What you did wrong with the wither was that you spawned him in a plains biome. Withers can't spawn if there are blocks under the top part of the T shape, and there was grass under the wither. It has to do with how the wither spawn is coded. The bottom layer has to be: Air, Soul Sand, Air. You had: Grass, Soul Sand, Air.
