to find out from you if I could return to playing sports with this meniscus tear if it’s not really giving me much pain. Your help on the matter would be much appreciated. Thank you

    Hey Dr. Geier,
I'm 19 years old and have been dancing for most of my life. Over a year ago during my senior year in high school, my knee just began to give me pain in dance class, so I had to sit out of dance for a few months. As I took a break from dance, there was a little pain from daily tasks, but it was manageable for the most part, although I could not squat. Months later I began to dance again and my knee hurt terribly. I received an MRI and the doctor said that there was a tear in my lateral meniscus and that I should get the arthroscopic surgery. However, we cannot afford it. I just want to dance again with no pain. Do you have any insight on the alternative treatments? I've heard about injections and physical therapy.
    Thanks, Summer

Injection Will reduce your pain. If you want to continue your dance activities you should go for the surgery. In India especially in Kerala it is less expensive and high quality.

51 years young, was extremely active, now I only walk...Rt Knee Impression: Small Vertical flap tears of the medial meniscus at posterior horn, no chondrosis but but high grade localized chondrosis within the posterior lateral tibia. Both knees feel week and hurt. 30 years of 20 plus miles a week running and mens soccer on Sundays. I feel like if I get this surgey, I will still have issues and be in the same place. My tears seems to slowly be getting better with my new activity level. Thinking no to the surgery to clean it up. What do you think?

At 17 I had a medial meniscus tear and a loose fragment locked my knee. It's the only time I've ever lost consciousness from pain. Arthroscopic surgery was required, and for the majority of 25 years after I referred to it as my 'good' knee. However, the past 3 years I've been dealing with constant swelling & moderate pain, and its gradually lost range of motion to the point of being unable to bend much beyond 90°. I wondered if arthritis was the culprit, but after an osteopath visit, xrays and MRI, its another medial tear. I'm getting it scoped again in 6 weeks.

Wow! I am sorry to hear that. Did get any arthritis after all this time? I have a medial meniscus tear in one knee and another meniscus tear in the other knee with a lose fragment. Even though a Dr told me to do surgery, I’m reluctant not knowing exactly what to do. Even the one that has only the meniscus he told me to do surgery. I am in pan but not really sure yet what to do. I hope your surgery and recovery goes well. God bless! 🙏🏼😊

@@TheJessica44023 Its developed mild arthritis over the course of 30 years. Nothing to the extent that I've lost any sleep or ever felt like it was necessary to take ibuprofen, aspirin, etc to manage. Overall I've had considerably less discomfort with it than the limitations and pain levels before the procedures. But the vast majority of the time it feels no different (and sometimes slighly better than) the knee that's never had any problems. The 2nd scope went well. I only needed crutches for 3 days and was off work for a week. My first day back at work a couple of people looked a little bewildered and asked if I had my knee surgery. Since I was able to walk at my normal pace by then, they thought I might have canceled or rescheduled it. I hope you get some relief from yours, whichever route you choose.

I am scheduled for surgery in a week but still am unsure. I am 50 year old fairly active person with minimal grade 0-1 level arthritis. I have had episodic pain and swelling gradually that was gradually getting worse. I had PT and seems to quelled the pain and returned full function. MRI report said horizontal tears in both medial and lateral with a horizontal flap tear and bakers cyst. My Ortho said the decision is up to me and didnt say one way or the other. My PT said it seems counter intuitive because my knew feels great but most likely the pain will return again with the swelling, pain and loss of function. There is some popping and instability depending on the motion, load, and position.

+Gerald Braxton I think it really depends on the cause of your pain. If it is the meniscus tear causing a patient's knee pain, arthroscopic surgery can often help. If not, then other treatments like physical therapy, injections and more might be useful.

    Dear Dr. Geier,
I have a tear in my medial meniscus posterior horn. The first doctor I have been to said I should have an operation. And they will cut out 1/3 of the meniscus. Then I will be very comfortable.
And the second doctor said, If I were you I would not have a surgery. Looking at the physical examination and the symptoms I did not need a surgery. I may have a little discomfort but in the long time my knee will become older then me.
I am a 40 year old active man. I do workout almost everyday, fitness, crossfit, bicycle, basketball etc.
I have heard that if I do not have some part of the meniscus (considering the workout I am doing everyday) then the bones will rub each other and it will cause calcification and rheumatism in time.
I am confused, should I have surgery or not?
Or without surgery should I ease the workout. (not jumping too much, Not doing heavy squats etc.)
    Any suggestions,
    Thank you in advance,
    Kind Regards.

Great video! I like the insight and clarity in your description and explanations! I have a medial meniscal tear on the right side. I'm going through 12 weeks of PT to hopefully conservatively heal the tear. I'm a runner, and do not want to lose any part of my meniscus via surgery if at all possible, however, I cannot currently run because it is very painful. I'm hoping that PT can heal the tear.. I'm 6 weeks in and it's still too painful to run, but it does feel better than it did. My question is, if I do end up going the surgical route (if PT fails to resolve it), is it typical to expect to return to running? I'm a 35 year old male, in otherwise great health.

