    Nope- I think you are spot on. This situation is really making me re-think my career choice. I am a newer nurse ( 6 months licensed) and this has only put me more on edge. I couldn't imagine what this woman has gone through.

      All you have to do is follow your training and you will be fine. Every patient we care for deserves to be cared for in the same way we would want for ourselves and loved ones.

      But that is THE thing. You miss the point entirely. The poor working conditons and ridiculous staffing ratio makes it impossible to do that kind of nursing. The kind we were all taught. If I had done every single thing required I would have to double or triple the time I had with each patient. I quit because I could not, would not do the type of poor nursing required to half do things in the time allowed. Also trying to supervise all those under me in that situation. My conscience would not allow it. Glad for all who can get out. I was an RN for 40 years. I dont't say it lightly. Also I am facing cancer a second time and will be needing a nurse soon.

      @@glendagrant9042 this particular nurse testified she was not overworked or understaffed that day. So that can not be used as an excuse for her poor performance. I wish you well with your treatments and hope you have nothing but wonderful nurses. God bless you.

    Vanderbilt is a HUGE institution here in Nashville and I’m sure they don’t want the bad press/reputation so they are washing their hands and throwing it all on this poor RN.

      Actually this is going to backfiring on Vanderbilt.
      There is no doubt that nurses will leave and I suspect it will very difficult to recruit experienced nurses as the institution is obviously willing to throw nurses under the bus.

How about having malfunctioning safety systems and instead of fixing them, sending out information to all the nurses on how to bypass the safety system? How about covering it up (which the nurse didn’t do!)

    Vanderbilt is SHAMEFUL in scape goating this RN, their Administrators including safety should be held to the same charges!!! Read the CMS report and noted Vanderbilts ongoing years of systems/software failures. The RN is already serving a lifetime sentence knowing she made a huge error which results in the death of this patient.

    It seems like the hospital wants to make it clear for the public that they are not in fault. I am a RN and I was shocked to hear their risk manager say that they weren't aware neglect can happen in hospital setting, that neglect happens in senior homes.

    She over road the damn Pyxis at least 5 times then failed to see the warning label on the vial. And she never questioned it. That’s pretty damn reckless to me. I am a nurse and I don’t want to work with nurses that mindlessly just give meds. Nurses like that scare me, and I hate working behind them. I’ve seen patients get hurt with fatal med errors before and it’s traumatic for everyone. Report it on another nurse and the facility is after you. JUST READ. That’s all you have to do, is read and question everything. Do your checks. She could’ve prevented the entire situation had she followed the rights of medication administration. I hope this verdict will awaken the nursing community, and go back to basics - the rights of medication administration. Maybe this verdict will push out the nurses just here for the money doing a sloppy job. Vanderbilt this Vanderbilt that. Of course they try to hide stuff, but ultimately she injected that fatal paralytic and as a result the patient died a horrible horrible death. This profession is embarrassing. Yes humans make mistakes but showing pity for her just being outright stupid and not following the protocol… how do you think the patient felt when she was dying? Would you want that to be you or your loved one? Her actions are completely below the standard of care. That’s putting it nicely. She knew what she signed up for when she went to nursing school, YES YOU CAN HURT SOMEONE WITH MEDICATION. So read, pay attention, and do your checks and if you do those things, you shouldn’t worry about how we the govt decides they want to continue with cases like this. No reason to be scared of you practice cautiously.

    Family just made a statement today and they are disgusted by the reaction to the verdict and how they are making the da and the family look like bad guys. They did want the conviction and thought they had closure until everyone reacted horribly to it. Look online please and read their statement

    I'm heartbroken for her :(. I'm heartbroken for all current and future nurses. I have a family of retired nurses and hope to become one soon. This is frightening to say the least.
    If a nurse ever made an innocent mistake and I paid for it with my life, I would not want this for the nurse. I would want education and better procedures and nothing more.

