For those wanting to know what happened: mechanic forgot to put the screws onto the bottom right ice boot. This caused it to fling off and them to loose lift causing a stall.
Quite wrong The maintenance team had a whole night to swap our both ice boots. The inspector and the maintenance leader decided not to do the usual paperwork because it would take too long. The inspector helped the crew unscrew. He unscrewed the left top and didnt tell the next team. Then while descending the plane, they pick up the highest top speed that day and the left ice booth came off, which caused the plane to drop because it was unstable. Which caused the whole tail to come off.
Second I didn't see them pull back on the throttles, not that it would have helped all that much in this case, I immediately thought the plane would tear apart and the pilots would blackout from the G force

If you notice the plane's livery in the wreckage, it was painted in a gold and red paint at the time of the accident instead of the modern Continental/United livery shown in the animation. Either way, the animation is still spot-on.

The actual NTSB report for this accident was much more detailed then this lousy Smithsonian channel representation. the fact is maintenance crews didn't finish installing the screws on the leading edge Of one side of the horizontal stabilizer when they were replacing the DE icing boots it was human error they didn't finish the procedure properly that's why they had a problem nothing came loose in the flight.

The maintenance engineers can never have a bad day that inevitably leads to carelessness and it becomes a really bad day for many others. This is so tragic!!!
The cause was a mechanic who removed 40 screws from the left elevator . When the leading edge of the elevator broke off , the plane plummeted towards the ground as it was an uncontrollable situation. The first officers high speed dive created enough gravitational forces that ripped the leading edge off. The wing ripped off due to the high speed fall and it was dangling on the fuselage , fuel inside the wing ignited and created a fireball .
The 3 worst place scenario's you could be in during a plane crash: 1: Being the pilot and co-pilot 2: Outside of the plane crash and not being able to help 3: Being the person who builds the plane/ Maintenance

What's even more haunting is that this accident happened 10 years before the tragic terrorist attacks in New York City, Washington DC, and Pennsylvania. R.I.P. to the 11 passengers and the 3 crew members on board.
When I was young, I always believed that even if you did something on accident that still killed people, I believed it was still murder and they should get arrested. I don't believe that anymore.

The head service tech responsible to oversee and check off that the work had been done correctly, got bored and decided to help which is against regulations. Some how the bolts for holding the top of one of the boots weren't replaced, the head service tech the last one to work on it before he left for shift change. This last flight finally dislodged the boot because the pilot was coming in steep and fast, and achieved a speed much greater than the several flight earlier in the day.

I find it interesting that a small piece of metal coming off the leading edge of the horizontal stabilizer causes this extreme pitch over however there are many examples of planes with much worse damage making successful landings. Such as WW2 B17s coming back with most of the tail missing or huge cuts and holes in the fuselage. or more recent events such as Aloha Airlines 243. plus many others.