Good info, I started to look upon meniscus tear, I had my ACL broken and meniscus lesion grade 3 after MRI, did a surgery to do my ACL but the surgeon said that he couldn’t do nothing for my meniscus. Still feel pain from my meniscus and gets swollen when I run only. I’m thinking to look in for a surgery on meniscus, is it a good idea ?

Thank you so much for your informative video. By default Have not had arthroscopy surgery yet. My HMO paperwork and communication between my primary referral to the specialist has taken almost 2 weeks. My MRI showed a torn meniscus with osteoarthritis.I am so glad that I came across your video so I am not stressing as much. When the pain was intense I was prescribed 800mg ibuprofen which resulted in high BUN and Creatinine and contrast MRI could not be done.
Over time and not putting weight on my right leg the pain is a tad better.

Good evening Dr.Geier, I have a meniscus tear in both knees , they hurt when I walk a lot, or I'm standing for long periods, if i squad my knee swells up. But it's a pain I can deal with , I been told told that I need surgery but I seriously don't want to. Is there something I can do beside surgery? Should I lose weight? Work out my legs? Idk. Thank u for all your help! ( removing a piece of my meniscus knowing it wouldn't heal again scares me) I need to make a choice as soon as possible . My pain is not that bad yet.. should I delay it? Avoid it? Or go for it? I'm a construction worker, how long will I be off ? I don't want to make any more damage after surgery... please help me. Thank u

I have a lateral meniscus tear from about nine months ago. I jumped from about 10-15 feet and didn't anticipate my landing well. The first few days it hurt a lot, and knee movement was limited, and though I'm able to walk and sit fine, I haven't ran much since the incident. As someone who used to regularly run 3-4 miles about five days a week, I don't know if I can get back to running with this tear. The few times I've tried it's hurt a bit, and just doesn't feel as natural as it used to. (I'm scared I'll make it worse). That being said, I can still cycle and do other less weight bearing cardio activities with relative ease, including squats and most knee bending movements, as long as I don't twist it.
Has anyone been able to get back to running with a lateral meniscus tear or has surgery always been the best option? I'm hesitant to have surgery because the pain is overall minimal, and I can still do most of my daily activities with ease, but would love to get back to running. I would just hate to have surgery and the pain be worse than prior.

    • @willwill3755

If you attempt to run again I would recommend a very foamed running sole. I run in the Hoka Bondi 7, and it is very padded. I gave up running for a while about 8 years ago; however once I found the Bondi in 2015 , and after loosing 10 pounds I can run pain free again. I haven't been diagnosed with a meniscus tear, but I might have a minor one. Plus you know, I have known several runners who never get long term injuries because if they feel an odd pain in their knees, or ankles, tendons, feet etc., they force themselves to NOT run until the weird onset of pain goes away. Better to lose 5 days of running than forcing it getting injured and being out for 3 months.

Age 41. I also stupidly fall from three meters 5 months ago. Have horizontal tear on lateral meniscus in half of body and posterior horn which comunicates with upper side of this horn. Manipulation with knee by surgeon without symptoms. But MRI showed. Can Run 8km almost without symptoms and fully go by bike. If there would Chance to repair(not menisectomy) but stitch it. I would do this.

Is it possible to diagnose a meniscus tear without an MRI? I had an ultrasound done and my family doctor says there is no meniscus tear. But I experience almost every symptom of a meniscus tear. Felt about 5-7 pops in my knee when it was first injured and if I even lightly jog for more than 6-10 seconds my knee locks up or gives out. And if I run I land flat on my butt cause my knee gives out completely. Yet my doctor brushes it off and sends me for some physio. I'm 21 years old and no longer have the ability to jog or run which I used to enjoy. In need of some advice. :(

Dr Geier I had a meniscectomy in 2001 right knee lateral meniscus. 2 weeks later i tried wrestling on it. I fell back (knee to heel) and was in severe pain. It never healed properly (no pt either) and 5 months later had to have it again.
Last one was in 2002 so its been 15 years. I still wrestle, jump rope and do yoga almost daily. Sometimes if I am in a squatting motion and move forward it clicks or snaps. There is some slight bone abnormality and its a little puffy. my question is is this typical after 15 years and could my knee now be osteoarthritic? im 31 and plan to contine being extremely active.

I am 54 and a disabled vet . I did not hurt my knee in the accident in the NAVY as it was back in the 80s I was cutting a small tree about 4 inches around. It knocked me back and pushed my knees back . I had a MRI done and it showed 1 complete tear and the other knee 70% tear. I tried cortisone shots in both knees and 3 weeks rehab . Neither worked . I am 5'4 and am thin at 125 lbs. I also have diabetes and went under the knife 3 weeks ago. It was my right knee complete tear and the doc shaved it to get it away from the nerve . It still hurts pretty bad I am wondering should I not lift very much as when I do lift over 10 pounds it really makes it hurt. Any Ideas ?

I had a tibia plateau break. It has been 8 months since surgery for it. I just had a MRI and it showed to meniscus tears and some jaggedness to the meniscus. The surgeon wants to go in with a scope and file down the meniscus.
I have been watching your videos and find them very helpful, I would like your opinion please, is it necessary to do that? Will it cause more problems later to the meniscus. Please, I really need your opinion. Thank you!!!

I am going back to the way I was eating, I feel that it has helped with the slow progress of the arthritis and of course exercise for the leg.