      @@margueritemcbride2034 does that clear a path for her to file a lawsuit? I'm not too familiar with this case but if she disclosed self reporting n the hospital took the steps to cover it up. An error is an error. The guy who's brakes failed in a company truck that I believe was not properly maintained should have been on the hook even more so than him. The same way the hospitals higher ups initiating a cover up should be on the hook as well. It's just further proof of how quickly companies will hang you out to dry.

    Criminal cases are always brought by the government, not victims. It's not up to the family or victim. In most cases the victim or victim's family have no control over what charges are filed, what deals are made, or if the case is prosecuted. Sometimes they are given the option to "not press charges" but that is at the discretion of the prosecutor.

    Here's my two cents I'm done with nursing/medical field been in it for 35 years medic/nurse. I retired last June well forced out. I agree vanderbilt wanted to make a example out of her I ultimately think top bass at all these big corporate hospital can care less about the nurses nurses are expendable just like they are in Ohio particularly the hospital at work. This verdict hurts the profession even move good luck in finding nurses. I believe there should be a nationwide walk out. But it won't happen most nurses are slaved to the system. Shut up take your shot and get back to work . Remember it's the medical industrial complex and they control the system.

    Kim Potter the cop who accidently drew her pistol instead of her taser in in prison right now. That's an exact equivalent to a nurse giving the wrong medication. If you're cool with Kim Potter being locked up and not Radonda, you're a massive hypocrite.

      You know you make a good point. I initially thought that cop should go to jail now I'm reconsidering. What about the engineers that designed that faulty Boeing airplane that crashed killing all those people? Do those engineers need to be locked up? Where does it end?

      @@steeveekeys1904 The nurse was not in a situation where she needed a drug stat. She testified they were not busy at the time and even had a trainee that could have checked the vial label. The hospital and nurse are both at fault and the nurse at minimum should lose her license and be sued in civil court. If you are pulling drugs out of the pyxis/omnicell using override that should be only for an emergency or to prevent an emergency. She broke multiple protocols and had been a nurse for two years with little drug knowledge. She should have found it odd that she had to reconstitute Midazolam since it isn't sold in powder form to US hospitals. I have been a hospital pharmacy tech for years and I restock pyxis/omnicell machines for a living. I am starting nursing school soon and unlike so many nurses in the field now, I don't pull drugs without looking. What people should be doing is harassing the administration at Vanderbilt for running a shitty hospital even though they are snobs.

    I wondered for a while since this, but I will definitely will continue to self report. Now I just need to get NSO and I will safe. A nurse told me not to self report and I lost all respect for her. We are not the crooked car mechanics, this is what sets us apart.

    You know I've always believed to some degree that going through schooling is more of a formality for legal purposes so that they can say well she knew better because she went to school we're as many of us know that most of the things that we know how to do as nurses we actually learned on the job not from a book in nursing school

    THE HOSPITAL IS ALSO LIABLE WITH THIS ….because of their UNSAFETY patient-nurse Ratio that is why this mistake happening ….Most of the nurses are understaffed and they handling too much patient that causes this medication error …I am a registered nurse in a hospital and handling 10 or more patients and it’s totally frustrating …even you don’t want to commit mistake you cannot avoid it because of too much patients , understaffed in the area ,lots of nursing procedure and medication order that you need to finish in some specific time…even eating, drinking or going to Comfort room you cannot do it because of lots of patient that you need to handle at the same time…The worst thing is the HOSPITAL , THE HIGHER UP PEOPLE WHO IN THE POSITION AND YOUR MANAGER are blind with those problem and they will PUT THE BLAME ON YOU ALL….even they knew that their UNSAFETY PATIENT-RATIO IS THE REASON OF IT..

      @@pedinurse1 Exactly, I'm a hospital pharmacy tech starting LPN school soon. I can't believe they had vecuronium in their machines. All of the other hospital groups I worked(I work in TN too) for keep them in surgical trays, pharmacy, and ER machine only. I haven't heard of anyone else besides Vanderbilt doing that in our state. My understanding is everyone on that unit was using override to pull meds. So she used override and didn't even check the vial...

