I think the reason he didn't use Gear 4th beside the plot was because he was already badly wounded, and using Gear 4th might have a fatal effect on his body because the pressure would be too much. Also look at how destroyed Dressrosa was when King Kong Gun punched Doffy into the ground, so it might more or less affect the Fishman Island. 3rd, he needed time to prepare for King Kong Gun, and Noah might've reached the island by that time. Elephant Gatling both destroying and slowing Noah down.
