This Toy is located inside the City of Threads dungeon and can be bought from Fliq'ri. The problem is Fliq'ri is surrounded by a guard that will teleport you back to the entrance if you get close to it.

How To Buy The Toy Without Getting TP Out!
If you have friends, que up for the dungeon together and each take turns taking the frontal while each of you run to the vendor and purchase it.
Another way, if you are doing this solo, is que up for it in follower dungeon mode and move one of your NPC's into the circle. Once inside the circle, attack the Pale Priest. He will put the frontal on your tank and you can run in and purchase it.

Mage - Blink to the Vendor
Demon Hunter - Vengance To the Vendor
Lock - Demonic Gateway
Hunter - Using your Pet to take the frontal