    Vanderbilt has a hx of this sort of behavior. I do question whether the family didn’t want her prosecuted, but I don’t believe it’s up to them but the state. They had a sigh of relief after her conviction and have not made comment but did testify in court. They lost a love one tragically. We don’t know who the anonymous tip was either. The family may be understanding that it wasn’t intended but they did hug and thank the prosecution after the trial ended for their work. It is a tragic situation and with the many overlooked fail safes, I can understand if the family did want her prosecuted. How would any of us feel if it was our loved one. The hospital played a huge role in this yet the only one who has shown integrity is the nurse. I hope the judge is lenient as these types of cases are usually med malpractice suits. I did also read though if it involved gross negligence that a case can then be a criminal one. Idk much about law, so I’m not sure what constitutes gross negligence.

    There you go! I'm an evidence based and informed professional. I have listened to multiple snippets of related presentations and interviews from the DA to what appeared to be an expert witness. The expert witness testimony was outdate and based on perfect circumstances in Perfect Hospitalvile. The DA was quite accusatory that she did not scan the patients bracelet in radiology and stated that there were no reasons for overriding the Pyxis. If that is indeed the case why did this one person's medical record show an inordinate number of overrides in 24 hours. Something is not right. Vaught was only charged after Vanderbilt was called to task for mishandeling the case and to shift the issues away from their bad behavior they reached for the nurse. We've got to look at the upgrading X 3 for Nurse Vaught's case. This sends a loud roar to business and further further undermines one's ability to care for the vulnerable among us. Forcing nurses and other healthcare providers to function on or in constant fear is not the way to improve care. If she has worked as a nurse for 2 yrs (2015 she graduated from school and the fatal error occured in 2017). Her comments to the press/public about how she felt screams of moral distress and now moral injury. How did that happen in a matter of 2 years. Work under bad conditions and I will tell you how it happens. There go I but for the grace of God.

    We cannot change the choices made in the past for Rodonda by speculating on all the outcomes and fallouts which will occur. I believe we need to go forward. I believe we need to relieve the angst of our peers, new nurses and students instead of creating more fear. Let's rely on education to help. We have the tools to prevent a sentinel event. Such as evidenced based protocols and procedures along with patient and med admin safety guidelines. We need to encourage what makes us safe and prevent our downfalls. Let's not teach that workarounds, overrides etc. are okay. Lets not be dismissive or blame others for something if we are not practicing our best selves as nurses.

    It's clear that someone or some group of people want her held responsible. Or else there would not have been an anonymous person contact CMS after 8 months. They clearly found it bad enough to try in a court of law. I think this will cause people to not tell the truth but with electronic records they truth will always come out. I think if her med error had of happened in an emergency situation such as your near miss or in a situation with a horrible patient to nurse ratio she would not be on trial and now a convicted felon. I believe that since she wasn't assigned to any patients and was not in any rush that regular non nursing people and some nursing people are wondering why not just take the time to do it right.

      Well can't necessarily disagree I think her life is destroyed. I nurse last year so many new nurses I told you still need look I know the scanning pretty accurate but look at the goddam meds. Got to stick with the fundamentals I don't care how advance a computer system is advanced.

    I know drug books are on top the pixis, and with the smart phone, this never should have happened. Hind sight wait for pharmacy to fill the order. I probably spend 15 percent of my time calling pharmacy, and some unresolved issues I spend days.

      Most of the time the pharmD is waiting on the physician to enter or correct orders. I had a nurse call me for a stat bolus I made an hour earlier and she was actually holding it her other hand while telling me I never sent it through the tubes.

    Vanderbilt is not necessarily an ethical organization when it throws a single employee under the bus for a system error. Add to that, our wonderful Davidson county liberal DA, Glenn Funk, who sought to prosecute her. He is also a professor at Vanderbilt! Go figure. This setback our work on patient safety and just culture a good 20 to 30 years. Nashville is Davidson county if you need to look it up. Funk needs to go!

    I feel she does NOT deserve to be in jail! She didn’t even try to hide her mistake!!! Prosecutors make mistakes all the time in not going after rapists, serious criminals…why are they not prosecuted? 🤷🏻‍♀️ He are human!
    Putting her license in hold for a certain year amount and having her to do remedial nursing classes during this time and doing community service…I feel would be enough punishment. Especially, since the family does not want her prosecuted/jailed. 🤷🏻‍♀️

    Breaks my heart. She told the truth with that mistake. But the gay guy, I'm lesbian, that lied & upset other people that looks up to him. Then he has to go to jail for like ?180?. Then he was able to leave the jail place. I hope that if she has to go to jail or prison. She should be let go less then a week.     I've worked as an RN for almost 40 years. This whole thing is terrifying. I don't think I could confess to a mistake if it meant going to prison.

      The whole floor apparently was using override to get into the pyxis/omnicell...she used override in a machine that stocks anesthesia vials and apparently didn't check the vial and didn't stay to monitor after pushing what she thought was midazolam. If that is correct then Vanderbilt is a shit hospital and she did cause this death from total neglect. Vanderbilt could be using her as a scapegoat but she was terrible at her job and killed her patient.

    This is SO wrong. Where are her supervisors and upper management?? why are they not coming out and standing up for her? I really hope she knows that SO many people know this is wrong and unfair and it could easily be someone else (like me!)

      she didn't read the label and injected someone with a drug she had to prepare in many steps. Would you do that to yourself? No. But she did that to someone else who died a horrible death because of it. There is no excuse for this.

    It’s probably a DA that’s wanting to make a name for themself by “holding bad nurses accountable.” I’ve also heard that this DA took on a different case that most others would not take on.

    If you work in Vanderbilt health care, you should leave. Everyone who works in healthcare knows, the hospitals and administrators push healthcare providers beyond a safe level to increase their profits.

      So when are we prosecuting physicians who remove the wrong limb? Misdiagnose or not diagnose a fatal illness? This is NOT a criminal act. The IOM did a whole study called “To Err is Human” and promoted fail safes (double checks, technology, etc) to try to stop human error. VANDERBILT failed to have those in place and properly functioning. This will set healthcare back so much further and everyone will pay when they seek healthcare.

      @@luismonzon1713 show me a doctor who isn’t covered under malpractice and has been arrested for making a mistake! I’ll wait…he has an entire valid point because they’d never prosecute a doctor and Vanderbilt covered this up…SHE admitted her mistake. Sad? Absolutely. Was she human and overworked, understaffed? Yes! You have no point because mistakes happen, no nurse is perfect!

    It's heartbreaking and sad for her.. And if the family is more compassionate than the state it's because the place she worked at don't want to have a bad reputation 🤔. She made a mistake yes but she should continue to work as a nurse but not pass meds again ever. She is remorseful she can't go back in time.

    Money money money. Demonize the RN in the media and we will still get customers. Vanderbilt: “Look what we did to this horrible nurse that we had under our belt. All her fault. Don’t worry she’s gone. This will never happen again.” Sent an innocent soul to jail as if she was a threat to society. The protective veil needs to be removed from Vanderbilt’s image because we as healthcare professionals see it…. But the public doesn’t!

    Families do not prosecute, it is the district attorney who brings charges and prosecutes. . No family or private citizen has or should have the ability to decide that because there is too much emotion involved for one thing. If some one commits a crime and this nurse DID, there should be a consequence for what the criminal did and yes she is now a criminal. I hope she gets prison time because she took someone else's life through her CHOICE to disregard ALL of the safeguards that were in place to protect the patient and to protect even this idiot nurse from herself. There is no way to sugar coat the blatant irresponsibility of this nurse. I think this case should be held up as an example for all nurses to NOT GET complacent in our jobs. All she had to do was to follow the 5 rights of med administration and she would not have made this catastrophic error. She deprived this patient of the rest of her life, she deserves to loose some of hers. Once her sentence is served she can be released from prison and get on with her life, something the patient she killed will never get to do....

    It doesn't matter liz nurses are sanction by the state. Board of nursing is not your friend. There There to protect the public from you hence meaning the nurses. When gravity of the situation out the Board of nursing in Tennessee looked like incompetent buffoons. The Board of nursing had to take harsh action and so did the state. Weather people like it or not you new nursing better remember them by heart. Also it is important to remember nurses are usually the last safety. I know it's had to accept she the nurse Rhonda fail all the safety checks prior to administering the drug. It's a cruel world out there nurses can be outrage all they but she made too many fatal mistake. It's lonely on a island all by yourself.

      Didn't they protect the public by stripping her license years ago. I'm unfamiliar with any state boards filing lawsuits after they have denied someone's ability to practice.

    This case has gotten an exorbitant amount of press and social media with knowing all the facts. I think that if we were to take the time to check out how many other cases of nurses or healthcare providers being criminal charged we might be surprised. I also think we need to leave the families decisions out of this. They were most likely not provided information or made decisions based on what others i.e. lawyers, hospital advisors etc. led them to believe what was best. We do not know or may never know all the details about Vanderbilt. Believe me they are not the first health care system to have done wrong. Rodonda made terribly wrong decision by choosing not to follow med admin and safe practice guidelines. She choose to ignore safe practice. We cannot blame a health care system or machines for that. I do not think we will ever know why really. There had to be a lack of knowledge on her part, that is why this was not malicious. Never would I blame administration for my choices if I ignored the tools to prevent such a devastating outcome.

    I do hope she goes to jail, I’ve seen many medicine workers getting away with killing “making a mistake” and now they can think twice and do their work as they should. Ofc the company is main culprit here to cover this bs, but I want to see justice 🙃

Another Nurse confronted her with the evidense, Vaught knew she was caught and admitted she gave it. She also didn't chart she gave it. She wouldn't have said anything if the other Nurse didn't speak up. How many patients had she harmed that we will never know about?

    I hope that this case needs to be appealed to a higher court. I think that Vanderbilt and the prosecutor combined to shift the blame from them to a single person.

      It means - do your rights of medication administration and question orders if they don’t seem right. If you do that you don’t have anything to worry about. DeRonda did the EXACT opposite of what you are supposed to do when pulling medication.

      I agree with Misses_Munster. You are given tools of evidence based, best practice, patient safety and med admin guidelines so that you can be a competent ethical best nurse you can be. You will make mistakes we all have but not to this degree. If feel something is not right or your uncomfortable go over all the steps again before doing something. After 30 yrs of nursing I still question my self, my abilities, my lack of knowledge, my peers opinions, and ask questions. As a student nurse be aware of in clinical if the nurse is teaching correctly or not following protocol. Do not allow them to talk you into something that is not with in your realm of knowledge. Go to your instructor to question situation. Be brave, use the knowledge, tools and guidelines for best practice. Rodonda did not and I believe part (only a minor) of this was a lack of knowledge, fear to ask questions, and not knowing her own limitations.

      Wow. Just cover it up eh? She literally killed someone by paralyzing their breathing and the person consciously felt themselves suffocate to death and you are just like - "We need to cover this up!" Disgusting!

      That exactly is the question that should be answered publicly....would appear the motive was to expose VUMC's failure to follow the law and report the error. It got terribly twisted into saving the institution by putting the nurse who promptly acknowledged the error, reported the error and was investigated by the license board that found no action was deemed appropriate. Corruption is rampant here....could VUMC's president be risking a $4.3M annual salary?

      If you are replying to me then no this should never have been covered up.
      Maybe you should ask that question to Vanderbilt administration as well as the nursing board that said there was no criminal intent involving the nurse, but when Medicare and money became involved she was arrested.
      Critical thinking skills should let you understand this was a cover up to protect Vanderbilt.

Critical thinking should make you realize that she wasn't overworked, she was the last line of defense for the patient and she failed. She injected something into someone that killed them horribly... and you are here trying to say it is ok because the the hospital system blah blah blah. No. Gotta draw a line somewhere... you don't seem to think there is a need to read bottles before injecting someone and if you are a nurse, you should quit.
